Traumatic optic neuropathy. A case report.

A case of visual loss following cranio-maxillofacial trauma is reported. The patient had a sudden partial blindness associated with a fracture of the roof, medial and lateral orbital walls. Access to the orbit was achieved through a transethmoidal approach using the Howarth-Lynch medial incision and resecting the bone fragments which impinged on the optic nerve. The patient had total return of visual acuity, without surgical complications. The role of orbital and optic decompression in the management of patients with traumatic optic neuropathy is discussed. Its indications are controversial an

[Prenatal screening in utero of urinary abnormalities. Management].

The major benefit of antenatal ultrasonography is to allow early diagnosis of urinary tract malformations before postnatal infection worsens the prognosis. In the majority of cases, there is a unilateral and moderate dilatation requiring a check-up only during the second week of life; treatment subsequently depends on the etiology of the uropathy and the consequences on the kidney. Unilateral severe dilatations and bilateral, but moderate, dilatations must be managed immediately after birth in a pediatric surgical center. In the rare cases of severe bilateral obstructive uropathy, in utero dec

Vestibular neurectomy and saccus decompression surgery in Meniere's disease.

Results of vestibular neurectomy, total eighth nerve section, and saccus decompression in 42 patients with Meniere's disease are reported. Vestibular nerve section was found in isolated cases to be a very effective method of abolishing the symptom of vertigo. Hearing is not affected but may be lost owing to opening of the vertical canal or disruption of blood supply. Saccus surgery might be the surgical treatment of choice in early cases with good hearing but in patients with fixed non-fluctuating hearing loss, rehabilitation can be effected only by vestibular neurectomy. In bilateral cases ei

[Method of combined surgical treatment of compression forms of lumbar osteochondrosis with alloprosthesis of damaged intervertebral disks].

A method of surgical treatment of compressive forms of osteochondrosis of the lumbar portion of the vertebral column is described, the said method being based on the employment of a prosthetic replacement of the intervertebral disks with a promptly hardening polyurethane SKU-PFL. Along with the decompression of nervous and vascular structures in the vertebral canal the introduced technique permits to correct the stato-dynamic incompetance of the spine. 62 patients were followed-up for up to 5 years. Better results were noted in patients with a clinical pattern of radicular compression. A less

[Debatable questions concerning the surgery of lumbar osteochondrosis].

Three tendencies exist at present in the surgical management of lumbar osteochondrosis: orthopedic treatment aimed at stabilizing the vertebral segment (the procedure of choice being anterior total disectomy with vertebral intercorporal spondilodesis), neurosurgical treatment striving to decompress the nervous structures clinched by the disc, osteal growth, scars, and a combined management achieving both of the above purposes. Proceeding from an analysis of two thousand operations performed within 20 years the author concludes that each of the mentioned methods has its merits and indications d

Intraoperative Lower Extremity SSEP Absence from Over-Tightened Safety Strap: Case Report.

Somatosensory evoked potentials (SSEPs) are used in spinal surgeries to monitor the functional integrity of dorsal column medial lemniscal pathways. Changes to waveform amplitudes can be due to surgical manipulation, in addition to many outside factors. We describe a posterior lumbar decompression/fusion case in which lower extremity (LE) SSEPs were performed and deteriorated bilaterally. These changes were found to be related to a safety strap traversing the posterior thighs that was excessively tight, causing neurological dysfunction. After loosening the strap, SSEP amplitudes and latencies

Arthroscopy-Assisted Core Decompression Combined with Octacalcium Phosphate/Gelatin Composite Implantation for Osteonecrosis of the Femoral Head: A Study Protocol for a Single-Center Externally Controlled Trial.

Background/Objectives: Osteonecrosis of the femoral head is a progressive disease that frequently leads to femoral head collapse and secondary osteoarthritis. Although total hip arthroplasty provides reliable outcomes, its use in younger patients is limited due to concerns regarding implant longevity. Joint-preserving procedures such as core decompression have been widely used; however, their efficacy remains controversial. This study aims to evaluate a combined approach using arthroscopy-assisted core decompression and an osteoconductive bone substitute. Methods: This study is designed as a s

Lumbar paraspinal compartment syndrome with concomitant rhabdomyolysis and multifidus necrosis.

Acute lumbar paraspinal compartment syndrome is a rare, function-threatening emergency often misidentified as benign musculoskeletal strain. We describe a 29-year-old male who presented to the emergency department (ED) with acute low back pain following a touch football game. Despite an initial discharge for muscle spasm, he returned 24 h later with refractory pain and laboratory evidence of rhabdomyolysis (creatine kinase 14,399 U/L). Magnetic resonance imaging revealed edema of the left paraspinal musculature. Due to persistent pain and progressive laboratory abnormalities, emergent surgical

Intraoperative neuromonitoring in degenerative cervical myelopathy surgery: a systematic review and meta-analysis of diagnostic accuracy and postoperative neurological outcomes.

OBJECTIVES: Degenerative cervical myelopathy (DCM) often requires surgical decompression but carries a risk of intraoperative neural injury. Intraoperative neurophysiological monitoring (IONM) is widely used to detect impending spinal cord compromise during such surgery, yet its impact on postoperative neurological outcomes remains debated. This systematic review and meta-analysis evaluated (1) the diagnostic accuracy of IONM modalities - somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), and multimodal monitoring - and (2) the effect of IONM versus no IONM on postoperative

Recurrent sigmoid volvulus in pregnancy.

Intestinal volvulus during pregnancy is a rare and potentially severe cause of bowel obstruction, associated with high maternal and fetal morbidity and mortality. Diagnosis is challenging due to nonspecific symptoms, requiring a high index of suspicion. We report a primigravida in her late 30s diagnosed with sigmoid volvulus at 25 weeks of gestation, initially treated successfully with endoscopic decompression. Recurrence occurred at 36 weeks, leading to an urgent caesarean section with sigmoidopexy, achieving excellent maternal and fetal outcomes. This case emphasises the diagnostic challenge

Endoscopic lumbar decompression in patients with obesity does not correlate with inferior outcomes.

PURPOSE: Endoscopic spine surgery (ESS) may reduce perioperative risks traditionally associated with obesity in open lumbar procedures. This study evaluated 90-day complications, healthcare utilization, and insurer-payments in obese versus non-obese patients undergoing endoscopic lumbar decompression. METHODS: A national administrative database was queried for adults undergoing endoscopic lumbar decompression from January 2017 to April 2023. Patients with trauma, neoplasm, or infection within 90-days preoperatively or < 90-day follow-up postoperatively were excluded. Cohorts were stratified b

Vacuum-assisted closure of the scalp flap following malignant cerebral edema during craniotomy: pediatric case presentations and review of the literature.

Intraoperative malignant cerebral edema is a rare but life-threatening complication that can arise during a traumatic decompressive craniectomy. The swelling may be significant enough to preclude skin closure. The current standard of care is to resect sufficient edematous brain tissue to close the scalp to establish an immune-competent barrier to the intracranial space. We report two pediatric patients who developed malignant edema during craniectomy that prevented skin closure. Both patients underwent placement of a temporary vacuum-assisted closure (VAC) device, facilitating resolution of th

Biomechanical performance of a novel zero-profile interbody cage: A cadaveric study.

Zero-profile cage (ZPC) products have been widely used in anterior cervical decompression and fusion (ACDF) surgery. To develop a ZPC that meets the biomechanical requirements of the Chinese population, we designed a novel zero-profile cage (NZ) by analyzing the critical anatomical parameters of the cervical spine in healthy Chinese people. This study aims to investigate and assess whether the biomechanical properties of the newly designed NZ could satisfy the criteria for clinical application. The biomechanical properties of the NZ were evaluated by being implanted into cervical cadaveric spe

Influence of decompression surgery on sagittal balance parameters in patients with lumbar spinal stenosis.

In this study we investigated the effect that lumbar decompression for lumbar spinal stenosis (LSS) has on sagittal balance and its clinical significance. This was an observational cohort study for LSS cases treated with decompression surgery. Core Outcome Measures Index (COMI), EuroQoL (EQ-5D) and Oswestry Disability Index (ODI) were used preoperatively and at 1 year follow-up. Pelvic incidence (PI), pelvic tilt (PT), sagittal vertical axis (SVA) and lumbar lordosis (LL) were measured before and 1 year after surgery. Hierarchical clustering (HC) was performed to identify subgroups with distin

Posterior fossa decompression in syndromic children with chiari-like posterior fossa crowding: a nationwide US-based study.

INTRODUCTION: Posterior fossa crowding, due to cerebellar tonsil herniation, often requires surgery with posterior fossa decompression (PFD). Although most cases are due to a Chiari-1 malformation (CM1), some are due to concomitant congenital conditions, mimicking a radiological CM1. The aim of this study was to compare PFD outcomes between CM1 and the syndromic Chiari-like crowding of the posterior fossa. A national pediatric surgical database was used to compare baseline characteristics and short-term postoperative outcomes. METHODS: Pediatric patients undergoing PFD (2012-2021) were identi

Upper-limb neurovascular compression, pectoralis minor and quadrilateral space syndromes: A narrative review of current literature.

Pectoralis minor syndrome (PMS) and quadrilateral space syndrome (QSS) are uncommon neurovascular compression disorders affecting the upper extremity. PMS involves compression under the pectoralis minor muscle, and QSS results from compression in the quadrilateral space-both are classically observed in overhead-motion athletes. Diagnosing PMS and QSS may be challenging due to variable presentations and similarities with other, more common, upper-limb pathologies. Although there is no gold standard diagnostic, local analgesic muscle-block response in a patient with the appropriate clinical cont
