Allergic Bronchopulmonary Mycosis Due to Schizophyllum commune Treated Effectively with Voriconazole.

A 63-year-old woman presented to our hospital for cough, sputum, and abnormal shadows on chest X-ray. Schizophyllum commune was isolated from mucous plugs. Positive specific IgE and IgG against the fungi, elevated serum IgE, and mucous plugs with typical histologic findings of allergic bronchopulmonary mycosis (ABPM) led to the diagnosis of ABPM due to S. commune. We initially administered itraconazole unsuccessfully. Changing the antifungal agent to voriconazole resulted in improvement of the symptoms and chest imaging findings. Her ABPM has not relapsed for two years since the cessation of v

Diabetic Retinopathy -Incidence And Risk Factors In A Community Setting- A Longitudinal Study.

AIM: To evaluate the natural history of diabetic retinopathy (DR) in diabetic patients and to assess long term risk for other chronic diseases associated with DR. METHODS: Retrospective, community-based study. Diabetics who underwent their first fundoscopic examination during 2000-2002, and had at least one follow- up examination by the end of 2007 were included. The primary outcome was the development of DR (proliferative diabetic retinopathy (PDR), non PDR (NPDR) or macular edema. Patients were followed for another 9 years for documentation of new diagnosis of related diseases. RESULTS: 51

Community perceptions and personal accounts of HIV discordance in rural western Kenya.

Among HIV-discordant couples, the literature is sparse regarding issues related to stigma, relationships and coping. Objectives were to explore: 1) perceptions about discordant HIV status; 2) understanding of HIV discordancy; 3) effects of discordancy on couples; and 4) adaptation and coping strategies for discordant HIV status. A survey was administered to 202 members of heterosexual discordant couples in rural western Kenya. In addition, to understand the community perspective, in-depth interviews (IDI) (n = 26) and focus group discussions (FGD) (n = 10) were conducted with community opinion

Implementing a Prison Medicaid Enrollment Program for Inmates with a Community Inpatient Hospitalization.

In 2011, North Carolina (NC) created a program to facilitate Medicaid enrollment for state prisoners experiencing community inpatient hospitalization during their incarceration. The program, which has been described as a model for prison systems nationwide, has saved the NC prison system approximately $10 million annually in hospitalization costs and has potential to increase prisoners' access to Medicaid benefits as they return to their communities. This study aims to describe the history of NC's Prison-Based Medicaid Enrollment Assistance Program (PBMEAP), its structure and processes, and pr

HIV/AIDS and the African-American Community 2018: a Decade Call to Action.

HIV/AIDS disproportionately affects African-Americans more than any other racial or ethnic group in the USA. Currently representing only 12% of the US population, African-Americans now comprise close to half of the total reported HIV/AIDS cases in the USA according to the Centers for Disease Control and Prevention since the initial reporting of HIV/AIDS. In this paper, we examined the prevalence and current direction of the HIV/AIDS epidemic in the African-American community especially in comparison to our first call to action in 2008. The situation remains dire and broader attention is necess

Assessment of Cardiovascular Health among Community-Dwelling Men with Incarceration History.

Returning to the community after incarceration is a particularly vulnerable time with significantly increased risk of death in the first 2 weeks. The elevated risk of death persists as long as 2 years, with cardiovascular disease (CVD) among the leading causes. African-Americans, especially African-American men, have higher rates of incarceration and community supervision (e.g., probation and parole) and an earlier onset of hypertension compared to Whites. Few studies have objectively assessed the cardiovascular health profile of criminal justice involved individuals. This study is designed to

A Statewide Model for Assisting Nursing Home Residents to Transition Successfully to the Community.

Minnesota's Return to Community Initiative (RTCI) is a novel, statewide initiative to assist private paying nursing home residents to return to the community and to remain in that setting without converting to Medicaid. The objective of this manuscript is to describe in detail RTCI's development and design, its key operational components, and characteristics of its clients and their care outcomes. Data on client characteristics and outcomes come from the Minimum Data Set, staff assessments of clients and caregivers, and Medicaid eligibility files. Most clients transitioned by the RTCI had ente

Approaches to communication assessment with children and adults with profound intellectual and multiple disabilities.

Communication assessment of people with profound intellectual and multiple disabilities (PIMD) has seldom been investigated. Here, we explore approaches and decision making in undertaking communication assessments in this group of people. A questionnaire was sent to UK practitioners. The questionnaire elicited information about assessment approaches used and rationales for assessment choices. Fifty-five speech and language therapists (SLTs) responded. Findings revealed that the Preverbal Communication Schedule, the Affective Communication Assessment and the Checklist of Communication Competenc

Communicating and enhancing teachers' attitudes and understanding of influenza using refutational text.

Influenza is an important public health issue and schools play an important part in preventing influenza outbreaks. However, little is known about teachers' knowledge and attitudes toward influenza and vaccination. The purposes of this project were to assess preservice and early career teachers' knowledge and attitudes of influenza, evaluate the effectiveness of a refutational text on increasing teachers' attitudes towards and knowledge of influenza and vaccination, and evaluate the effects of participant epistemology, motivation, and cognitive engagement on influenza attitudes and knowledge.

Circumstances of child deaths in Mali and Uganda: a community-based confidential enquiry.

BACKGROUND: Interventions to reduce child deaths in Africa have often underachieved, causing the Millennium Development Goal targets to be missed. We assessed whether a community enquiry into the circumstances of death could improve intervention effectiveness by identifying local avoidable factors and explaining implementation failures. METHODS: Deaths of children younger than 5 years were ascertained by community informants in two districts in Mali (762 deaths) and three districts in Uganda (442 deaths) in 2011-15. Deaths were investigated by interviewing parents and health workers. Investig

Psychological well-being in individuals living in the community with traumatic brain injury.

BACKGROUND: Well-being and quality of life issues remain a long-term problem for many individuals with traumatic brain injury (TBI). Meaningful activity is key to developing life satisfaction and a sense of contribution to society, yet individuals with TBI are often unable to return to competitive employment. OBJECTIVE: To describe the self-reported psychological well-being of a cohort of unemployed individuals living in the community at least 1 year post TBI with low life satisfaction. METHODS: Seventy-four unemployed individuals with low life satisfaction at least 1 year post TBI were admi

Childhood sexual abuse, mental health, and religion across the Jewish community.

Current estimates of childhood sexual abuse among Jews in the United States are only available for females and do not include a spectrum of religiosity. We examined sexual abuse, mental health, and religion, in a religiously diverse sample of male and female Jewish adults from North America, using a novel methodology to minimize sampling/response biases. A total of 372 diversely religious Jews participated. Prevalence of any form of childhood sexual abuse was statistically equivalent to national rates, except that females reported less involuntary penetration (OR = 0.53). All Jewish religious

Obstacles to mineralocorticoid receptor antagonists in a community-based heart failure population.

AIM: Previous studies and national assessments indicate an undertreatment of mineralocorticoid receptor antagonists (MRA) in heart failure with reduced ejection fraction (HFrEF). This study aimed to investigate why MRA is not used to full extent. METHODS: A complete community-based heart failure population was studied. Several variables were collected, and medical records were scrutinized to identify reasons for not prescribing MRA. RESULTS: Of 2029 patients, 812 had EF ≤40%. Five hundred and fifty-three patients (68%) tried MRA at some point but 184 of these (33%) discontinued therapy. Ther

Environmental Risks and Children's Health in a Mayan Community from Southeast of Mexico.

BACKGROUND: The World Health Organization (WHO) estimates that 25% of global morbidity and one-third of childhood morbidity may be attributable to environment. Low and high-income countries displayed different environmental risks. Mexico demonstrates the necessity for creating a national environmental health program. In southeastern Mexico, Tixméhuac, is a Mayan community with a high marginalization degree and social backwardness. The main childhood morbidities are acute and chronic diseases. OBJECTIVE: The aim of the study was to recognize environmental risks for children's health in Tixméha

Biodegradation of oxytetracycline and enrofloxacin by autochthonous microbial communities from estuarine sediments.

This work investigated the potential of microbial communities native to an estuarine environment to biodegrade enrofloxacin (ENR) and oxytetracycline (OXY). Sediments collected from two sites in the Douro river estuary (Porto, Portugal) were used as inocula for the biodegradation experiments. Experiments were carried out for one month, during which ENR and OXY (1 mg L-1) were supplemented individually or in mixture to the cultures at 10-day intervals. Acetate (400 mg L-1) was added to the cultures every 3 days to support microbial growth. A series of experimental controls were established in p

Penicillin Trunk Injection Affects Bacterial Community Structure in Citrus Trees.

Huanglongbing (HLB), caused by Candidatus Liberibacter asiaticus (CLas), an uncultured α-proteobacterium, is the most destructive disease of citrus trees worldwide. In previous studies, trunk injections of penicillin reduced CLas titers and HLB symptoms in citrus. However, antibiotic effects on the whole plant microbial community, which include effects on taxa that interact with CLas, have not yet been addressed. In this study, we investigated the effects of penicillin injection (0, 1000, and 6000 mg L-1) on rhizospheric and endophytic bacterial communities of grapefruit trees in field and gre
