Resection arthroplasty for radial head fractures: Long-term follow-up.

BACKGROUND: Complex radial head fractures treated by early resection arthroplasty yield good clinical results at long-term follow-up. METHODS: Fifty-one radial head excisions were performed for the treatment of radial head fractures. Twenty-seven patients (20 men, 7 women) were retrospectively reviewed. The mean age was 37 (range, 18-61) and their fracture types according to the Mason classification were 5 type II, 16 type III, and 6 type IV. The mean follow-up period was 17 years (range, 10-24). Patients were clinically evaluated: Mayo Elbow Performance Score (MEPS), Disabilities of the Arm,…

[Bilateral floating forearm: a case report].

The combination of elbow dislocation and perilunate dislocation is rare. The prognosis of this condition depends mainly on that of the wrist. While some cases of floating forearm have been reported, no bilateral affection has -of yet- to our knowledge, been published. The authors report a case of a young patient who presented with bilateral floating forearm after a fall from a height. The treatment was surgical at the wrist. The functional result is similar to cases reported in the literature.

Troublesome heterotopic ossification after central nervous system damage: a survey of 570 surgeries.

The hip was the primary site of HO (328 [60.9%]), then the elbow (115 [21.3%]), knee (77 [14.3%]) and shoulder (19 [3.5%]). For all patients, 181 of the surgeries were performed within the first year after the CNS damage, without recurrence of HO. Recurrence was not associated with etiology (p = 0.46), sex (p = 1 …

Positive identification by a skull with multiple epigenetic traits and abnormal structure of the neurocranium, viscerocranium, and the skeleton.

Wormian bones are small ossicles appearing within the cranial sutures in more than 40% of skulls, most commonly at the lambdoid suture and pterion. During the skeletal analysis of an unidentified male war victim, we observed multiple wormian bones and a patent metopic suture. Additionally, the right elbow was deformed, probably as a consequence of an old trauma. The skull was analyzed by cranial measurements and computerized tomography, revealing the presence of cranial deformities including hyperbrachicrania, localized reduction in hemispheral widths, increased cranial capacity, and sclerosis…

[Possibilities of orthopaedic treatment of patients with renal failure and its meaning in the continuation of haemodialysis and quality of life improvement].

Good cooperation between the fields of orthopaedic surgery and nephrology creates a possibility of multidirectional surgical treatment of the patients with terminal renal failure despite massive general burden and existing risk. Taking a very difficult decision concerning the multidirectional surgical treatment of these patients may to a very great extent improve their locomotor functions and their life comfort. An example of such a situation is the described case of elbow replacement, vascular surgery procedure, total hip replacement and carpal tunnel release surgery.

The Essex-Lopresti lesion: a variant with a bony distal radioulnar joint injury.

The Essex-Lopresti lesion is an unusual injury, consisting of a radial head or neck fracture, distal radioulnar joint (DRUJ) injury and interosseous membrane rupture. To date, all reported Essex-Lopresti lesions have consisted of soft tissue injuries at the DRUJ. We present a case of an Essex-Lopresti lesion with a bony variant, in which the DRUJ injury consisted of an ulnar head fracture associated with radial head fracture and acute proximal migration of the radius. The management involved plating of the ulnar head fracture and titanium replacement of the radial head.

[Delayed radial paralysis after Monteggia fracture--a case report].

Concomittant lesions of neural structures represent a rare type of complications in Monteggia's fractures. In acute fractures spontaneous neurological remission usually occurs after reduction of the dislocated radial head. In the presented case a 33-year old man experienced a trady palsy of the posterior interosseus nerve 27 years after a Monteggia's fracture with the radial head left dislocated. Following a minimal trauma in badminton a neurological deficiency probably caused by distraction occurred and resulted in impairment of wrist extension and extension of the fingers. Initiated conserva

Fasciocutaneous flaps based on fascial feeding vessels for defects in the periolecranon area.

Skin defects over the olecranon process are usually a result of wound dehiscence following trauma or following joint replacement. Rarely, breakdown occurs over the olecranon in degenerative joint diseases and as the result of pressure necrosis. We have used a fascial feeding vessel-based fasciocutaneous flap to reconstruct 10 defects in nine patients. The technique provides a thin and supple local flap that is easy to raise and is reliable.

Interlocking wedge osteotomy for cubitus varus deformity.

The authors reviewed 20 patients with cubitus varus deformity who were treated with the interlocking wedge osteotomy. All deformities includ … degrees of valgus, compared to 9.4 degrees of valgus on the uninjured side. The mean flexion of the elbow increased from 123.9 degrees to 134.3 degrees, and the mean hyperextension decreased from 12.0 degrees to 7.5 degrees. Three-dimensional correction and stable fixation can be achieved with this osteotomy.

MRI of clear cell hidradenoma.

Clear cell hidradenoma is a rare benign skin appendageal tumor whose imaging findings have been even more rarely reported. We experienced a large clear cell hidradenoma of the left elbow in a 22-year-old man. MRI revealed a complex solid and cystic appearance with enhancing solid component.

Septic arthritis and osteomyelitis due to Bordetella petrii.

A case of Bordetella petrii septic arthritis and osteomyelitis in an elbow resulted from a dirt bike accident in Hawaii. Two months of intravenous antibiotics and repeated surgeries were required to cure this infection. Our case, and literature review, suggests that extended-spectrum penicillins, tetracycline, and trimethoprim-sulfamethoxazole are good treatment options.

Osteochondral lesions of the talus.

Osteochondral lesions of the talus (OLT) are rare joint disorders. The talus is the third most common location of this disorder, following the knee and elbow joints. OLT represents 4% of all osteochondral lesions in the body. This article discusses the surgical treatment and postoperative rehabilitation of osteochondral lesions of the talus.

Congenital Radial Head Dislocation: A Narrative Review of Etiology, Diagnosis, and Treatment.

Congenital radial head dislocation (CRHD) is an uncommon orthopedic anomaly affecting the elbow joint and is frequently associated with other congenital deformities. This narrative review synthesizes evidence identified through targeted searches of PubMed and Web of Science, with emphasis on peer-reviewed studies addressing the epidemiology, developmental and genetic mechanisms, diagnostic evaluation, clinical differentiation, management strategies, and treatment outcomes of CRHD. CRHD may present as either unilateral or bilateral dislocation, with posterior dislocation being the most frequent…

Superficial lateral arm flap for post-burn antecubital contractures.

This paper presents a new variant of the distally based lateral arm fasciocutaneous flap that involves a straightforward and easy dissection as no specific vessel identification or inclusion is needed. Ten fresh cadavers were dissected to study the vascular supply. All three recurrent arteries-radial, ulnar, and interosseus-nourish the flap. The study also identified muscular perforators from the radial recurrent artery, piercing brachioradialis, and, in addition, septocutaneous perforators from the ulnar and interosseous recurrent arteries entering the base of the flap. Clinical application i

Morphological characteristics of coronoid process and revisiting definition of coronoid hyperplasia.

The aim of this study was to assess the morphological characteristics of the coronoid process (CP) and define coronoid hyperplasia (CH) using cadaveric mandibles of a Caucasian population. A sample of 151 adult dry cadaveric mandibles (302 CPs) was acquired. Three distances were measured, which included the width, height, and length of CP. The surface area measurements involved area A: above the width distance line; area B: between incisura mandibulae-Alveolar ridge line and width distance line; area C: between distance lines of width and height. Finally, angulations of the CP and gonial angle
