Carotid endarterectomy without angiography. The reliability of Doppler ultrasonography and duplex scanning in preoperative assessment.

OBJECTIVE: To validate the use of Doppler ultrasonography and duplex scanning as the only means of preoperative assessment in carotid disease. DESIGN: A retrospective study performed between January 1980 and December 1989. PATIENTS: Of 597 carotid endarterectomies carried out by one author, 130 procedures were performed on 118 patients (47 women, 71 men, ranging in age from 49 to 85 years) without preoperative cerebral angiography. Justifications for proceeding without angiography were risk of angiography (27 patients), iodine allergy (8 patients), renal insufficiency (5 patients), technical…

Indirect revascularization after bypass surgery for intracranial atherosclerotic steno-occlusive disease.

OBJECTIVES: Indirect revascularization after bypass surgery for intracranial atherosclerotic steno-occlusive disease (ICAD) remains controversial. We aimed to evaluate indirect revascularization after direct bypass surgery in patients with ICAD. METHODS: Among 245 patients with ICAD who underwent direct bypass surgery between 2003 and 2022, 191 patients were ultimately enrolled in this study. Treatment outcomes were compared between combined bypass (n = 162 [84.8 %]) and direct bypass alone (n = 29) groups. Multivariable analyses were performed to identify significant predictors of indirect r

Cerebral collaterals and stroke in patients with isolated carotid artery dissections.

To investigate potential association between collateral arterial supply and stroke in patients with isolated internal carotid artery dissection (iCeAD). We hypothesized a lower risk of stroke in patients with more robust collateral supply. This is a single-center, retrospective review of iCeAD patients between 1994 and 2018. iCeAD patients with sufficient neuroimaging data were included. Patients were categorized based on cerebral infarction (stroke) on neuroimaging. The collaterals score, ranging from 0 to 8 with higher scores indicating more robust collaterals, was assessed based on contribu

Dominance of the Anterior Cerebral Artery as a Predictor of Vasospasm-Related Cerebral Infarction After Surgical Treatment of Ruptured Blood Blister-Like Aneurysm in the Internal Carotid Artery.

OBJECTIVE: Vasospasm (VSP)-related ischemic complications have been associated with poor outcomes in patients with subarachnoid hemorrhage due to ruptured blood blister-like aneurysms (BBAs) in the communicating segment of the internal carotid artery (ICA). The purpose of the present study was to investigate the incidence of, and factors related to, VSP-related cerebral infarction (VSP-CI) in patients with surgically treated BBAs in the communicating segment of the ICA. METHODS: The medical records of 25 consecutive patients with surgically treated BBAs in the communicating segment of the ICA

Low Cerebral Blood Volume Identifies Poor Outcome in Stent Retriever Thrombectomy.

BACKGROUND: Mechanical thrombectomy (MT) is an efficient treatment of acute stroke caused by large-vessel occlusion. We evaluated the factors predicting poor clinical outcome (3-month modified Rankin Scale, mRS >2) although MT performed with modern stent retrievers. METHODS: We prospectively collected the clinical and imaging data of 105 consecutive anterior circulation stroke patients who underwent MT after multimodal CT imaging. Patients with occlusion of the internal carotid artery and/or middle cerebral artery up to the M2 segment were included. We recorded baseline clinical, procedural a

Indocyanine Green (ICG) Videoangiography-Guided Dissection of the Sylvian Fissure on the Transsylvian Approach: Technical Note.

BACKGROUND: The transsylvian approach is a standard method of approach for anterior circulation aneurysms or paraclinoid tumors. Care must be taken, however, to avoid inadvertent obliteration of sylvian veins to prevent postoperative brain swelling and venous infarction. Sometimes, the superficial sylvian veins have complex connections or are not visible because of the thickness of the arachnoid membrane, especially in the case of subarachnoid hemorrhage (SAH). The present report describes a simple method to dynamically visualize the sylvian vein via indocyanine green (ICG) angiography for the

Endovascular treatment of fusiform cerebral aneurysms with the Pipeline Embolization Device.

OBJECT: Despite advances in surgical and endovascular techniques, fusiform aneurysms remain a therapeutic challenge. Introduction of flow-diverting stents has revolutionized the treatment of aneurysms with wide necks and of complex morphology. The authors report their experience with the endovascular treatment of fusiform aneurysms using the Pipeline Embolization Device. METHODS: A retrospective review of 146 patients with cerebral aneurysms treated with the Pipeline Embolization Device between June 2011 and January 2013 was performed. Twenty-four patients were identified as having fusiform a

Cerebral hyperperfusion syndrome following the excision of a mycotic aneurysm with superficial temporal artery-to-middle cerebral artery bypass: case report.

The authors report a rare case of cerebral hyperperfusion syndrome (HPS) following the excision of a mycotic aneurysm with superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass. A 74-year-old woman with infective endocarditis presented with progressive cerebral infarction and subarachnoid hemorrhage due to a mycotic aneurysm, which was excised with a STA-MCA bypass. Postoperatively, the patient developed HPS that was considered to be exacerbated by a previous ischemic event. Therefore, cerebral hemodynamics should be evaluated before bypass surgery to prevent subsequent hyperp

Cortical stimulation mapping and Wada results demonstrate a normal variant of right hemisphere language organization.

PURPOSE: Exclusive right hemisphere language lateralization is rarely observed in the Wada angiography results of epilepsy surgery patients. Cortical stimulation mapping (CSM) is infrequently performed in such patients, as most undergo nondominant left hemisphere resections, which are presumed not to pose any risk to language. Early language reorganization is typically assumed in such individuals, taking left hemisphere epileptiform activity as confirmation of change resulting from a pathologic process. We present data from CSM and Wada studies demonstrating that right hemisphere language occu

Outcome evaluation of carotid stenting in high-risk patients with symptomatic carotid near occlusion.

Management of symptomatic carotid near occlusion especially in high-risk patients is different from outcome analysis of NASCET. We evaluated outcome in high-risk patients with symptomatic near occlusion. For 48 patients with near occlusion out of 166 symptomatic high-risk patients who underwent carotid stenting, we assessed the procedural success defined as residual stenosis <30%, modified Rankin Scale (mRS) at one and six months following stenting, and the 13 cerebrovascular factors related to the outcome. Initial National Institutes of Health Stroke Scale (NIHSS) ≥4, 1-3 and 0 were 13, 14 an

Vertebral artery-posterior inferior cerebellar artery bypass with a superficial temporal artery graft to treat aneurysms involving the posterior inferior cerebellar artery.

OBJECT: In patients with aneurysms that require occlusion of the posterior inferior cerebellar artery (PICA), revascularization of this artery should be performed. A novel surgical method for revascularization of the PICA is presented. METHODS: After a segment of the superficial temporal artery (STA) was harvested, the aneurysm was treated by trapping, followed by placement of a vertebral artery (VA)-PICA bypass in which the STA segment was used as a graft. When the length of the proximal PICA was inadequate, the distal end of the STA was anastomosed to the proximal PICA in an end-to-side fas

Hyperperfusion syndrome with hemorrhage after angioplasty for middle cerebral artery stenosis.

Hyperperfusion syndrome is a well-documented complication of carotid endarterectomy, as well as internal carotid artery angioplasty and stent placement. We report a similar complication after distal intracranial (middle cerebral artery [MCA] M2 segment) angioplasty. To our knowledge, this is the first report of hyperperfusion syndrome after intracranial angioplasty of a distal MCA branch.

[Intracranial vertebral artery stenosis successfully revascularised by cerebral angioplasty and stenting in chronic stage: a case report of prompt improvement of the neurological impairment].

A 72-year-old man with a history of hypertension had a left cerebellar infarction and followed by a right cerebellar infarction within about one and a half months after the initial stroke. Brain magnetic resonance images(MRI) showed infarctions in both middle cerebellar peduncles and in the mid-portion of lower pons. Right veretebral artery(VA) terminated in posterior inferior cerebellar artery(PICA). Left intracranial VA has a high-grade eccentric atherosclerotic stenosis(91%) proximal to the left PICA. No collateral circulation was developed from bilateral carotid arteries. Three months afte

Striatocapsular infarction: MRI and MR angiography.

We present a case of left striatocapsular infarction manifest clinically as a transient right hemiparesis. MRI showed a left striatocapsular infarct. Striatocapsular infarction, unlike lacunar infarction, is often associated with occlusive disease of the carotid artery. In order to screen the carotid vessels, cervical MR angiography (MRA) was performed during the same examination, demonstrating a left internal carotid artery occlusion, confirmed by contrast arteriography. MRA, a noninvasive modality, can be a useful adjunct to MRI, when diagnostic information concerning the cervical carotid ar

Moyamoya disease: computed tomographic and angiographic correlation in 10 caucasoid patients.

The computed tomographic (CT) features in 10 patients with moyamoya disease have been analysed. The scan findings were correlated with the corresponding angiographic profile and the stage of the vascular disease. Multiple infarcts (82%), abnormal enhancement patterns (50%) and cerebral atrophy (60%) were the frequent abnormalities detected. Though the abnormalities on scan did not correlate entirely with the angiographic and the clinical profiles, the CT Scans were most frequently abnormal in later stages of the disease (Stages-4 and 5). No scan abnormalities were detected in the territory of

Intracranial vascular lesions: optimization and clinical evaluation of three-dimensional time-of-flight MR angiography.

The clinical value of three-dimensional time-of-flight magnetic resonance (MR) angiography was prospectively evaluated in 26 patients with congenital intracranial vascular lesions; 12 had arteriovenous malformations (AVMs), and 14 had venous angiomas. In the initial phase of the study the entire region of interest was imaged with one large acquisition volume (60-120-mm-thick slab). Later, the angiograms were obtained with adjacent but slightly overlapping, 30-mm-thick slabs, which clearly improved vascular detail. Gadolinium enhancement slightly improved depiction of veins but not of arteries.
