Restricted dietary chloride with sodium bicarbonate supplementation for Holstein cows in early lactation.

Over 2 wk, beginning 3 wk postpartum, daily changes of feed and water intake and milk production and composition were monitored in 25 cows allotted randomly among four dietary treatment groups differing in sodium chloride, sodium bicarbonate, and ad libitum supplementation by salt block. The basic complete diet was 54% corn silage, 27% shelled corn, and 16% soybean meal, dry basis. The four dietary treatments were control, .42% chloride; low, .10% chloride; low plus salt block, complete ration same as low with ad libitum salt block consumption; and control plus .7% sodium bicarbonate. Differen

[Perioperative substitution therapy in congenital adrenogenital syndrome with salt loss].

Patients with an inborn error of adrenal steroid synthesis suffer from primary adrenocortical insufficiency requiring corticosteroid replacement therapy and appropriate stress coverage. The case report of a two year old infant with congenital adrenal hyperplasia of the salt-losing type demonstrates the consequences of an accidental steroid underdosage. The necessity of increased cortisol replacement therapy in case of anesthetic procedures--even of short duration and without surgical measures--is underlined. The details of adequate perioperative steroid and i.v. fluid therapy in pediatric pati

[Emergencies in geriatrics].

The more pronounced a so called polypathia is present in the aged, the earlier, the more often and/or the more severe is the tendency that an acute disease develops to an emergency situation to which it never would become per se. Pathophysiological aspects of outstanding importance for treating or preventing such emergency situations are discussed.

Syndrome of inappropriate ADH secretion concealed by hypokalaemia due to ectopic ACTH production.

A case is described of a patient with an oat cell carcinoma of the bronchus with moderately elevated levels of plasma corticotrophin (ACTH) and antidiuretic hormone (ADH). Ectopic secretion of ACTH induced severe hypokalaemia and concealed the effects of concomitant ADH secretion on renal function. Normal renal responsiveness was restored following correction of hypokalaemia. The hypokalaemia was associated with evidence of a marked increase in corticosteroid secretion but plasma ACTH concentrations did not show a proportionate elevation. Chromatographic studies on tumour extracts suggest that

Accidental systemic exposure to sodium hypochlorite (Chlorox) during hemodialysis.

A case of accidental exposure of a patient undergoing hemodialysis to a sodium hypochlorite solution is reported. A 61-year-old woman was co … alkaline degradation, or another mechanism. The sudden rise and fall in the concentrations of serum electrolytes and subsequent hyperkalemia was the most probable cause for the cardiac arrest. Cardiopulmonary resuscitation was started, the patient was intubated, given oxygen, sodium bicarbonate, atropine, dopamine, and isoproterenol. Sodium thiosulfate 5 g was …

[Uremia, hydroelectrolytic and acid-base changes in kidney failure of chronic hepatopathies].

The author presents 16 patients with chronic liver diseases that developed renal failure. The author emphasizes the picture characteristics as prolonged evolution of non-oliguric renal failure tending to the rise of serum sodium in 2 cases and fall of serum potassium and less seriousness of metabolic acidosis compensated by respiratory alkalosis, and a further patient with metabolic alkalosis and paradoxal acidury. The collection of these elements perhaps explain the evolution of the diseases in the patients without dialysing treatment measures. The clinic pictures should be considered as part

Hypersecretion of anti-diuretic hormone due to tuberculous meningitis.

Three patients with tuberculous meningitis complicated by hypersecretion of anti-diuretic hormone are described. The recognition of this complication is important in the treatment and prognosis of tuberculous meningitis.

Clonidine-induced diuresis in the rat: evidence for a renal site of action.

Clonidine, an alpha adrenergic agonist which causes a diuresis in experimental animals, was studied in unanesthetized, conscious Brattleboro rats heterozygous or homozygous for hereditary hypothalamic diabetes insipidus to determine if the diuresis was due to alpha adrenergic inhibition of antidiuretic hormone (ADH) release or to another mechanism of action. Heterozygous rats given clonidine s.c. in doses of 50 to 300 mu/kg b.w. exhibited a prompt dose-related diuresis. The diuresis was transient and could not be maintained beyond 4 hr even when clonidine was administered continuously by s.c.

Secondary hypertriglyceridemia and hyperlipoproteinemia in patients with primary asymptomatic gout.

We carefully selected 30 men with primary gout, rendered asymptomatic by therapy, to examine the frequency and type of hyperlipidemia and hyperlipoproteinemia, with the objective of determining whether serum uric acid, alcohol intake, liver function, kidney function, and (or) drugs were participating in the secondary lipid disorder. Sixty-one age- and sex-matched men were used as controls. About 73% of the gout patients had hypertriglyceridemia, 1.6-fold the frequency found in the control group. Types IV and IIb lipoprotein electrophoretic patterns were most prevalent in the gout group. Neithe

The effects of long-term oral treatment with chlorothiazide or furosemide on hereditary diabetes insipidus in rats.

The polyuria of homozygous Brattleboro (BB) female rats is halved when they are given chlorothiazide (about 250 mg/day) or furosemide (about 60 mg/day) orally for one day. The effect of chlorothiazide is still found after 16 days of treatment, whereas the effect of furosemide entirely disappears within 5 days. Both diuretics induce chronically increased plasma renin activity (PRA) and decreased natriuresis as long as they are added to the diet; the effect on Na is more evident during furosemide treatment. Urinary urea content as well as urinary osmolality are increased by chlorothiazide and "f

Malignant hyperthermia.

Malignant hyperthermia is now recognized as a distinct entity in anesthetic practice and can be considered as a pharmacogenetic disease of obscure etiology occuring in man and pigs with a dominant inheritance. A close association with myopathy has been noted. Commonly used muscle relaxants or anesthetic drugs can act as triggering agents in genetically susceptible patients, who develop a real hypermetabolic state, characterized by a rapid rise in body temperature, muscular rigidity, tachycardia and tachypnoea, cyanosis and severe respiratory and metabolic acidosis, the lethality being about 60

Effect of subacute manganese feeding on serotonin metabolism in the rat.

To clarify the effect of subacute manganese feeding on serotonin and mineral metabolism, Wistar rats were separated into two groups and fed two different diets, one a normal diet and the another a manganese-supplemented diet. After three weeks on these dietary regimens, the rats on the manganese-supplemented diet manifested the following abnormalities: blood pressure was decreased; brain serotonin was decreased; L-aromatic amino acid decarboxylase activity in brain was decreased. Manganese levels in heart, lung, and kidney increased, whereas sodium, potassium, magnesium and calcium levels in t
