Implementation of an evidence based guideline reduces blood tests and length of stay for the limping child in a paediatric emergency department.

INTRODUCTION: Acute non-traumatic limp is a common reason for children to present to the emergency department (ED). There is a wide differential diagnosis for these patients, and there are certain serious conditions which cannot be missed. An evidence based guideline for the 'limping child' was designed and the impact of guideline implementation on a number of specific, predefined quantitative outcomes was assessed. METHODS: An initial retrospective chart review over 3 months was carried out for all patients presenting with acute non-traumatic limp. Following guideline introduction and implem

Service-Level Utilization in Risk-Based Contracts and a Benchmarking Approach for Fee-Schedule Reforms.

IMPORTANCE: Despite revived interest in reforming the physician fee schedule, it is unclear how policymakers should modify more than 1000 different physician service prices to promote high-value care. One systematic approach could leverage service-level utilization patterns in full-risk contracts, which have not been previously characterized. OBJECTIVE: To compare service-level utilization patterns between health care professional organizations in full-risk contracts and those in a traditional fee-for-service (FFS) payment structure, adjusted for detailed patient-level differences, and to sim

Challenges During a Chlorine Gas Emergency Response.

OBJECTIVE: A chlorine gas release occurred at a poultry processing plant as a result of an accidental mixing of sodium hypochlorite and an acidic antimicrobial treatment. We evaluated the public health and emergency medical services response and developed and disseminated public health recommendations to limit the impact of future incidents. METHODS: We conducted key informant interviews with the state health department; local fire, emergency medical services, and police departments; county emergency management; and representatives from area hospitals to understand the response mechanisms emp

Maintaining health-system functionality in response to the surge of COVID-19 cases due to the Omicron variant, Japan.

PROBLEM: The Omicron variant of severe acute respiratory syndrome coronavirus 2 caused the largest surge of coronavirus disease (COVID-19) cases in Japan starting in the summer of 2022. We describe the mechanisms introduced to provide appropriate health care to all Omicron cases, provide appropriate health care to all non-COVID-19 patients, and protect health-care workers (HCWs) while providing necessary health services. Optimization of care for elderly patients was particularly important. CONTEXT: Japan is home to 125 million people, of whom 28.6% are 65 years or older. Between January and J

Association between screening for suspected COVID-19 cases and outcomes of patients revisiting the emergency department.

OBJECTIVES: Patients who revisit the emergency department (ED) shortly after discharge are a high-risk group for complications and death, and these revisits may have been seriously affected by the COVID-19 pandemic. Detecting suspected COVID-19 cases in EDs is resource intensive. We examined the associations of screening workload for suspected COVID-19 cases with in-hospital mortality and intensive care unit (ICU) admission during short-term ED revisits. STUDY DESIGN: We conducted a retrospective cohort study using electronic health record data from a tertiary teaching hospital. METHODS: We

Association of Shelter-in-Place Hotels With Health Services Use Among People Experiencing Homelessness During the COVID-19 Pandemic.

IMPORTANCE: Some jurisdictions used hotels to provide emergency noncongregate shelter and support services to reduce the risk of COVID-19 infection among people experiencing homelessness (PEH). A subset of these shelter-in-place (SIP) hotel guests were high users of acute health services, and the association of hotel placement with their service use remains unknown. OBJECTIVE: To evaluate the association of SIP hotel placements with health services use among a subset of PEH with prior high acute health service use. Clinical outcomes of admitted patients with COVID and an opioid overdose.

OBJECTIVES: To date, research on opioid overdose during the COVID-19 pandemic has focused on rates of Emergency Department (ED) visits related to opioids but has not considered how interaction with concurrent COVID-19 infection may have influenced clinical outcomes. We hypothesized that COVID-19 infection increased the need for respiratory support, prolonged hospital stays, and increased mortality among ED patients admitted to the hospital after presenting with opioid overdose. METHODS: The 2020 National Emergency Department Sample (NEDS) was used to identify patients admitted to the hospital

Outcomes for people experiencing homelessness with COVID-19 presenting to emergency departments in Canada, compared with housed patients.

BACKGROUND: Whether people experiencing homelessness (PEH) have different COVID-19 outcomes than housed patients in Canada remains unclear. We sought to ascertain whether rates of in-hospital mortality, hospital admission, critical care admission, and mechanical ventilation differed between PEH and housed people with symptomatic SARS-CoV-2 infection. METHODS: We conducted a propensity score-matched cohort study to compare the outcomes of PEH and housed patients presenting to emergency departments for acute symptomatic COVID-19. We used data from the Canadian COVID-19 Emergency Department Rapi

Increased 30-day mortality associated with weekend emergency admission for alcohol-related liver disease in England: a record-linkage study using the Clinical Practice Research Datalink.

OBJECTIVE: Deficiencies have been highlighted in acute hospital care for alcohol-related liver disease (ARLD). Such problems may be worse at weekends (WEs). Increased 30-day mortality for WE admissions has been reported for several acute conditions, but data for ARLD are limited. We aimed to compare patient and pathway characteristics between WE and weekday (WD) admissions and investigate the 'weekend effect' on mortality. METHODS: Retrospective cohort study (2008-2018) using linked electronic databases (Hospital Episode Statistics-Clinical Practice Research Datalink and death registration) i

Benefits of subspecialty adherence after asthma hospitalization and patient perceived barriers to care.

BACKGROUND: Comparative studies have demonstrated that asthma education to pediatric patients decreases average hospital usage and that allergy specialists provide stronger asthma education and more improved outcomes. OBJECTIVE: To evaluate the real-world benefits of allergy subspecialty involvement outside inpatient consultation and the impediments for patients in establishing allergy subspecialty care. METHODS: The study population was composed mostly of minority children 0 to 18 years old seen at a large university-affiliated stand-alone children's hospital who had a hospital discharge di

Potential impact of a computerized system to report late-arriving laboratory results in the emergency department.

BACKGROUND: Results of some laboratory tests for Emergency Department (ED) patients return hours to days after the patient is discharged. Inadequate follow-up for these late-arriving results poses medical and legal risks. We have developed, but not yet implemented, a computerized system called the Automated Late-Arriving Results Monitoring System (ALARMS). ALARMS scans the hospital's laboratory and ED registration databases to generate an electronic daily log of all late-arriving abnormal laboratory results for ED patients. OBJECTIVE: To determine the potential impact of ALARMS by assessing o
