Work stress, dietary patterns, and physical activity on metabolic and hepatic biomarkers among healthcare professionals in Ghana.

OBJECTIVE: This study investigated the association between lifestyle exposures (work stress, dietary patterns, and physical activity) and metabolic and hepatic biomarkers among healthcare professionals in Ghana. METHODS: A cross-sectional survey was conducted among 119 participants at the Kibi Government Hospital. The survey included demographic and lifestyle questionnaires, work stress assessments, anthropometric measurements, and biochemical analyses. Metabolic syndrome (MetS) was defined according to the International Diabetes Federation criteria. Binary logistic regression models were app

Scrotal Migration of the Ventriculoperitoneal Shunt in a 1-Year-Old Pediatric Patient: A Case Report and Systematic Literature Review.

Background: Migration of the peritoneal end of the ventriculoperitoneal shunt (VPS) into the scrotum is a rare but recognized complication. Inguinoscrotal migration typically occurs as a result of increased intra-abdominal pressure combined with a patent processus vaginalis. A 14-month-old pediatric patient presented to the emergency department with abdominal pain, vomiting, and swelling of the right scrotum that had persisted for two days. The patient had a history of a head injury that had resulted in a large secondary arachnoid cyst for which a VPS had been placed at eight months of age. Ex

Tranexamic Acid for reduction of intra- and postoperative TRansfusion requirements in elective Abdominal surgery (TATRA): study protocol for an investigator-initiated, multicenter, double-blind, placebo-controlled, randomized superiority trial with two parallel groups.

BACKGROUND: Intra- and postoperative hemorrhage is a relevant problem in major abdominal surgery, leading to acute anemia and necessitating transfusion of packed red blood cells. It is estimated that in 30% of abdominal surgeries, intra- or postoperative transfusion is required. Transfusion potentially has detrimental health effects and poses a considerable socioeconomic burden. Tranexamic acid, a lysine analog inhibiting plasminogen activation and providing clot stability, has been used to reduce hemorrhage. While there is ample evidence in other surgical disciplines, it is almost completely

CT scans for pediatric injury in a middle-income country trauma center: Are we repeating past mistakes?

PURPOSE: Sustained efforts in high-income countries have decreased the rate of unnecessary computed tomography (CT) among children, aiming to minimize radiation exposure. There are little data regarding CT use for pediatric trauma in low- and middle-income countries. We aimed to assess the pattern and utility of CT performed during evaluation of trauma patients presenting to a middle-income country (MIC) trauma center. METHODS: We reviewed pediatric (age<18) trauma admissions at a single tertiary referral center in South Africa. Patient demographics, injury details, surgical intervention(s),

Disulfide high mobility group box-1 causes bladder pain through bladder Toll-like receptor 4.

BACKGROUND: Bladder pain is a prominent symptom in several urological conditions (e.g. infection, painful bladder syndrome/interstitial cystitis, cancer). Understanding the mechanism of bladder pain is important, particularly when the pain is not accompanied by bladder pathology. Stimulation of protease activated receptor 4 (PAR4) in the urothelium results in bladder pain through release of urothelial high mobility group box-1 (HMGB1). HGMB1 has two functionally active redox states (disulfide and all-thiol) and it is not known which form elicits bladder pain. Therefore, we investigated whether

Sequential pancreatitis and diverticulitis following elective anterior lumbar disc replacement: a case report of a rare postoperative domino effect.

INTRODUCTION: Anterior lumbar disc replacement (LDR) is a motion-preserving alternative to fusion for degenerative disc disease but carries risk of vascular and visceral injury. We report the first documented case of a sequential "domino effect" of acute postoperative pancreatitis followed by perforated diverticulitis after LDR surgery. CASE PRESENTATION: A 66-year-old man with diabetes underwent elective L2-L3 LDR via a left anterior retroperitoneal approach. Although initially stable, he developed acute pancreatitis on postoperative day (POD) 1, confirmed by elevated enzymes and CT findings

A Rare Presentation of Native Aortic Valve Endocarditis Secondary to Streptococcus gordonii: A Case Report.

Streptococcus gordonii is an alpha-hemolytic Gram-positive bacterium most commonly present in the oral cavity and gastrointestinal tract. Despite an increase in the incidence of infective endocarditis, S. gordonii cases reported on a native aortic valve are relatively uncommon. Reported cases have been linked to spondylodiskitis, intra-abdominal infection, and oral procedures. A 79-year-old male was admitted for acute kidney injury, diarrhea, and dehydration, and was found to have S. gordonii bacteremia with endocarditis. A 1.7 cm × 0.9 cm vegetation on the native aortic valve was revealed, an

A Massive Type IV Hiatal Hernia With Gastric Volvulus and the Complicated Road to Recovery.

Type IV paraesophageal hiatal hernias are rare but carry a significant risk of life-threatening complications, including gastric volvulus, incarceration, and the involvement of other intra-abdominal organs. Pancreatic herniation and associated pancreatitis are exceedingly uncommon. We report the case of a 74-year-old woman with Barrett's esophagus, chronic obstructive pulmonary disease (COPD), and obesity who presented with nausea, vomiting, and abdominal pain after a recent increase in the dose of tirzepatide. Imaging demonstrated a massive type IV paraesophageal hernia with organoaxial gastr

Beyond Hemolysis: Babesiosis-Associated Atraumatic Splenic Infarction in an Immunocompetent Patient.

Babesiosis is a protozoan infection transmitted by the tick, Ixodes scapularis. The disease presentation varies from asymptomatic to fatal illness. Usually, severe illness occurs in asplenic and immunocompromised patients. In rare cases, the complication of splenic injury, including infarcts and rupture, has been observed in young individuals. We present the case of a middle-aged woman who presented with abdominal pain and fatigue. She was found to be febrile and hypotensive with thrombocytopenia, hemolytic anemia, and elevated transaminases. Imaging of the abdomen demonstrated multiple spleni

Resistivity and Pulsatility Indexes Renal Values Do Not Differ When Obtained from Abdominal or Transrectal Approaches in Healthy Horses.

Early detection of renal deficient function is imperative for the success of treatments and determining prognosis. In horses, a renal ultrasound can be performed through the abdominal wall (TA) or transrectally (TR), being an important tool for the evaluation of the kidneys. Besides renal morphology, resistivity (RI) and pulsatility indexes (PI) may be early tools to detect renal injury as they reflect arterial compliance. Twenty-nine healthy adult horses were sonographically evaluated by TA and TR approaches, measuring RI and PI on both kidneys. The mean values from the TA for RI and PI in th

Ultrasound-based abdominal muscles and diaphragm assessment in predicting extubation failure in patients requiring neurointensive care: a single-center observational study.

Extubation failure rates are notably high in patients in neurointensive care. Ineffective cough is the variable independently associated with extubation failure, but its quantification remains challenging. Patients with primary central nervous system injury requiring invasive mechanical ventilation were included. After a successful spontaneous breathing trial (SBT), abdominal muscles and diaphragm ultrasound were performed under tidal breathing and coughing. 98 patients were initially recruited for the study, and 40 patients were ultimately included in the final analysis. Extubation failure oc

Attempted Suicide by Cerbera odollam Ingestion in Switzerland: A Case Report.

We report the case of a 37-year-old female patient admitted to our hospital with unstable bradyarrhythmia, abdominal cramping, vomiting and visual disturbances including xanthopsia and diplopia. Ten hours prior to admission, she reported ingestion of four seeds of Cerbera odollam and four crushed nutmegs with suicidal intent. The patient was hypotensive and suffered from hyperkalemia (serum potassium > 5.5 mmol/L), leucocytosis and acute kidney injury. Her heart rate ranged between 28 and 40 bpm, and electrocardiogram (ECG) revealed a second-degree atrioventricular (AV) block and ST-depression

Acute pancreatic panniculitis in a domestic ferret (Mustela putorius furo): diagnostic insights and a potential drug-induced etiology.

Panniculitis is a rare condition in ferrets (Mustela putorius furo), previously linked to nutritional deficiencies, infections, trauma, and injections. Pancreatic panniculitis, caused by the systemic release of pancreatic enzymes during pancreatic injury, has been documented in humans and other animal species, but not in ferrets. This report describes the first known case in a domestic ferret. A 2-year-old male ferret presented with ulcerated cutaneous nodules predominantly affecting the hind limbs and inguinal abdominal region. The ferret had been treated with rifampicin and clarithromycin fo

A Rare Case of Non-cirrhotic Hepatocellular Carcinoma in a Female Adolescent: Diagnostic and Multimodal Treatment Challenges in a Low-Resource Setting.

Hepatocellular carcinoma (HCC) is rare in children and adolescents, particularly in non-cirrhotic livers. Pediatric HCC often presents with aggressive behaviour and requires individualized, multidisciplinary care. Liver transplantation is a potential curative option, but this procedure is not readily accessible in many regions. In this case, a 15-year-old female patient presented with vague right-sided abdominal pain, fatigue, and abdominal distension. Imaging revealed a large heterogeneous hepatic mass with right portal vein thrombosis and mass effect on the inferior vena cava. Laboratory inv

Spontaneous Splenic Rupture in Severe Acute Pancreatitis: A Rare Life-Threatening Complication and Its Successful Management.

The severity of acute pancreatitis ranges from mild discomfort to severe illness with significant complications. While most cases resolve with supportive care, severe acute pancreatitis may lead to rare but serious issues such as spontaneous splenic rupture. A 46-year-old female with a history of alcohol use, hypertension, depression, and anxiety presented with persistent abdominal pain, nausea, and vomiting. Initial imaging revealed acute pancreatitis with peripancreatic fluid collections. Despite conservative management, her symptoms persisted. She experienced sudden worsening of abdominal p

Acute hepatitis in a biological male patient secondary to suspected disseminated gonococcal infection in the absence of Fitz-Hugh-Curtis syndrome: a case report.

BACKGROUND: Disseminated gonococcal infection is well described in the literature and most commonly presents with signs and symptoms of systemic inflammation including arthritis, tenosynovitis, dermatitis, or imaging findings consistent with Fitz-Hugh-Curtis syndrome. This condition is characterized by inflammation of the liver capsule with adhesion formation-most commonly a sequela of chronic pelvic inflammatory disease. However, there are no documented cases of acute hepatitis secondary to disseminated gonococcal infection in the absence of these findings. CASE PRESENTATION: Here we present

Gynura segetum induces hepatic sinusoidal obstruction syndrome in a child: A case report.

RATIONALE: Hepatic sinusoidal obstruction syndrome (HSOS), which includes hepatic stasis and portal hypertension, is a rare vascular disorder of the liver. It is often associated with hematopoietic stem cell transplantation. It is also possible to treat this disease using Chinese herbal medicines that contain pyrrolizidine alkaloids (PAs). This disease is extremely rare in children and poses a serious threat to their health. To our knowledge, this is the first case of HSOS in a child with PAs. PATIENT CONCERNS: We report a 4-year-old boy suffering from abdominal pain, hepatomegaly, massive as

EMG Validation of a Subject-Specific Thoracolumbar Spine Musculoskeletal Model During Dynamic Activities in Older Adults.

Musculoskeletal models can uniquely estimate in vivo demands and injury risk. In this study, we aimed to compare muscle activations from subject-specific thoracolumbar spine OpenSim models with recorded muscle activity from electromyography (EMG) during five dynamic tasks. Specifically, 11 older adults (mean = 65 years, SD = 9) lifted a crate weighted to 10% of their body mass in axial rotation, 2-handed sagittal lift, 1-handed sagittal lift, and lateral bending, and simulated a window opening task. EMG measurements of back and abdominal muscles were directly compared to equivalent model-predi
