Do Attachment Style and Emotion Regulation Strategies Indicate Distress in Predictive Testing?

Predictive genetic testing for a neurogenetic disorder evokes strong emotions, and may lead to distress. The aim of this study is to investigate whether attachment style and emotion regulation strategies are associated with distress in persons who present for predictive testing for a neurogenetic disorder, and whether these psychological traits predict distress after receiving test results. Self-report scales were used to assess attachment insecurity (anxiety and avoidance) and maladaptive emotion regulation strategies (self-blame, rumination, catastrophizing) in adults at 50 % risk for Huntin

A report of a patient with duplication of 7p13-->pter and deletion of 2p23-->pter due to a maternal 2p;7p translocation.

We report a patient with severe developmental delay, failure to thrive, microbrachycephaly, large anterior fontanel, ocular hypertelorism, broad nasal bridge, low-set ears, long philtrum, micrognathia, partial cleft palate, broad distal digits, abnormal palmar creases, joint contractures, and cardiovascular anomaly. Cytogenetic analysis with high resolution chromosome banding showed an unbalanced karyotype of 46,XX, der(2)t(2;7)(p23;p13) originating from a maternal balanced translocation. Our patient showed a duplication of 7p13-->pter and a deletion of 2p23-->pter. Our analysis suggests that

[Genetic causes of male infertility].

BACKGROUND: Male infertility is due to genetic factors in 15% of cases. Identification of genetic causes is important for both prognosis and treatment. The article gives an overview of the most frequent genetic causes of male infertility. MATERIAL AND METHOD: The article is based on literature retrieved through a search of Pubmed. RESULTS AND INTERPRETATION: Klinefelter syndrome, 47, XXY, and microdeletion in AFZ (azoospermia factor) c on the long arm of the Y chromosome are the most common genetic causes of male infertility. Both are present in about 10-15% of males with azoospermia. Other

Currarino syndrome shown by prenatal onset ventriculomegaly and spinal dysraphism.

The most frequent cause of ventriculomegaly is spina bifida when associated with Arnold-Chiari type II malformation. We report on a prenatal diagnosis of severe ventriculomegaly in association with spinal dysraphism that was indicative of a Currarino syndrome (CS) due to a c.584delA, p.H195fsX28 truncated mutation within the HLXB9 gene. Physiopathology of the ventriculomegaly is discussed in reference to the fetopathological examination and CS embryopathology. In the present case, prognosis was poor and pregnancy termination was authorized. However, such a decision may be controversial in fetu

Attitudes towards prenatal diagnosis and termination of pregnancy for thalassaemia in pregnant Pakistani women in the North of England.

OBJECTIVES: Most births of children affected with beta-thalassaemia major in the United Kingdom are to parents of Pakistani origin. A popular explanation for this is that Pakistanis decline termination of pregnancy on religious grounds. However, various factors influence people's attitudes towards prenatal diagnosis and termination of pregnancy, which have not been investigated in a UK Pakistani sample. This study is aimed at exploring the attitudes of pregnant Pakistani women towards prenatal diagnosis and termination of pregnancy for beta-thalassaemia major in the North of England. METHODS:

We are talking, but are they listening? Communication patterns in families with a history of breast/ovarian cancer (HBOC).

This study explores how family communication patterns and family scripts influence the dissemination of genetic information and the sharing of feelings about genetic inheritance in families of healthy women who have attended a cancer genetics risk clinic because of their family history of breast and, or ovarian cancer. Family scripts are sets of expectations, beliefs, and norms that assign meaning to patterns of interaction, connect generations and provide guidance for action. We conducted an exploratory, qualitative study at a major clinical and research cancer center in the United Kingdom fr

Transient neonatal diabetes, a disorder of imprinting.

Transient neonatal diabetes (TND) is a rare but distinct type of diabetes. Classically, neonates present with growth retardation and diabetes in the first week of life. Apparent remission occurs by 3 months but there is a tendency for children to develop diabetes in later life. Evidence suggests it is the result of overexpression of an imprinted and paternally expressed gene/s within the TND critical region at 6q24. Two imprinted genes, ZAC (zinc finger protein associated with apoptosis and cell cycle arrest) and HYMAI (imprinted in hydatidiform mole) have been identified as potential candidat

BACKGROUND: Idiopathic torsion dystonia is a clinically and genetically heterogeneous movement disorder. A GAG deletion at position 946 of t … the lack of family history was due to reduced penetrance. CONCLUSIONS: For accurate diagnosis and genetic counseling, screening for the DYT1 deletion is of great interest in cases with generalized dystonia without a family history. In other cases, positive results are rare.

Integration of family history and medical management of patients with hereditary cancers.

The family histories of individuals affected by a wide variety of cancers have provided information about the principal features of hereditary cancer. Surveillance protocols, indicating the most appropriate modalities and the age at which to initiate them, have been derived from what has been learned about the age of onset and the cancer sites associated with specific cancer syndromes. Likewise, cancer management has been based on what is known of the natural history of the syndrome cancers from studying multiple affected families. Family history has even been an essential tool for molecular g

Implications of prenatal diagnosis of sickle cell disease.

Prenatal diagnosis (PND) of sickle cell disease (SCD) has been feasible since about 15 years. The number of PND performed for SCD has constantly increased during these years, but its availability raises difficult ethical questions for parents and counsellors. Concerning at-risk parents, only 50% (data in the literature) to 70% (personal data) ask for PND. Our study shows that mainly cultural reasons, then religious ones, educational level and the number of children in the family weigh on the parents' decision to request this diagnosis. The counsellors' position is difficult since clinical seve

Ascertainment of familial ovarian cancer in the Aberdeen Genetic Clinic.

Ovarian cancer is the fifth most common malignancy in women in the UK. Patients with a family history including ovarian cancer make up nearly 15% of family cancer referrals to the Genetic Clinic in Aberdeen. To date, only one pedigree has been suitable for linkage studies, which has enabled us to target screening more accurately at those people at highest risk. Following discovery of a strong candidate for the BRCA1 gene, direct mutation testing may soon be possible. People who seek testing will require further counselling. Therefore we anticipate an increased demand on both clinical and labor

Risk estimates for balanced reciprocal translocation carriers--prenatal diagnosis experience.

An analysis was performed on 40 families at risk for an unbalanced rearrangement in the fetus because one of the parents is a reciprocal translocation carrier. The overall risk at second trimester prenatal diagnosis was 14% (8/57). The individual risk for unbalanced offspring at second trimester prenatal diagnoses and at birth were estimated using empirical data by Stengel-Rutkowski et al. (1988). The risks at birth ranged from 0%-21.6%. Most reciprocal translocations (22 or 55%) were at low risk. Without risk (7 or 17.5%), medium risk (6 or 15%) and high risk (5 or 12.5%) translocations were

A single centre study on birth of children with transfusion-dependent thalassaemia in Malaysia and reasons for ineffective prevention.

OBJECTIVE: We aimed to identify factors contributing to ineffective prevention of thalassaemia in Malaysia by studying events leading to the births of children with transfusion-dependent thalassaemia (TDT) including their parents' reproductive choices. METHODS: Thirty children with TDT born in the year 2005 to 2012 were included. Their parents underwent a semi-structured, face-to-face interview in 2012 and were followed up until 2014 to evaluate the actual decisions made during subsequent pregnancy. RESULTS: In 70% of the cases (21/30), neither parent was aware of their carrier status until

Prenatal diagnosis of Pallister-Killian syndrome: resolution of cytogenetic ambiguity by use of fluorescent in situ hybridization.

We report a case of Pallister-Killian syndrome initially diagnosed prenatally as tetrasomy 21. A 33-year-old primiparous woman was noted at 24 weeks' gestation to have moderate polyhydramnios. Ultrasonography showed diminished fetal stomach filling, hydronephrosis, and prominence of the cisterna magna. Cytogenetic analysis of cultured amniocytes was initially interpreted as mosaic tetrasomy 21: 46,XX/47,XX,+i(21q). The patient was then referred to our centre for genetic counselling. At 34 weeks' gestation, a dysmorphic infant was delivered and died within 30 min. Physical features were consist

Counseling prior to cfDNA screening: are we giving the right numbers?

OBJECTIVE: In this modeled study, we aim to evaluate the test performance of cfDNA screening for trisomy 21, 18, and 13 individually and in combination to help with pre-test counseling. METHODS: We used the German birth statistics 2023 and calculated the expected number of trisomy 21, 18, and 13 for each maternal age. The number of screen-positive tests (true and false) for trisomy 21, 18, and 13 was estimated based on maternal and gestational age-related risks for the three trisomies. The positive predictive value was calculated for each trisomy individually and in combination according to m

Multidisciplinary management of agenesis of corpus callosum: a narrative review.

The corpus callosum (CC) is the most significant of the three cerebral commissures that join the human left and right hemispheres. Numerous vascular, chemical, genetic, and metabolic disturbances can impede the growth of the CC. Complete agenesis of the CC is the most prevalent abnormality. As a result of improvements in our knowledge and advancements in ultrasonography technology, an increasing number of congenital CC abnormalities have been identified. Prenatal magnetic resonance imaging is crucial to identify related central nervous system abnormalities. Counseling about the postnatal resul
