Clinical and Epidemiological Profile of Atrial Fibrillation in a High-Altitude Tertiary-Care Setting.

Background and Objectives: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and a major cause of stroke, heart failure, and death. Evidence on AF in Latin American and Andean tertiary-care settings remains limited. The objective was to describe the demographic and clinical profile, documented thromboembolic and bleeding risk, in-hospital management, and outcomes of adults with AF treated at a tertiary hospital located at 2850 m above sea level. Materials and Methods: We retrospectively reviewed unique adult patient records from January 2021 through December 2023 identif

Dabigatran-induced acute hepatitis in a patient with atrial fibrillation: a rare case report.

BACKGROUND: Dabigatran etexilate is a prodrug converted into active dabigatran, a reversible thrombin inhibitor, after oral administration. The Food and Drug Administration approved it for stroke prevention in non-valvular atrial fibrillation patients and for treating deep venous thrombosis and pulmonary embolism. In the Randomized Evaluation of Long-Term Anticoagulation Therapy trial, dabigatran demonstrated non-inferior efficacy and reduced haemorrhagic stroke risk compared with warfarin. Herein, we present a rare case of a patient with atrial fibrillation who developed acute hepatitis follo

APOE ε4 and Risk of Intracranial Hemorrhage in Patients With Atrial Fibrillation Taking Apixaban.

IMPORTANCE: The APOE ε4 variant is causally linked to cerebral amyloid angiopathy and is a risk factor for intracranial hemorrhage (ICH) among warfarin-treated patients with atrial fibrillation. Nevertheless, its impact on those treated with apixaban remains unknown. OBJECTIVE: To test the hypothesis that APOE ε4 allele carriership is associated with an increased risk of ICH in patients with atrial fibrillation taking apixaban. DESIGN, SETTING, AND PARTICIPANTS: This cohort study involved data from the All of Us Research Program, a longitudinal, population-based study in the United States. I

Hypertension and Atrial Fibrillation: Insight From Basic to Translational Science Into the Mechanisms and Management.

There is a parallel epidemic of atrial fibrillation (AF) and hypertension (HTN) occurring globally. Both AF and HTN are no longer confined to the older population. The pathophysiology of AF related to HTN is complex with many inter-related factors. Hypertension causes hypertrophy of the ventricular musculature, dilatation of atria, elevated intracardiac pressures, atrial and ventricular mechanical dysfunction, abnormal expression of ion channels, dysregulation of the renin-angiotensin-aldosterone system, and the autonomic function. The pathophysiological alterations and structural changes that

Benefits of Early Catheter Ablation in Patients With Atrial Fibrillation: A Nationwide Cohort Study.

BACKGROUND: Although early rhythm control showed better clinical outcomes in patients with atrial fibrillation (AF), limited data exist on the relationship between the timing of atrial fibrillation catheter ablation (AFCA) and clinical outcomes. OBJECTIVES: This study investigated the impact of early vs late AFCA on clinical outcomes. METHODS: Using the Korean claims database, 944,710 AF patients without previous AFCA were investigated. These patients were categorized into 3 groups based on the duration from their AF diagnosis to AFCA: <1 year (early-AFCA), 1 to <3 years (intermediate-AFCA),

Prognostic Implications and Global Perspectives of Atrial Fibrillation in Patients Hospitalized for Heart Failure.

BACKGROUND: Atrial fibrillation (AF) and heart failure (HF) each contributes to global disease burden and can coexist. The interplay of prior HF, prior AF, and presenting rhythm have not previously been jointly considered in prognostic implication. OBJECTIVES: The authors sought to assess 1-year all-cause mortality according to permutations of prior HF, prior AF, and AF as presenting rhythm, in a global cohort of patients hospitalized for HF. METHODS: REPORT-HF enrolled patients during hospitalization for acute HF from 44 countries over 6 continents. Cox proportional hazard models were used

Differential effects of anesthetics and sex on supraventricular electrophysiology and atrial fibrillation substrate in rats.

Rodents are increasingly used in atrial electrophysiology research, yet such studies are often performed under anesthesia owing to technical challenges. Here we developed an implantable device for comprehensive atrial studies in ambulatory rats and investigated the effects of commonly used anesthetics on supraventricular electrophysiology and arrhythmic substrate, comparing them with the unanesthetized state (UAS). Adult rats were evaluated 4 weeks after implantation. Studies were conducted in the UAS under 2% isoflurane (ISO) and under 40 mg/kg pentobarbital (PEN). Pacing protocols determined

Appropriateness of antithrombotics in geriatric inpatients with atrial fibrillation: a retrospective, cross-sectional study.

BACKGROUND: Atrial fibrillation occurs in nearly half of geriatric inpatients and is a major cause of morbidity and mortality. Suboptimal anticoagulation use is an important concern in this population. This study aimed to evaluate the appropriateness of antithrombotic therapies in this patient cohort. METHODS: A retrospective analysis was conducted on the geriatric wards of a teaching hospital in Belgium, on a background of clinical pharmacy services. The first 90 atrial fibrillation patients from 2020 to 2022 were included if they received an oral anticoagulant. We assessed utilisation and a

Surgical ablation of atrial fibrillation with concomitant cardiac surgery: a state-of-the-art review.

Patients with preoperative atrial fibrillation (AF) undergoing cardiac surgery face a heightened risk of complications and reduced survival. Concomitant surgical ablation (SA) has shown promise in mitigating the arrhythmic burden, prompting guideline upgrades by major scientific societies. However, SA remains underutilized, with performance rates varying between 22% and 48%, depending on the type of procedure. The goal of this narrative review is to summarize current evidence to aid physicians in decision making regarding AF management during cardiac surgery. This review examines existing lite

CHA2DS2-VASc Score in Patients With Atrial Fibrillation and Cancer: A U.S. Nationwide Study.

The CHA2DS2-VASc score is widely accepted as the most reliable tool for risk stratification to guide the initiation of anticoagulation in patients with atrial fibrillation/flutter. However, it has not been validated for use in patients with malignancy, and lacks cancer-related parameters. We aimed to evaluate the CHA₂DS₂-VASc score's association with acute cerebrovascular accident (CVA) in cancer patients during hospitalization for atrial fibrillation/flutter in the United States. We conducted a cross-sectional analysis of the Nationwide Readmissions Database (NRD) from 2016 to 2019, extractin

Comparative effectiveness and safety of direct oral anticoagulants in atrial fibrillation patients with dementia.

INTRODUCTION: Patients with atrial fibrillation (AF) and dementia face unique challenges in stroke prevention, particularly in selecting appropriate anticoagulation therapy. Direct oral anticoagulants (DOACs) effectively reduce stroke and embolism risks, but evidence comparing their effectiveness and safety in this population remains limited. METHODS: This retrospective, population-based cohort study used data from Taiwan's National Health Insurance Research Database to evaluate outcomes of four DOACs (dabigatran, apixaban, edoxaban, and rivaroxaban) in AF patients with dementia aged 50 years

Anticoagulation in Atrial Fibrillation: Is a Paradigm Shift From Xa to XIa Inhibitors on the Horizon?

Atrial fibrillation constitutes a substantial global health issue, with its prevalence expected to increase significantly in the coming years. The effective management of atrial fibrillation, particularly regarding the prevention of ischemic stroke and systemic embolism, has progressed with the introduction of direct oral anticoagulants, which have shown a reduction in bleeding risks when compared to warfarin. The oral factor Xa inhibitors, such as apixaban and rivaroxaban, are considered the first line for anticoagulation in cases of atrial fibrillation. Several factor XI inhibitors, includin

Stroke prevention in atrial fibrillation: A narrative review of current evidence and emerging strategies.

BACKGROUND: Atrial fibrillation (AF), the most prevalent cardiac arrhythmia, is associated with a significantly increased risk of mortality and morbidity from stroke, thromboembolism and dementia. Recent advances in stroke prevention strategies necessitate an updated approach to management. RESULTS: Published evidence shows that the Atrial Fibrillation Better Care (ABC) pathway significantly improves stroke prevention outcomes in AF patients, reducing mortality, stroke incidence and bleeding events. Characterisation of AF using the 4S-AF framework helped guide personalised treatment selection

Anticoagulation strategies for stroke prevention in atrial fibrillation: a comprehensive review of current literature.

Atrial fibrillation (AF) significantly increases stroke risk and hence requires anticoagulation for the prevention of stroke. This review discusses strategies of anticoagulation, including the paradigm shift from traditional vitamin K antagonists (VKAs), such as warfarin, to direct oral anticoagulants (DOACs): dabigatran, rivaroxaban, apixaban, and edoxaban. VKAs are effective but require regular monitoring of international normalized ratio and also pose challenges because of their drug and dietary interactions. In contrast, DOACs provide predictable pharmacokinetics, fewer interactions, and n
