From 1978 to 1988, microbiologically proven Pseudomonas osteochondritis and septic arthritis following nail puncture wound to the foot was diagnosed in 77 children aged 18 mo-19 y (77 and 17 cases, respectively). The syndromes were found in children with a history of wearing tennis shoes (70 cases), other shoes (5), and no shoes (2). Two-hundred foot venograms were performed in patients suspected of deep vein thrombosis (DVT). Eighty studies were positive for DVT in the calf, popliteal, superficial femoral, or common femoral veins. Of these, 16 (20%) had an associated venous thrombosis in the foot. One hundred and ten studies were negative. Of these, none had a positive foot venogram. It is therefore our feeling that an additional study of the foot need not be performed unless there are symptoms specifically referable to this area.

A series of four cases of refractory plantar ulcer in Charcot feet was successfully treated by a wide plantar exposure, excision of the ulcer, and primary closure over suction drainage. Thorough saucerization of the underlying convexity in the tarsals is thought to be necessary to achieve healing and prevent recurrence. Surgical intervention is considered worthwhile in those cases that show a marked rocker-bottom foot deformity on lateral radiography and have a persistent ulcer after conservative treatment in a plaster cast.

A family, of which thirteen members suffered from hereditary onycho-osteodysplasia (HOOD), is presented. Four of the thirteen patients also had foot deformities, and it is postulated that this anomaly may be a true part of the HOOD syndrome, and not just an incidental finding.

The most frequent referral diagnosis of childhood to an orthopaedic department is a limb rotational anomaly which manifests itself as either an abnormality of gait or flatfeet. The child is usually asymptomatic, the anxiety being generated in the minds of the parents. A clustering outbreak of hand, foot, and mouth disease (HFMD) occurred from July, 1981 to January, 1982 in Matsue City and Gotsu City, Shimane Prefecture. Massive trauma involving the foot was treated by external fixation in 33 cases and followed for 1 to 3 years. Amputations were performed in seven (21%) because of uncontrollable deep sepsis in two and massive injury in five. The other 26 (79%) had functionally acceptable feet at follow-up, but all had some stiffness and cosmetic deformity. Undecylenic acid and tolnaftate were tested in an in vitro test system to evaluate their relative "killing time" efficacy against Trichophyton mentagrophytes, Trichophyton rubrum, and Epidermophyton floccosum. Commercial products containing these active agents were tested similarly. The pure active agents were equivalent in activity. The commercial product containing undecylenic acid appeared to be more effective against the test organisms than did the product containing tolnaftate.

Congenital metatarsus varus may be regarded as a deformity caused by a dislocation and in that respect is similar to congenital clubfoot and vertical talus but not to calcaneovalgus. The dislocation causes adaptive bone changes as well as secondary contracture of the soft tissues. That the dislocation in the forepart of the foot may occur in a maximally dorsiflexed foot also may explain the valgus position of the hindfoot.
