[The acute effects of the new angiotensin I-converting enzyme inhibitor, enalapril maleate, on blood pressure, plasma renin, aldosterone and kinins in hypertensive patients].

The acute antihypertensive effect of a new long-acting oral angiotensin I-converting enzyme (ACE) inhibitor, enalapril maleate, was assessed in 20 hypertensive patients, of whom 14 had essential hypertension, 4 had renovascular hypertension, one had hypertension associated with chronic renal failure, and one had primary aldosteronism. Enalapril maleate significantly lowered the blood pressure in either low-renin or normal- and high-renin hypertensives. There was a significant correlation for all patients as a group between the pretreatment levels of serum ACE activity and the reduction in mean

Effects of a single administration of captopril on hemodynamics and serum angiotensin converting enzyme activity, plasma renin activity and plasma aldosterone concentration in hypertensive patients.

Hemodynamic responses and behaviors of the renin-angiotensin-aldosterone system to a single administration of captopril (50 mg) were studied in 16 hypertensive patients (essential hypertension, n = 10; renovascular hypertension, n = 3; primary aldosteronism, n = 2; Cushing's syndrome, n = 1). In 10 essential hypertensive patients, the immediate blood pressure reduction caused by decreased total peripheral resistance after the administration of captopril was observed without changes of cardiac output and heart rate. Serum angiotensin converting enzyme activity and plasma aldosterone concentrati

A Novel Angiotensin I-Converting Enzyme Inhibitory Pentapeptide from Red Mold Rice: Identification, Stability, In Vivo Antihypertensive Effects, and Underlying Mechanisms.

Bioactive peptides from fermented food have potential for hypertension management. Red mold rice (RMR), a traditional food made by fermenting rice with Monascus strains, has shown antihypertensive effects. However, research on ACE inhibitory peptides from RMR remains limited. In this study, a novel pentapeptide Val-Leu-Thr-Gly-Trp (VLTGW) was isolated from RMR and displayed potent angiotensin-converting enzyme (ACE) inhibitory activity with an IC50 value of 17.4 μM. VLTGW's 550-fold selectivity for ACE over renin indicated a potential advantage for reducing off-target effects. VLTGW retained o

Age-stratified efficacy and safety of esaxerenone versus angiotensin II receptor blockers in older patients with hypertension: a predefined subanalysis of the ESCORT-HT study.

The ESCORT-HT study recently demonstrated non-inferiority of esaxerenone to angiotensin II receptor blockers (ARBs) in lowering morning home systolic blood pressure (SBP) in Japanese patients aged ≥65 years with inadequately controlled hypertension on amlodipine. This predefined subgroup analysis of the ESCORT-HT study analyzed age-related differences in the efficacy and safety of esaxerenone relative to ARBs. In the 65-74-year subgroup, 98 patients were treated with esaxerenone and 94 with an ARB; in the ≥75-year subgroup, 104 were treated with esaxerenone and 106 with an ARB. The least squar

Association of prior Angiotensin receptor-neprilysin inhibitor use with the initial eGFR dip after initiation of SGLT2 inhibitors in patients with chronic kidney disease.

BACKGROUND: Sodium-glucose cotransporter 2 (SGLT2) inhibitors provide cardiorenal protection; however, a transient decline in estimated glomerular filtration rate (eGFR), known as the ''initial dip'', is frequently observed after initiation. Although this phenomenon reflects changes in intraglomerular hemodynamics, excessive decline may be associated with worsening renal function. Angiotensin receptor-neprilysin inhibitors (ARNIs) have demonstrated renal protective effects in heart failure, but their impact on the initial dip after SGLT2 inhibitor initiation remains unclear. METHODS: We retro

PDZK1 is downregulated in ovariectomized mice with angiotensin II-induced hypertension and interacts with β2-AR to regulate endothelial ERK1/2/c-Fos signaling.

BACKGROUND: Estrogen deficiency is a major risk factor for postmenopausal hypertension. PDZK1, an estrogen-responsive scaffold protein, may interact with the β2-adrenergic receptor (β2-AR) to regulate vascular function, but this axis has not been characterized in the context of estrogen-deficient hypertension. METHODS: We used ovariectomized and PDZK1 knockout mice infused with angiotensin II to model hypertension in vivo, and HUVECs for mechanistic studies in vitro. Techniques included hemodynamic measurements, histology, molecular interaction assay, chromatin immunoprecipitation, and functi

Expert Opinion on Optimising Guideline-Directed Medical Therapy and Expanding the Role of Angiotensin Receptor-Neprilysin Inhibitors (ARNIs) for Heart Failure Management in India.

Strong clinical evidence and clear guideline recommendations support the use of guideline-directed medical therapy (GDMT) - particularly angiotensin receptor-neprilysin inhibitors (ARNIs) - in heart failure (HF) management, yet real-world adoption in India remains limited. Indian registry data indicate that just one quarter of patients leave the hospital with complete GDMT, and less than 5% receive ARNIs, even though robust evidence strongly favors these drugs. Hypotension, renal dysfunction, delayed initiation, and limited familiarity are common barriers that often also hinder optimal use. Sa

TangShenKang decoction plays an anti-inflammatory and anti-fibrotic role in diabetic nephropathy by activating AT2R/ATIP1/SHP1 axis and antagonizing angiotensin II.

Diabetic nephropathy (DN) is the most prevalent microvascular complication of diabetes, where a hyperglycemic environment can induce chronic damage to renal vessels and parenchyma, severely endangering patients' health. TangShenKang decoction (TSK) is a traditional Chinese medicine formula developed based on the clinical and pathological characteristics of DN. In this study, through transcriptomic data analysis of a rat DN model and a TSK intervention group, the key regulatory gene AT2R was identified. Molecular mechanisms underlying TSK's effects on inflammation and fibrosis in DN were invest

FGF18 aggravates angiotensin II-induced vascular smooth muscle cell dysfunction via the FGFR-ERK/AKT-GSK3β/β-catenin pathway.

BACKGROUND: The development and progression of thoracic aortic aneurysm (TAA) are closely related to vascular smooth muscle cell (VSMC) apoptosis, oxidative stress, and phenotypic transformation. This study investigated the role of fibroblast growth factor 18 (FGF18) in angiotensin II (Ang II)-induced TAA and its underlying mechanisms. METHODS: An in vitro Ang II-induced VSMC injury model was established, and TAA-related candidate genes were screened from GSE83675 via bioinformatics analysis. VSMCs were subsequently treated with recombinant FGF18 or small interfering RNA targeting FGF18 (si-F

Impact of continuous positive airway pressure therapy on renin-angiotensin-aldosterone system in obstructive sleep apnea: an updated systematic review and meta-analysis.

PURPOSE: While continuous positive airway pressure (CPAP) reduces cardiovascular risk in obstructive sleep apnea (OSA), its effects on renin–angiotensin–aldosterone system (RAAS) modulation remain controversial. This meta-analysis investigates CPAP's differential impacts on RAAS components and identifies responsive patient subgroups. METHODS: Sixteen studies were included in the systematic review and eight (231 patients) in the meta-analysis. Primary outcomes included plasma aldosterone concentration (PAC), plasma renin activity (PRA), and angiotensin II (AngII) changes. Subgroup analyses exam

WTAP Silencing protects human aortic smooth muscle cells from angiotensin II-induced senescence, apoptosis, ferroptosis, and inflammation by regulating PCSK9.

Abdominal aortic aneurysm (AAA) is a life-threatening vascular disease characterized by aortic wall degeneration and inflammation. The molecular mechanisms underlying AAA development remain unclear. Wilms tumor 1-associated protein (WTAP) has been implicated in various biological processes, but its role in AAA pathogenesis, particularly in cardiomyocyte regulation, has not been fully explored. Quantitative real-time PCR (qRT-PCR) was performed to detect the mRNA levels of WTAP and proprotein convertase subtilisin/kexin type 9 (PCSK9). Western blotting assay was used to analyze protein expressi

KLF4 regulates FAM3A to promotes angiotensin II-induced proliferation and migration of vascular smooth muscle cells through the PI3K/AKT signaling pathway.

BACKGROUND: Hypertension, a major cause of cardiovascular disease, is linked to vascular remodeling, which is influenced by phenotypic changes in vascular smooth muscle cells (VSMCs). Studies have shown that KLF4 influences vascular remodeling by promoting VSMC dedifferentiation, increasing proliferation, and enhancing inflammatory responses, while FAM3 may play a key role in VSMC migration and proliferation. Angiotensin II (Ang II) contributes to remodeling, but the mechanisms are unclear. METHODS: Ang II was used to stimulate VSMCs in order to evaluate the expression levels of KLF4 and FAM3

Secreted Protein Acidic and Rich in Cysteine (SPARC) Induced by the Renin-Angiotensin System Causes Endothelial Inflammation in the Early Stages of Hypertensive Vascular Injury.

Secreted protein acidic rich in cysteine (SPARC), one of the extracellular matrix proteins, is highly induced during inflammation. We investigated the pathophysiological regulation and role of SPARC in vascular inflammation in a rat model of hypertension created using deoxycorticosterone acetate (DOCA, 40 mg/kg/week, s.c.) and salt (1% in drinking water). DOCA-salt administration time-dependently increased systolic blood pressure during the 3-week treatment period, blunted endothelium-dependent vasodilation, and increased monocyte chemoattractant protein-1 (MCP-1) and lectin-like oxidized low-

Anticoagulant and angiotensin-converting enzyme inhibitory activities of a fucoidan from Sargassum horneri and its low molecular weight fragments prepared by acid degradation.

A purified fucoidan SF (348 kDa) was isolated from Sargassum horneri. Six low molecular weight fragments SFs (75-8.1 kDa) were prepared by acid degradation from SF. Anticoagulant properties of SF and SFs were evaluated. SF and SFs with 75-17 kDa had better anticoagulant activities with the activated partial thromboplastin time (APTT) greater than 120 s at 200 μg/mL. The decrease in the molecular weight caused a decrease in anticoagulant activity, and a slight decrease almost resulted in loss of anticoagulant activity when below 17 kDa (APTT < 59 s). Angiotensin-converting-enzyme (ACE)-inhibito
