Interventions to reduce tuberculosis mortality and transmission in low- and middle-income countries.

Tuberculosis is among the top ten causes of global mortality and affects low-income countries in particular. This paper examines, through a literature review, the impact of tuberculosis control measures on tuberculosis mortality and transmission, and constraints to scaling-up. It also provides estimates of the effectiveness of various interventions using a model proposed by Styblo. It concludes that treatment of smear-positive tuberculosis using the WHO directly observed treatment, short-course (DOTS) strategy has by far the highest impact. While BCG immunization reduces childhood tuberculosis

Intensive tuberculosis screening for HIV-infected patients starting antiretroviral therapy in Durban, South Africa.

BACKGROUND: The World Health Organization (WHO) recommends cough as the trigger for tuberculosis screening in human immunodeficiency virus (HIV)-infected patients, with acid-fast bacillus (AFB) smear as the initial diagnostic test. Our objective was to assess the yield and cost of a more intensive tuberculosis screening in HIV-infected patients starting antiretroviral therapy (ART) in Durban, South Africa. METHODS: We prospectively enrolled adults, regardless of tuberculosis signs/symptoms, who were undergoing ART training from May 2007 to May 2008. After the symptom screen, patients expector

Computerized tomography findings of abdominal tuberculosis: report of 19 cases.

BACKGROUND: Abdominal tuberculosis usually presents with general symptoms and obscure abdominal complaints for which computerized tomography is often the first imaging study. OBJECTIVE: To evaluate the CT findings of abdominal tuberculosis. METHODS: The CT scans of 19 patients (10 men and 9 women aged 20-85 years) with proven abdominal tuberculosis were retrospectively reviewed to define the location and extent of the disease. The patients were referred for the study mainly with general systemic symptoms. Additional abdominal complaints were present in four, including acute abdomen in one. T

Community-acquired pneumonia mimicking tuberculosis in a patient with Down syndrome treated with adjunctive injectable corticosteroids.

Community-acquired pneumonia (CAP) is a frequent cause of morbidity in individuals with Down syndrome (DS) due to their inherent immunological vulnerabilities and anatomical predispositions. We present the case of a young woman in late adolescence with DS who developed severe CAP with radiological findings mimicking pulmonary tuberculosis. Despite initial diagnostic uncertainty, the patient improved remarkably following initiation of antibiotics combined with injectable corticosteroids. This case highlights both the diagnostic challenge of distinguishing CAP from tuberculosis in high-risk popu

Severe tubulointerstitial nephritis: tracking tuberculosis even in the absence of renal granuloma.

Extra-pulmonary tuberculosis is frequently located in the kidneys and, in such cases, could be associated with a granulomatous interstitial nephritis. Granulomas are not always detected, especially in human immunodeficiency virus (HIV)-positive patients. We report here a case of tubulointerstitial nephritis without granulomas in an HIV-negative patient. Since all laboratory tests failed to isolate Mycobacterium tuberculosis in the kidney, a targeted biopsy guided by positron emission tomography-computed tomography was performed on a mediastinal node, revealing a positive culture. After 6 month

Vaccines against tuberculosis: A review.

Tuberculosis (TB) has taken toll of many lives, therefore a need of effective TB vaccine, which can provide sufficient immunity to prevent developing of disease has been felt for a longer time. BCG, the only available vaccine, though prevents against severe form of primary tuberculosis in paediatric population, failed to have its efficacy in pulmonary patients. Few candidates are in the pipeline undergoing clinical trial. An extensive research is needed to ensure their safety and efficacy before their acceptance as a TB vaccine to be incorporated in national immunization programmes.

Extensive Inflammatory Gingival Tumor in a Young Nonsmoking Woman: Back to Basics.

Tuberculous lesions of the oral cavity are rare and can be a diagnostic challenge, particularly in young immunocompetent patients. Most of the cases in the literature are secondary to pulmonary disease, whereas primary form is uncommon. This paper presents a case of gingival tuberculosis in a 26-year-old Indian female patient, manifesting as a rapidly extensive ulcer. The diagnosis was confirmed by histopathology and immunological investigations. Although oral manifestations of tuberculosis are rare, clinicians should include them in the differential diagnosis of various types of oral ulcers.

Endocardial tuberculosis.

We report a 30-year-old male patient with persistent fever, history of stroke, and a left atrial mass. He was diagnosed as miliary pulmonary tuberculosis radiologically and had fever despite 2 months of antitubercular treatment. The mass was excised and fever resolved. Acid fast bacilli (AFB) were demonstrable on Ziehl Neelsen stain and routine histopathology. To the best of our knowledge, the visualization of AFB from an intracardiac lesion on ZN staining has not been reported earlier and tuberculosis must be considered in the differential diagnosis of left atrial masses.

[Adenosine deaminase in the blood cells and serum of patients with tuberculous pleurisy].

The common complication of pulmonary tuberculosis is pleurisy that requires not only special treatments, but also gives rise to difficulties in the differential diagnosis of tuberculosis. The investigation has studied the possibility of determining the activity of adenosine deaminase (ADA) as a marker of pleural effusion of tuberculous etiology. The patients with tuberculous pleurisy have been found to have higher lymphocytic and red blood ADA, and ADA, activities and a significantly increased serum ADA2 activity. Thus, the changes in the activity of ADA and its isozymes have been shown to be

Persistent mediastinal and axillary lymph node tuberculosis in a renal transplant patient with successful outcome.

Tuberculosis is an opportunistic infection that carries substantial morbidity and mortality in renal transplant recipients. We report here about a 21 year-old man with a living related renal transplant from his mother who developed persistent extra-pulmonary tuberculosis. The disease showed aggressive invasion to the axillary and mediastinal regions with abscess formations, despite standard antituberculosis treatment. During the course of the disease, immunosuppressive therapy was stopped, and the patient received extraordinary doses of multiple antituberculosis drugs. The patient then showed

Mediastinal lymphadenopathy due to mycobacterial infection.

Tuberculous lymphadenitis without pulmonary manifestation is an uncommon entity in developed countries, and the possibility of tuberculous infection is usually ignored in the differential diagnosis of lymphadenopathy. Therefore, appropriate treatment may be delayed. Paralysis of the recurrent laryngeal nerve caused by mediastinal lymphadenopathy due to tuberculosis is an extremely rare condition. In this paper, we present a patient who had vocal cord paralysis caused by tuberculous lymphadenopathy of the superior mediastinum. After anti-tuberculosis treatment, vocal cord function was only part

[Female genital tuberculosis, a changing reality].

From 1964 to 1979 we diagnosed 185 new cases of female genital tuberculosis. We report the used diagnostic methods and the changes observed in this time relating to age distribution, subjective symptoms, previous and present specific lesions, clinical and anatomo-pathologic manifestations. We show either the therapeutic schedule, including rifampin, isoniazid and ethambutol, or the surgical treatment indications. With evolution and therapy of pulmonary manifestations, we debate the decrease in genital forms of extrapulmonary tuberculosis and, within the limits of that, the pelviperitonitis dis

Tuberculosis in the Context of Idiopathic CD4 Lymphocytopenia: A Case Report and Literature Review.

BACKGROUND Idiopathic CD4 lymphocytopenia is a rare immunodeficiency syndrome characterized by a persistent reduction in CD4 lymphocyte count to fewer than 300 cells/µL or less than 20% of total T lymphocytes on multiple occasions. This diagnosis is made in the absence of HIV infection or other known immunodeficiencies. Herein, we present a case report of idiopathic CD4 lymphocytopenia in an adult patient, contributing to the limited literature on the management challenges of this rare immunodeficiency. CASE REPORT A 42-year-old male patient presented with shortness of breath, productive cough

Tuberculosis in Brazil and cash transfer programs: A longitudinal database study of the effect of cash transfer on cure rates.

INTRODUCTION: Tuberculosis incidence is disproportionately high among people in poverty. Cash transfer programs have become an important strategy in Brazil fight inequalities as part of comprehensive poverty alleviation policies. This study was aimed at assessing the effect of being a beneficiary of a governmental cash transfer program on tuberculosis (TB) treatment cure rates. METHODS: We conducted a longitudinal database study including people ≥18 years old with confirmed incident TB in Brazil in 2015. We treated missing data with multiple imputation. Poisson regression models with robust v

Limitation of Deleterious iNOS-Expressing Interstitial Macrophages Is Dependent on Protective IL-1α and IL-17 in Tuberculosis and Existing Obesity.

IL-1α, IL-1β, IFN-γ, IL-17 and the expression of inducible nitric oxide synthase (iNOS) enzyme by macrophages, which generate nitric oxide, are protective against tuberculosis, a disease caused by Mycobacterium tuberculosis. The severity of tuberculosis can be dependent on bacterial load and/or lung immunopathology. Tuberculosis is aggravated by existing comorbidities such as obesity. Obesity induces metabolic dysfunction, meta inflammation and dysbiosis, which increase the prevalence of respiratory diseases and worsen immunopathology. The deficiency of IL-1α (IL-1α-/-) induces deleterious lun

Microarchitecture of historic bone samples with tuberculosis.

Tuberculosis is among the leading causes of death from infectious diseases and affects many organ systems, including the skeleton. Skeletal tuberculosis is an extrapulmonary stage of tuberculosis, which occurs after the early and post-primary pulmonary stages of the disease. The aim of our study was to assess the microarchitecture of historic dry bone samples of subjects who have died of tuberculosis documented by post-mortem examinations. These preparations date to the pre-antibiotic era, and were provided by the Pathological-Anatomical Collection in the "Fools Tower" of the Natural History M
