The clinical and economic efficacies of antibiotic prophylaxis in the surgical unit of the Hospital were confirmed by the results of the analysis of 1313 case records of the patients operated during a year for acute appendicitis and acute cholecystitis. At the same time it was shown advisable to use antibiotic therapy in the patients with various pathological processes. Methicillin-resistant Staphylococcus aureus (MRSA) has become a major problem worldwide and is associated with significant morbidity and mortality. It is a particular issue in surgical patients especially vascular surgery involving prosthetic implants. We present an unusual case of MRSA gallbladder empyema and septicaemia in a previous vascular surgical patient. Teranishi et al reported a case of acute cholecystitis caused by MRSA in the Japanese literature but as far as we are aware this is the first case reported in the English literature.

A case report is presented of one preeclamptic patient with rupture of a big right subcapsular hematoma of the liver which was misdiagnosed with cholecystitis. A cesarean delivery was performed, which resulted is stillbirth, with abruptio placentae of 50%. The emergent treatment of the liver rupture was the right hepatic artery ligation, the postoperative development was favorable. Long term follow-up using ultrasound and liver function values were performed for 14 months and showed that the lesion has regressed with a normal hepatic imaging.

Polyarteritis nodosa (PAN) is a serious systemic vasculitis, which untreated has a five year survival rate of less than 15%. In the present case PAN was diagnosticed in a 72-year old Asian woman by microscopy of a gall bladder in acalculous cholecystitis. We conclude that PAN must be considered in patients with symptoms from several different organs and evidence of vascular involvement can be obtained by histological investigation of material from involved organs or mesenterial angiography.

Age alone does not increase the risk in biliary surgery. However, cholelithiasis in the elderly is often associated with an increased frequency of acute cholecystitis and cholangitis due to stone obstruction. The proportion of elderly people in the world is still growing, therefore, we can expect to see increasingly more complications of gallstones. Elective surgery for gallstone disease in all age groups is to be preferred to a policy of waiting for stone complications with the resultant risk of higher mortality and postoperative morbidity of emergency surgery.

Intraoperative cholangio-manometry with a miniature tensor sensor and graphic recording of the results were conducted to study the function of the major duodenal papilla in 53 patients. Of 17 patients who underwent heart transplantation in 1985-1988, three were later operated for reasons unrelated to the transplant. Two of them had acute cholecystitis and one haemothorax, due to rupture of the internal mammary artery during endomyocardial biopsy. Cholecystectomy was performed in the first two cases and thoracotomy in the third, with uneventful recovery in all cases. Early diagnosis and adequate surgical treatment are necessary in these cases. Consideration of the immunosuppressive medication is important during and after operation.

The authors analyse the results of treatment of 255 patients with obstructive jaundice of nonneoplastic origin. Various endoscopic methods were used. Endoscopic interventions were carried out in the first two days in 79.8% of cases. Laparoscopic cholecystostomy was conducted in 138 patients; jaundice was relieved in 3 to 20 days in 95% of cases. It is shown that the best results of operative treatment of patients with acute cholecystitis complicated by choledocholithiasis are produced in concurrent use of laparoscopic cholecystostomy and endoscopic papillosphincterotomy.

Extramedullary hematopoiesis associated with fibrosis is found frequently in the liver and spleen, but seldom in other organs. Acute abdomen due to extramedullary hematopoiesis has been reported in two patients with intestinal obstruction because of heavy infiltration of the terminal ileum. This report describes the case of a 71-year-old woman with myeloid metaplasia involving the gallbladder mimicking acute cholecystitis. As far as we know, involvement of the gallbladder by extramedullary hematopoiesis has never been reported before.

Gas chromatographic procedure was used for quantitative estimation of metabolic [volatile fatty acids (C2-C6)] of anaerobic bacteria in express diagnosis of nonclostridial anaerobic infection in surgical patients with purulent bacterial destruction of lungs and with abdominal impairments as well as in patients with calculous cholecystitis. Only traces of acid metabolites were detected in donor blood. A 10-30-fold increase in their content in blood of patients with surgical and gynecologic sepsis enabled to diagnose the anaerobic form of the disease.

We report four patients with a congenital gallbladder septum whose symptoms resembled those of cholelithiasis, in one case giving rise to acute cholecystitis. Cholecystectomy relieved symptoms in all cases and examination of the operative specimen confirmed the clinical diagnosis and X-ray findings. Ultrasonography made a positive diagnosis in the last two cases and no stones were found in any of the cases described. Cholecystectomy is advocated in symptomatic patients with this condition, even when gallstones are not present.

The objective of this article was to study the interrelationships between long emotional stress and gastroenterological diseases (cirrhosis of liver, chronic hepatitis, chronic cholecystitis, ulcerous disease, chronic colitis, ulcerative colitis). The psychological well-being among Abkhazian refugees and their health problems have been investigated. It was found relationship between psychological problems and clinical outcomes in patients with gastroenterological diseases. Emotional stress contributes to gastroenterological illness.

Gallbladder (GB) perforation is a life-threatening complication of acute cholecystitis, and early diagnosis prevents delay in patient management. We present two cases of GB perforation diagnosed by color Doppler ultrasonography. Each clearly showed a flow signal passing through the perforated site, leading to prompt and successful surgical treatment. This interesting observation suggests that color Doppler ultrasonography is useful in the diagnosis of GB perforation.

We present a case of HELLP (haemolysis, elevated liver enzymes, low platelets) syndrome that developed post partum in 31 year old woman. It was initially misdiagnosed as acute cholecystitis. The disease was complicated with acute renal failure and acute respiratory failure. Admission to the intensive therapy unit became necessary; patient required haemodialysis and short lasting mechanical ventilation. The outcome of described case was nevertheless good.

A 65-year-old man on maintenance dapsone therapy for dermatitis herpetiformis for 30 years was admitted to hospital with acute abdominal pain and vomiting. Investigations revealed a Heinz body haemolytic anaemia. Worsening symptoms prompted an emergency laparotomy that revealed a perforated gall bladder with pigmented biliary calculi. In previous reviews of the haematological abnormalities associated with dapsone therapy, life-threatening cholecystitis has not been described.

A long-term control study after endoscopic papillotomy was performed in 102 patients at least 12 months after the procedure. 14 patients were no longer alive, from those only one patient died due to biliary problems. Complications occurred in 10 patients (9,8%). There was no cholangitis, but 2 cases of an acute cholecystitis some time after papillotomy. Therefore a cholecystectomy after papillotomy is suggested in the interval.
