Mechanisms of mitral valvar insufficiency in children and adolescents with severe rheumatic heart disease: an echocardiographic study with clinical and epidemiological correlations.

We carried out a detailed clinical, epidemiological, and echocardiographic study in 41 patients < or = 14 years of age who were admitted in a public hospital in Salvador, Brazil, with severe rheumatic heart disease. Mitral insufficiency was severe in 90%, and moderate in 10%, of the patients. A posteriorly directed jet was seen in 93% of the patients. We identified three mechanisms producing the regurgitation: prolapse of the aortic leaflet of the mitral valve in 13 (32%) patients, rupture of tendinous cords in 14 (34%), and a retracted, non-coapting mural leaflet in 14 (34%). The mean ages, w

Antidepressant exposure in bipolar children.

BIPOLAR disorder is increasingly diagnosed in children and adolescents. Given that antidepressants may precipitate mania, and with increased use of antidepressants in youths, it is reasonable to ask whether antidepressant administration might play a role in inducing earlier manic episodes. We reviewed all consecutive admissions with a diagnosis of bipolar disorder to a university-affiliated children's hospital, and collected information regarding previous exposure to antidepressants and stimulants. The mean age of diagnosis of bipolar disorder in our cohort was 12 +/- SD 3.47 years. Children w

Adverse effects of BCG revaccination: a report on 13 cases from Rio de Janeiro, Brazil.

SETTING: BCG revaccination is not used worldwide. In Brazil, it has been recommended since 1994 in children aged 6 to 14 years. Reports in the medical literature of adverse reactions due to revaccination are rare. BCG revaccination remains officially recommended by the Brazilian health authorities, based on the current epidemiological tuberculosis situation in the country. OBJECTIVE: We report 13 cases of children and adolescents with complications due to BCG revaccination. DESIGN: Case reports of patients followed up in a pneumology unit of a university pediatric hospital from May 1996 to D

Management of lawnmower injuries to the lower extremity in children and adolescents.

Lawnmower-associated trauma remains a substantial source of extremity injury in the pediatric and adolescent patient populations, producing complex wounds that require a combined orthopedic and plastic surgical approach. The authors review their experience with 16 patients, 2 to 17 years of age (mean age, 6.2 years), who were admitted to Duke University Medical Center for lower extremity lawnmower trauma between January 1988 and December 1999. The average hospitalization time was 13.5 days, and an average of 2.9 surgical procedures per patient were performed. Early debridement and bony fixatio

The disruptive behavior disorders in the psychiatric emergency service.

Many children and adolescents presenting to the Psychiatric Emergency Service (PES) are diagnosed with the Disruptive Behavior Disorders (DBD), Conduct Disorder (CD), and Oppositional Defiant Disorder (ODD). Sometimes it may be difficult to reliably diagnose these disorders in the PES setting. Given these limitations, a large database of 6 years of PES visits showed 314 patients with DBD compared with 1625 without DBD. More DBD patient visits required the intervention of police and/or the mobile crisis team. These patients are more likely to have additional diagnoses of depression and attentio

Peripheral blood lymphocyte apoptosis: a clue to the diagnosis of acute infectious mononucleosis.

OBJECTIVE: To describe the morphology and significance of apoptotic lymphocytes in peripheral blood smears of patients with acute infectious mononucleosis. To our knowledge this has not been previously reported. DESIGN: Peripheral blood smears from 27 patients with a positive heterophile antibody test were collected and reviewed for the presence of apoptotic lymphocytes. Flow cytometry was performed on three cases to document the previously described low expression of bcl-2 in lymphocytes in infectious mononucleosis. Four control patient populations comprising 80 cases were similarly screened

The role of physical activity in the development and maintenance of eating disorders.

This study was intended to establish the pathogenic significance of sport and exercise in the development of eating disorders. Hospitalized eating disordered patients and an age-matched control group were assessed. Historical and current physical activity data were collected. An indepth interview was also conducted to ascertain the age of onset of the diagnostic symptoms for eating disorders, and to determine whether: (i) exercising predated dieting; (ii) patients had been involved in competitive athletics; (iii) exercise was excessive; and (iv) weight loss was inversely related to level of ex

Colovaginoplasty in infants and children.

OBJECTIVE: To develop a technique of vaginoplasty suitable for use in children with vaginal insufficiency, without the need for lubrication and regular dilatation. Enteric segment vaginoplasty, previously reported in adolescents and adults, does not require dilatation and has growth potential unlike current methods, involving skin flaps or free grafts. PATIENTS AND METHODS: We report the use of a segment of sigmoid colon in seven consecutive children (age range 4 months to 16 years). The diagnoses included congenital adrenal hyperplasia (1), androgen insensitivity syndrome (3), and vaginal ag

[Medical care in large camp grounds--a report of experiences].

This paper reports on the experiences gathered in respect of medical care during a ten-day youth camp with members of German girl guide and boy scout associations in the summer of 1988 comparising 6,000 youngsters. Large-scale tent camping of children and adolescents lasting for several days requires a costly and complicated infrastructure for self-support. A medical aid unit where patients can also be looked after on an inpatient basis if necessary, is mandatory if there are more than approximately 300 campers. There should be a doctor-participants ratio of 1:500 or 1:800 for "around-the-cloc

The morbidity of schistosomiasis mansoni in the Rusizi Plain (Burundi).

A cross-sectional study was made of the morbidity due to Schistosoma mansoni infection in the Rusizi plain, Burundi. An evenly distributed 5% population sample (n = 6203) was examined; each subject was submitted to a standardized medical history and abdominal palpation. The prevalence of infection was 33% and most infections were light. Diarrhoea was complained of by 26% of those infected and 21% of those not infected; "bloody diarrhoea" by 13% and 4%, respectively. The association with schistosomiasis was significant in all age groups. "Abdominal pain" was a very common complaint, "tiredness"

Pelvic inflammatory disease associated with Neisseria gonorrhoeae and Chlamydia trachomatis: clinical correlates.

The purpose of this study was to determine whether selected clinical features can distinguish salpingitis associated with endocervical Neisseria gonorrhoeae from that caused by Chlamydia trachomatis in black, inner-city adolescents. We reviewed retrospectively the charts of teenagers presenting to a university hospital outpatient department between January 1982 and January 1984 who were diagnosed as having salpingitis. We included all teenagers who presented with a history of low abdominal pain plus (1) either cervical motion tenderness (n = 15), adnexal tenderness (n = 13), or both (n = 57);

Pectus excavatum. Late results with and without operation.

The records were reviewed of 334 patients with a diagnosis of pectus excavatum at Presbyterian Hospital in New York, New York, from 1948 through 1977. No operation was done on 168; 174 operations were done on 166. In those not operated upon 18% of infants died of associated anomalies. The deformity improved or disappeared in half of the surviving infants and in some children up to the age of 6 years. Thereafter, it remained the same or worsened. Mild deformities were compatible with long life without symptoms; severe deformities were associated with chronic disability. In those operated upon,

Neurological manifestations of SARS-CoV-2 infection in hospitalised children and adolescents in the UK: a prospective national cohort study.

BACKGROUND: The spectrum of neurological and psychiatric complications associated with paediatric SARS-CoV-2 infection is poorly understood. We aimed to analyse the range and prevalence of these complications in hospitalised children and adolescents. METHODS: We did a prospective national cohort study in the UK using an online network of secure rapid-response notification portals established by the CoroNerve study group. Paediatric neurologists were invited to notify any children and adolescents (age <18 years) admitted to hospital with neurological or psychiatric disorders in whom they consi

Non-pharmacological interventions for sleep promotion in hospitalized children.

BACKGROUND: Healthy sleep is an important component of childhood development. Changes in sleep architecture, including sleep stage composition, quantity, and quality from infancy to adolescence are a reflection of neurologic maturation. Hospital admission for acute illness introduces modifiable risk factors for sleep disruption that may negatively affect active brain development during a period of illness and recovery. Thus, it is important to examine non-pharmacologic interventions for sleep promotion in the pediatric inpatient setting. OBJECTIVES: To evaluate the effect of non-pharmacologic
