Impacting Rare Diseases Healthcare by Case-Based Telelearning: Description of the Dutch Pilot Metabolic Diseases ECHO.

Inherited metabolic diseases (IMDs) are individually (ultra)rare but collectively relatively common. The Extension for Community Healthcare Outcomes (ECHO) model has been used worldwide since 2003 to disseminate knowledge and best practices in chronic diseases. In 2024, the Dutch Society for Inborn Errors of Metabolism (ESN) launched a pilot Metabolic Diseases ECHO Program. We aimed to evaluate whether ECHO is a feasible model for IMD knowledge dissemination. We conducted a retrospective review of the design, implementation, and impact of the program. Since February 2025, two ECHO hubs have be

From Benchmarking to Intervention: Using SECOND Trial Data to Improve Residency Culture.

Burnout and mistreatment remain persistent challenges in surgical residency, with wide-ranging consequences for both resident well-being and patient care. Much of the existing literature describes the prevalence and causes of resident burnout, but fewer studies provide practical insight into how programs can effectively address it. In 2019, our program enrolled in the Surgical Education Culture Optimization through Targeted Interventions based on National Comparative Data (SECOND) Trial, a national benchmarking initiative that measures resident wellness, work environment, and mistreatment rela

Non-surgical management of knee osteoarthritis: a stepped conservative care paradigm for early-stage disease and high-risk populations.

Knee osteoarthritis (KOA) is a leading cause of chronic pain and physical disability in the aging global population, placing an immense burden on healthcare systems. Despite its rising incidence, clinical practice and research remain disproportionately focused on late-stage joint replacement, neglecting early-stage preventive and conservative care for high-risk individuals. To address this gap, this narrative review searched major databases including PubMed/MEDLINE, Embase, Web of Science, and the Cochrane Library for clinical guidelines, high-quality systematic reviews, and randomized control

The perceived facilitators and barriers to the potential adoption of teledentistry among dental care providers in Saudi Arabia: A systematic review.

Teledentistry has emerged as a tool that holds great potential to enhance access, efficiency, and continuity of oral healthcare delivery. In Saudi Arabia, its adoption is important in the context of the health transformation programme and Vision 2030, particularly given the high prevalence of dental caries among schoolchildren. Despite this potential, actual implementation is still limited. In this context, there is a need to systematically assess the factors influencing its potential adoption from the care providers' perspectives. This review aimed to systematically identify how dental care p

Policy recommendations for overdose response hotlines and applications as a harm reduction tool: A commentary.

SETTING: The ongoing drug poisoning crisis continues to cause significant mortality, with a disproportionate number of overdose deaths occurring when individuals use drugs alone. While supervised consumption sites (SCS) have proven effective in reducing overdose fatalities, their impact is limited by geographic, social, and systemic barriers. In response, overdose response technologies have emerged to expand access to life-saving interventions beyond the reach of traditional harm reduction infrastructure. INTERVENTION: Overdose response technologies (e.g., National Overdose Response Service (

Embracing Large Language Models for Medical Applications, Part II: Building a Framework for Clinical Stewardship.

Large language models (LLMs) have moved rapidly from experimental demonstrations to early clinical, educational, and administrative use. The first phase of medical LLM discourse emphasized opportunities in transfer learning, domain adaptation, clinical decision support, education, privacy, fairness, and regulation. The next phase requires a more specific framework for clinical stewardship. Since 2023, medical LLMs have demonstrated substantial progress in medical question answering, diagnostic reasoning in interactive clinical dialogue, patient communication, documentation support, and retriev

Selective intraoperative cholangiography: A retrospective single-centre analysis compared with national data and implications for surgical training.

PURPOSE: Intraoperative cholangiography (IOC) can assist in visualising the biliary anatomy during laparoscopic cholecystectomy, particularly in cases with unclear anatomical conditions, and may thereby help to prevent complications such as bile duct injury. Despite its potential relevance, the method is infrequently used in many hospitals and is often not included in surgical training curricula. The aim of this study was to quantify the selective, on-demand use of IOC in a specialised visceral surgery centre, compare local data with national statistics, and interpret the findings in the conte
