SUBJECTS AND METHODS: Retrospective review of a series of 60 qualified cases of congenital/infantile esotropia surgically binocularly "aligned" (= orthotropia +/- 8 PD) before 30 months of age and showing at one postoperative examination date at least some degree of (subnormal) binocular vision. Every patient was monitored over a very long postoperative followup period (mean 14.7 +/- 3.7 years).

BACKGROUND: Short Wavelength Automated Perimetry (SWAP) uses a two-color increment threshold procedure (blue-on-yellow) to assess the functional status of short-wavelength-sensitive (SWS) mechanisms. Reports that brain lesions may differentially affect global and local stereopsis suggest that anatomically separate mechanisms underlie these two functions. We demonstrate here that adaptation to global stereograms induces an after-effect on local stereopsis as well as on global stereopsis, showing that the two mechanisms cannot be entirely separate. We suggest that global stereopsis depends upon an interaction between primary visual area V1 and higher brain areas such as inferotemporal cortex.

Using cycloplegia, the change in ametropia of 113 infants was followed at 3 month intervals over the first year of life. Scatterplots of the spherical equivalent power show that the dioptric differences exhibit a significant myopic shift of -0.38 ds between 26 and 36 weeks and -0.38 ds between 36 and 52 weeks. Atropine autoinjectors are issued to aviators for use in the event of organophosphate poisoning on the battlefield. This investigation assessed the effects of unchallenged 2 mg and 4 mg doses on flight performance, vision, tracking, cognitive performance, and electroencephalograms of 12 Army aviators. A masking paradigm was used to measure the spatial frequency and orientation tuning of spatial mechanisms in the albino visual system. Threshold elevation curves obtained in this manner at test spatial frequencies of 0.25 cycles/deg (cpd), 0.50 cpd, and 1 cpd have the same shape as curves obtained from normal subjects at test frequencies two octaves higher. We performed corneal transplantation in nine microphthalmic eyes of five patients, who ranged in age from 1 to 63 months. The anteroposterior diameter of the eyes ranged from 12.5 to 16 mm. Complications from the surgery included superficial epithelial erosions in three eyes, leading to superficial scarring in one eye, glaucoma, and suture microabscesses. Despite these complications, seven of nine grafts have remained clear and vision developed in all eyes.

Using infra-red oculography, electro-oculography and fundus video-recordings, waveform characteristics (amplitude, frequency, waveform shape and foveation) were examined in over 150 individuals with congenital nystagmus. For many of the subjects the nystagmus exhibited marked variability in both space and time. The sources of this variability were explored and the state of attention of the observer was found to be a dominant factor. The waveform and precision of foveation were not found to be related to any one classification group (eg. albino or idiopathic).

Temporal contrast sensitivity was measured in patients with optic neuritis and anisometropic amblyopia. In optic neuritis, attenuated sensitivity was detected at all frequency ranges; however, its degree was more severe than that of retinal diseases even when visual acuity was the same. A retrospective review of the records of 114 subjects with accommodative dysfunction has been completed. Most subjects (N = 96) were found to have accommodative insufficiency.
