Aggressive Wound Care by a Multidisciplinary Team Improves Wound Healing after Infrainguinal Bypass in Patients with Critical Limb Ischemia.

BACKGROUND: A long period is generally required for ischemic ulcer to heal after revascularization. The strategy of postoperative wound care can affect wound healing. This study was conducted to investigate the degree to which aggressive wound care (AWC) by a team of multidisciplinary specialists actually shortens the time to wound healing and increases the rate of wound healing in limbs undergoing surgical bypass for ischemic tissue loss in a real clinical setting. METHODS: A total of consecutive 126 patients undergoing infrainguinal bypass for tissue loss from April 2011 to March 2015 were

Consensus Statement for Clinical Pathway Development for Perioperative Pain Management and Care Transitions.

The perioperative surgical home (PSH) model has been created with the intention to reduce costs and to improve efficiency of care and patient experience in the perioperative period. The PSH is a comprehensive model of care that is team-based and patient-centric. The team in each facility should be multidisciplinary and include the input of perioperative services leadership, surgical services, and support personnel in order to provide seamless care for the patient from the preoperative period when decision to undergo surgery is initially made to discharge and, if needed after discharge from the

Randomized comparison of the effectiveness of nasal intubation using a GlideScope video laryngoscope with Magill forceps versus vascular forceps in patients with a normal airway.

PURPOSE: The GlideScope® video laryngoscope (GVL) is widely used for nasotracheal intubation in dental and facial plastic surgery. The angle of the Magill forceps is different from that of the GVL blade, which suggests that the Magill forceps are not the ideal forceps for use with the GVL. The purpose of this study was to compare the effectiveness of the Magill forceps vs vascular forceps for nasotracheal intubation using the GVL. METHODS: This study included 60 patients scheduled to undergo elective surgery requiring nasotracheal intubation. Patients were assigned to one of two groups-i.e.,

The experience of setting up a resident-managed Acute Pain Service: a descriptive study.

BACKGROUND: The benefits of an Acute Pain Service (APS) for pain management have been widely reported, but its diffusion is still limited. There are two APS models: anesthesiologist-based and a nurse-based model. Here we describe the development of a different APS model managed by anesthesia residents, and we report the first year of activity in a tertiary Italian university hospital (Careggi University Hospital, Florence, IT). METHODS: Patients were included in the APS were those undergoing abdominal and urologic surgery causing moderate or severe postsurgical pain. The service was provided

Perioperative and periprocedural airway management and respiratory safety for the obese patient: 2016 SIAARTI Consensus.

Proper management of obese patients requires a team vision and appropriate behaviors by all health care providers in hospital. Specialist competencies are fundamental, as are specific clinical pathways and good clinical practices designed to deal with patients whose Body Mass Index (BMI) is ≥30 kg/m2. Standards of care for bariatric and non-bariatric surgery and for the critical care management of this population exist but are not well defined nor clearly followed in every hospital. Thus every anesthesiologist is likely to deal with this challenging population. Obesity is a multisystem, chroni

Intubation learning curve: comparison between video and direct laryngoscopy by inexperienced students.

Background: Direct laryngoscopy (DL) is considered the most common method of tracheal intubation. On the other hand, evidence shows the growing role of video laryngoscopy in danger airway administration. Objectives: Due to the importance of a proper training to accomplish an accurate and fast intubation by the student of anesthesia, this research was conducted to assess the effects of DL and video laryngoscopy (Glidescope VL) training on the success rate of tracheal intubation by low-skill students. Materials/Patients and styles: 50 undergraduate students of anesthesiology took part in this ra

A comparison of fospropofol to midazolam for moderate sedation during outpatient dental procedures.

Moderate intravenous (IV) sedation combined with local anesthesia is common for outpatient oral surgery procedures. An ideal sedative agent must be safe and well tolerated by patients and practitioners. This study evaluated fospropofol, a relatively new sedative/hypnotic, in comparison to midazolam, a commonly used benzodiazepine, for IV moderate sedation during oral and maxillofacial surgery. Sixty patients were randomly assigned to either the fospropofol or the midazolam group. Each participant received 1 μg/kg of fentanyl prior to administration of the selected sedative. Those in the fospro

[Trauma of the Membranous Trachea Managed by the ENT-Surgeon].

BACKGROUND: Isolated perforations of the membranous trachea are extremely rare but potentially life-threatening complications of endotracheal intubations and minimally invasive tracheostomy techniques. Most of the case-reports have been written by thoracic surgeons or anesthesiologists but both the diagnostic procedures and the therapy are not standardized. The aim of this study was to evaluate the position of the ENT-surgery in the management of these lesions. PATIENTS AND METHODS: Over a period of 6 years we treated 5 females, 3 males and 3 children with iatrogenic lacerations of the poster…

Evaluation of the safety and efficacy of deep sedation for electrophysiology procedures administered in the absence of an anesthetist.

UNLABELLED: Several procedures performed in the electrophysiology laboratory (EP lab) require surgical manipulation and are lengthy. Patients undergoing such procedures usually receive general anesthesia or deep sedation administered by an anesthesiologist. In 536 consecutive procedures performed in the EP lab, we assessed the safety and efficacy of deep sedation administered under the direction of an electrophysiologist and in the absence of an anesthetist. Patients were monitored with pulse oximetry, noninvasive blood pressure recordings, and continuous ECGs. The level of consciousness and v

Ketamine-Based Sedation Among Young Infants During MRI at Mzuzu Central Hospital: Initial Experience in a Resource-Limited Setting in Malawi.

BACKGROUND: Magnetic resonance imaging (MRI) in young infants requires effective immobilization to ensure diagnostic image quality. Although non-pharmacological approaches such as the feed-and-swaddle technique are widely recommended, they are not always successful. Sedation in neonates remains controversial, particularly in resource-limited settings. This study aimed to evaluate the feasibility and observed safety profile of intramuscular ketamine for MRI sedation in young infants at Mzuzu Central Hospital, Malawi. METHODS: This retrospective case series included five young infants (aged 3-9

Opioid-free blended analgesia in patients undergoing thoracic surgery: Preliminary results.

OBJECTIVE: Thoracic surgery is recognized as one of the most painful surgical procedures. Opioids represent the most effective analgesic drug for pain management after these procedures. However, opioid use is related to side effects such as nausea, constipation, drowsiness, hypotension, bradycardia. Because of this, recently, studies have focused on blended analgesia, which combines general and loco-regional anesthesia (LRA).This study aims to demonstrate the effectiveness of opioid-free blended anesthesia in pain management in patients undergoing open or endoscopic thoracic surgery. DESIGN:

Preemptive Low-Dose Norepinephrine Infusion for Reducing Hemodynamic Instability During Craniotomy for Brain Tumor Resection Under Propofol-Remifentanil Total Intravenous Anesthesia: A Randomized Controlled Trial.

Background: Previous trials of prophylactic norepinephrine have compared it with volume loading or non-norepinephrine vasopressors. Thus, it remains unclear whether a preemptive norepinephrine strategy provides incremental benefit over conventional management permitting reactive norepinephrine use. We evaluated whether preemptive low-dose norepinephrine infusion reduces hemodynamic instability during craniotomy for brain tumor resection under propofol-remifentanil total intravenous anesthesia (TIVA). Methods: Adult patients undergoing craniotomy for brain tumor resection under propofol-remifen
