Critical analysis of supracostal access for percutaneous renal surgery.

PURPOSE: Percutaneous renal surgery is currently performed for complex renal calculi as well as for various other endourological indications. In many patients an upper pole nephrostomy tract allows direct access to most of the intrarenal collecting system. Upper pole percutaneous access may be obtained via the supracostal or subcostal approach. The preferred route depends on the location and size of the specific stone or lesion. Previously others have cautioned against the supracostal approach above the 12th rib and many have discouraged an approach above the 11th rib due to concern about the

A new species of Microeledone from Galápagos Islands and an amended diagnosis of the Megaleledonidae (Octopoda: Incirrata).

The octopod fauna of the deep tropical Pacific Ocean remains poorly known, as is the octopod family Megaleledonidae Taki. A single female megaleledonid specimen collected at 1773 m depth near the equatorial Galápagos island of Darwin is named Microeledone galapagensis sp. nov. This small, squat, short-armed octopod has few arm suckers and gill lamellae. Its lack of a crop diverticulum, ink sac, and anal flaps suggests that it pertains to Thaumeledone. However, its smooth skin, which dorsally is nearly free of pigment, large rachidian tooth, and large funnel organ ally it with the monotypic Mic

Delayed Clinical Onset of Congenital Heart Disease in a Geriatric Macaw (Ara ararauna).

An approximately 33-year-old male blue and gold macaw (Ara ararauna) was presented for postmortem examination after dying without premonitory signs at the owner's home. The bird had a recent history of a grade II/VI left-sided systolic murmur, feather-destructive behavior, was overweight, and had bilateral cataracts. Gross and microscopic postmortem examinations revealed a subaortic interventricular septal saccular defect most compatible with a diverticulum intimately associated with a multifocally fibrotic right coronary aortic semilunar cusp, and its distended sinus. The resulting congestive

Thoracotomy for Esophageal Pseudodiverticulum Following Cardiac Surgery.

Esophageal pseudodiverticulum (EPD), a rare benign condition, is typified by the presence of outpouching flask-shaped lesions across the esophagus. EPD, often secondary to a motility dysfunction, is associated with various condition, however, has not be reported following mitral valve replacement (MVR). We report a case of a 45-year-old male with progressive dysphagia to solids and liquids over 1 month, along with regurgitation and vomiting. Twelve months ago, he had undergone thoracotomy for MVR. Physical examination revealed signs of malnutrition and epigastric tenderness. Upper gastrointest

A rare concurrence of acute appendicitis with Meckel's diverticulitis: a case report.

BACKGROUND: Meckel's diverticulitis is the inflammation of Meckel's diverticulum (MD), a common congenital structural abnormality of the gastrointestinal tract. The simultaneous occurrence of Meckel's diverticulitis with acute appendicitis is extremely rare. CASE PRESENTATION: A 17-year-old male presented with acute abdominal pain and vomiting, accompanied by tenderness in the right iliac fossa. Laboratory investigations revealed neutrophilia and an elevated leukocyte count, while ultrasound findings were consistent with acute appendicitis. The patient underwent an emergency open appendectomy

Spontaneous intracranial hypotension: searching for the CSF leak.

Spontaneous intracranial hypotension is caused by loss of CSF at the level of the spine. The most frequent symptom of this disorder is orthostatic headache, with the headache worsening in the upright position and subsiding after lying down. Neuroimaging has a crucial role in diagnosing and monitoring spontaneous intracranial hypotension, because it provides objective (albeit often subtle) data despite the variable clinical syndromes and often normal lumbar puncture opening pressure associated with this disorder. Spine imaging aims to classify and localise the site of CSF leakage as either (1)

A case report: Actinobaculum suis infection associated with formation of pyogranuloma, epididymitis and azoospermia in a boar.

BACKGROUND: Actinobaculum suis is a bacterium known to cause infections of the urogenital tract of sows. Infection can occur through close contact to boars, who frequently carry the pathogen in their preputial diverticulum but do not become clinically diseased themselves. In the current case, Actinobaculum suis was isolated from pyogranuloma of inflamed epididymis in a boar with poor fertility. CASE PRESENTATION: Increased return to oestrus rate, which worsened after the purchase of a new boar, was reported in an organic farm in Switzerland. During herd examination, azoospermia of the boar wa

Adult Ileocolic Intussusception Caused by Diffuse Large B Cell Lymphoma.

Intussusception is a medical condition, in which a proximal part of the intestine folds into the distal intestine. Adult intussusceptions are rare and account for approximately 5% of all cases of intussusceptions. The anatomical leading points include tumors, diverticulums, polyps, and strictures in 80-90% of adult intussusceptions, and 65% of colon intussusceptions and 30% of small bowel intussusceptions originate from malignant tumors. Treatments for adult intussusception have not been established, but most cases require surgical treatment. The gastrointestinal tract is the most common extra

The manifestation of vortical and secondary flow in the cerebral venous outflow tract: An in vivo MR velocimetry study.

Aberrations in flow in the cerebral venous outflow tract (CVOT) have been implicated as the cause of several pathologic conditions including idiopathic intracranial hypertension (IIH), multiple sclerosis (MS), and pulsatile tinnitus (PT). The advent of 4D flow magnetic resonance imaging (4D-flow MRI) has recently allowed researchers to evaluate blood flow patterns in the arterial structures with great success. We utilized similar imaging techniques and found several distinct flow characteristics in the CVOT of subjects with and without lumenal irregularities. We present the flow patterns of 8

Sequential transurethral enucleation of the prostate and laparoscopic bladder diverticulectomy.

Transurethral endoscopic technique and standard laparoscopic technique are surgical options for the management of benign prostatic hyperplasia (BPH) associated with urinary bladder diverticuli (UBD). In this article, we report laparoscopic diverticulectomy (LD) and transurethral plasmakinetic enucleation of the prostate (TUEP) in the same patient sequentially. To the best of our knowledge, this is the first case report of LD combined with TUEP. An 82-year-old patient with benign prostatic hyperplasia and two secondary large bladder diverticuli underwent sequential TUEP and LD. After completion

Periurethral cystic mass misdiagnosed: a case report.

BACKGROUND: Periurethral cystic masses, including Skene's duct cyst and urethral diverticula, are rare and are often misdiagnosed for more common vulvovaginal cysts. Given their proximity to the urethra, correct diagnosis is needed for appropriate referral and surgical management to prevent complications, including fistula formation. CASE: A 36-year-old woman was diagnosed by her primary care physician with a Bartholin's gland cyst, which was confirmed on MRI. She was referred to a gynecologist for incision and drainage. However, on examination the cyst was noted to be more anterolateral on t

Neuroendocrine tumors arising in Meckel's diverticula: frequency of advanced disease warrants aggressive management.

BACKGROUND: Meckel's diverticulum is a common anomaly of the GI tract, which occasionally gives rise to cancer. The most frequent tumors affecting these diverticula are neuroendocrine tumors (NETs), and whether these should be treated in similar fashion as small bowel NETs or appendiceal NETs is unclear. METHODS: A retrospective chart review was conducted at a single academic medical center between 1998 and 2012. Demographic, radiologic, biochemical, and clinicopathologic data were collected as well as status at last follow-up. RESULTS: Seven patients were identified with NETs involving Meck

Prevalence and clinical implications of diverticulosis of the vermiform appendix.

OBJECTIVE: The epidemiology and the aetiology of inflammatory diseases of the vermiform appendix remain poorly understood. The prevalence of appendiceal diverticulosis and diverticulitis in patients undergoing appendectomy for suspected acute appendicitis was investigated. METHODS: A retrospective study was completed on patients who underwent appendectomy for suspected acute appendicitis. Pathology reports of all patients were screened for diverticula of the vermiform appendix. Patients with either diverticulitis of the vermiform appendix or normal appendicitis were compared. RESULTS: Out of

No protection in chickens immunized by the oral or intra-muscular immunization route with Ascaridia galli soluble antigen.

In chickens, the nematode Ascaridia galli is found with prevalences of up to 100% causing economic losses to farmers. No avian nematode vaccines have yet been developed and detailed knowledge about the chicken immune response towards A. galli is therefore of great importance. The objective of this study was to evaluate the induction of protective immune responses to A. galli soluble antigen by different immunization routes. Chickens were immunized with a crude extract of A. galli via an oral or intra-muscular route using cholera toxin B subunit as adjuvant and subsequently challenged with A. g

Congenital band of the vitelline artery remnant as a cause of chronic lower abdominal pain in an adult: Case report.

INTRODUCTION: Intraabdominal bands of the vitelline vessel remnant are the rarest form of congenital mesodiverticluar bands which may or may not be associated with Meckel's diverticulum. In the majority of cases they cause an acute abdominal disease such as intestinal obstruction, especially in children. PRESENTATION OF CASE: We report a case of a 64 year old gentleman who experienced recurrent episodes of abdominal distension and bloating over two years. Computed tomography of his abdomen, colonoscopy, and barium follow through were all normal. Diagnostic laparoscopy revealed a single band a

[Female urethral diverticula--clinical and surgical aspects].

Urethral diverticula are a common cause of chronic genitourinary symptoms in women. They occur in 5% of women overall, with higher frequencies in selected populations of symptomatic women. The classical presentation is with recurrent urinary tract infections and post-micturition dribbling but almost any urinary symptom may be a presenting feature. Newer imaging modalities such as magnetic resonance imaging and perineal ultrasound are now widely available and urethral diverticula, that were previously unrecognized, can now be more easily detected. However, despite the availability of effective
