Lyme Disease Patient Trajectories Learned from Electronic Medical Data for Stratification of Disease Risk and Therapeutic Response.

Lyme disease (LD) is the most common tick-borne illness in the United States. Although appropriate antibiotic treatment is effective for most cases, up to 20% of patients develop post-treatment Lyme disease syndrome (PTLDS). There is an urgent need to improve clinical management of LD using precise understanding of disease and patient stratification. We applied machine-learning to electronic medical records to better characterize the heterogeneity of LD and developed predictive models for identifying medications that are associated with risks of subsequent comorbidities. For broad disease cate

Increased sensorimotor network connectivity associated with clozapine eligibility in people with schizophrenia.

Schizophrenia is a heterogeneous disorder that exhibits variable responsiveness to treatment between individuals. Here we conducted a resting-state functional magnetic resonance imaging (rs-fMRI) study to determine whether resistance to first-line antipsychotics is reflected in resting-state connectivity. rs-fMRI data were collected from 15 people who had failed to respond to first-line antipsychotics (clozapine-eligible) and 10 first-line treatment responders (FLR). Image pre-processing and analysis were performed using FMRIB's software library (FSL). Data was decomposed into spatial and temp

Current advances in the diagnosis and treatment of nonerosive reflux disease.

Nonerosive reflux disease (NERD) is a distinct pattern of gastroesophageal reflux disease (GERD). It is defined as a subcategory of GERD characterized by troublesome reflux-related symptoms in the absence of esophageal mucosal erosions/breaks at conventional endoscopy. In clinical practice, patients with reflux symptoms and negative endoscopic findings are markedly heterogeneous. The potential explanations for the symptom generation in NERD include microscopic inflammation, visceral hypersensitivity (stress and sleep), and sustained esophageal contractions. The use of 24-hour esophageal impeda

Renal involvement in autoimmune connective tissue diseases.

Connective tissue diseases (CTDs) are a heterogeneous group of disorders that share certain clinical presentations and a disturbed immunoregulation, leading to autoantibody production. Subclinical or overt renal manifestations are frequently observed and complicate the clinical course of CTDs. Alterations of kidney function in Sjögren syndrome, systemic scleroderma (SSc), auto-immune myopathies (dermatomyositis and polymyositis), systemic lupus erythematosus (SLE), antiphospholipid syndrome nephropathy (APSN) as well as rheumatoid arthritis (RA) are frequently present and physicians should be

[Treatment of depression secondary to psychotic disorders].

More than 25% patients suffering from psychotic disorders would show during their evolution some depressive disorders. More than 10% would die from suicide. Secondary depressions following psychotic disorders are heterogeneous. They have to be specified in order to assess therapeutics including biological psychological and socio-environmental approaches. Treatment of depressive disorders correlated to psychotic symptomatology, being mood congruent or not, is the same as characterized depression. However it is preferred to associate antidepressant and neuroleptic or electroconvulsivotherapy. De

Toxicities associated with CAR-T therapy compared with standard treatments in hematologic malignancies: a systematic review and meta-analysis.

BACKGROUND: Chimeric antigen receptor T-cell (CAR-T) treatment has revolutionized the therapeutic approach for relapsed or refractory hematologic malignancies. But its unique toxicity profile, particularly immune-mediated adverse events, is a significant question in comparison with other high-intensity therapy strategies. Systematic, comparative, specific assessment of CAR-T–associated toxicities are essential to guide clinical practice and institutional preparedness. METHODS: We conducted a systematic review and meta-analysis comparing the incidence and management of significant toxicities as

Single fixed-dose intranasal racemic ketamine treatment for the treatment of acute suicidality in a transdiagnostic patient population: Results of a pilot study.

AIMS: Ketamine has been shown to decrease suicidality in patients with depression. However, little is known about tolerability and efficacy in heterogenous, acutely suicidal patients in emergency settings. This pilot study aimed to assess the feasibility, acceptability, and safety of generic intranasal racemic ketamine (75 mg) in the treatment of acute suicidality regardless of the underlying diagnosis, as preparation for a subsequent double-blind randomized placebo-controlled trial (RCT). METHOD: From September 2021 to June 2022, 12 patients with acute suicidality were recruited. All receive

Visual and Refractive Outcomes of Corneal Refractive Surgery for Hyperopic and Mixed Astigmatism: A Systematic Review and Meta-Analysis.

TOPIC: To evaluate the visual and refractive outcomes of corneal refractive surgery (PRK, LASIK, and SMILE) for hyperopic and mixed astigmatism. The clinical question addressed was: in adults with hyperopic or mixed astigmatism, how effective and safe are modern corneal refractive procedures, without alternative surgical comparators, in reducing refractive cylinder and improving uncorrected distance visual acuity? CLINICAL RELEVANCE: Hyperopic and mixed astigmatism are less predictable to correct than myopic errors due to biomechanical and ablation-profile complexity. Historically, these trea

Safety and Efficacy of Different Therapeutic Interventions for Primary Progressive Aphasia: A Systematic Review.

Background: Primary progressive aphasia (PPA) is a neurodegenerative disorder that worsens over time without appropriate treatment. Although referral to a speech and language pathologist is essential for diagnosing language deficits and developing effective treatment plans, there is no scientific consensus regarding the most effective treatment. Thus, our study aims to assess the efficacy and safety of various therapeutic interventions for PPA. Methods: Google Scholar, PubMed, Web of Science, and the Cochrane Library databases were systematically searched to identify articles assessing differe

Behavioural modification interventions for medically unexplained symptoms in primary care: systematic reviews and economic evaluation.

BACKGROUND: The term 'medically unexplained symptoms' is used to cover a wide range of persistent bodily complaints for which adequate examination and appropriate investigations do not reveal sufficiently explanatory structural or other specified pathologies. A wide range of interventions may be delivered to patients presenting with medically unexplained symptoms in primary care. Many of these therapies aim to change the behaviours of the individual who may have worsening symptoms. OBJECTIVES: An evidence synthesis to determine the clinical effectiveness and cost-effectiveness of behavioural

Integrative Approaches to Atopic Dermatitis: Exploring Biomarkers, Current Therapies, and the Potential Role of Osteopathic Manipulative Medicine.

Atopic dermatitis (AD) is a chronic, relapsing, and remitting condition that affects approximately 10% of the population. It is a multifactorial disease involving immune dysregulation, gene mutations, and environmental factors that ultimately disrupt skin barrier restoration, causing pruritic lesions. Given the marked heterogeneity of AD and the presence of multiple immune and clinical endotypes, no single therapeutic strategy is likely to be universally effective. Instead, a growing panel of distinct biomarkers offers an avenue for true precision medicine by enabling more accurate endotype cl

Increased anergic B cells and decreased antibody-secreting cells during romiplostim treatment in patients with immune thrombocytopenia.

Patients with immune thrombocytopenia (ITP) exhibit substantial heterogeneity in treatment responses. Approximately 30%-50% of patients maintain remission after discontinuation of thrombopoietin receptor agonists (TPO-RA), suggesting immunomodulatory effects beyond enhanced thrombopoiesis. However, predictive or explanatory biomarkers for sustained remission after TPO-RA are currently lacking. In this study, we performed spectral flow cytometry on B cells of longitudinal samples from patients before, during and after treatment with the TPO-RA romiplostim. We observed that anergic B cells were

Integrating alternative therapies in overcoming chemotherapy resistance in tumors.

Chemotherapy-resistant tumors present a significant challenge in oncology, often leading to treatment failures owing to mechanisms such as genetic mutations, drug efflux, altered metabolism, and adaptations within the tumor microenvironment. These factors limit the effectiveness of treatment and contribute to tumor resistance. This review highlights the role of alternative therapies aimed at overcoming resistance mechanisms. Several alternative strategies with high efficacy rate against tumor resistance are being explored, including targeted therapies (58-64%), immunotherapy (80%), hormone the

Recent advances and future directions in newly diagnosed mantle cell lymphoma.

INTRODUCTION: There have been recent major advances in the management and treatment of mantle cell lymphoma (MCL). This uncommon subtype of mature B-cell lymphoma has a heterogeneous clinical course, including a spectrum of indolent and aggressive disease. While historically regarded as an incurable disease with a poor long-term prognosis, recent developments have improved outcomes. AREAS COVERED: The incorporation of targeted treatments, such as covalent Bruton's tyrosine kinase inhibitors (cBTKi), with or without chemo-immunotherapy in the upfront treatment setting is supported by recent cl
