Progressive intracranial aneurysmal disease in a child with progressive hemifacial atrophy (Parry-Romberg disease): case report.

Intracranial aneurysms are uncommon in children, and their presence often leads to suspicion of a systemic connective tissue disorder. We describe the case of a young male patient with progressive hemifacial atrophy (Parry-Romberg disease) and multiple intracranial aneurysms, a previously undescribed association, and propose that a neural crest defect may be the underlying abnormality in this patient. At age 5 years, the patient was treated for a giant aneurysm of the left cavernous carotid artery with carotid ligation in the neck and a superficial temporal artery-middle cerebral artery bypass

Soft Pillows, Hard Consequences: A Case Report Unveiling a Down Feather Neck Granuloma in a Pediatric Patient.

Down pillows, fitted with soft feathers, are commonly used among parents for their children due to their comfort, breathability, and warmth. In this case report, we present a 17-month-old boy who presented with a chief complaint of fever and progressive left neck swelling warranting a surgical excision after a failed course of antibiotic therapy. Histopathologic examination of the lesion demonstrated down feathers with associated granuloma with foreign-body type giant cell reaction, attributed to the use of the down pillow.

Takayasu's Arteritis With Acute Severe Aortic Regurgitation Requiring a Bio-Bentall Procedure.

Aortitis is a rare form of vasculitis that is associated with significant morbidity and mortality through the development of aneurysms, aortic rupture, dissection, and thrombotic occlusions. Common causes for non-infectious large vessel vasculitis include Takayasu's arteritis and giant cell arteritis. Delayed diagnosis and treatment can be devastating, resulting in lifelong disability or death. Here we present an unfortunate case of Takayasu's arteritis with aortitis and acute severe aortic regurgitation in a young patient requiring an emergent Bio-Bentall procedure and bowel resection.

Temporal Arteritis and Vision Loss in Microscopic Polyangiitis: A Case Report and Literature Review.

Microscopic polyangiitis (MPA) is an idiopathic autoimmune disease characterized by systemic vasculitis. While the lungs and kidneys are the major organs affected by MPA, it is known to involve multiple organ systems throughout the body. Temporal artery involvement is a very rare finding in MPA. This report presents a patient whose initial presentation was consistent with giant cell arteritis but was ultimately found to have microscopic polyangiitis. It highlights the importance of considering alternative types of vasculitis in the differential diagnosis for patients with atypical temporal art

Pulmonary pathologic alterations associated with biopsy inserted hydrogel plugs.

The prevention of pneumothorax after percutaneous lung biopsy is a major patient safety concern. The insertion of hydrogel plugs into biopsy sites to mitigate this complication is a new intervention. The histology of the plug has not been previously reported, and in this study the histologic reaction is reported in 13 cases. The hydrogel plug forms a spherical basophilic matrix pool with an adjacent foreign body giant cell reaction and patchy eosinophilia. No extension to the pleural surface is present. The potential diagnostic errors related to the presence of the plug are discussed.

Isolated large vessel pulmonary vasculitis leading to pulmonary artery aneurysm formation: a case report and literature review.

Pulmonary artery (PA) vasculitis occurs in association with primary vasculitides-in particular, giant cell arteritis, Takayasu's arteritis, or Behçet's disease-or secondary vasculitis as a result of infections or malignancy. However, PA vasculitis in isolation and with concomitant aneurysmal dilation is an unusual finding. We present a rare case of PA aneurysm secondary to isolated PA vasculitis in an asymptomatic patient with no features of systemic vasculitis. This case highlights one of the first cases of PA vasculitis managed with surgical resection alone.

From venoms to BBB-shuttles. MiniCTX3: a molecular vector derived from scorpion venom.

The present study aims to develop chlorotoxin (CTX), from Giant Yellow Israeli scorpion venom, as a new BBB-shuttle. Minimised versions of CTX were prepared to reduce its complexity while enhancing its BBB-shuttle capacity and preserving its protease-resistance. MiniCTX3, a monocyclic lactam-bridge peptidomimetic, was capable of transporting nanoparticles across endothelial cell monolayers. Our results reveal animal venoms as an outstanding source of new families of BBB-shuttles.

Crowned dens syndrome diagnosed on ¹⁸F-FDG PET/CT.

An 87-year-old woman with corticosteroid-resistant polymyalgia rheumatica underwent ¹⁸F-FDG PET/CT for suspected giant cell arteritis or neoplastic disease. FDG uptake in the immediate vicinity of the odontoid process, with a crownlike calcification, was identified on the CT scan on the posterior side of the dens, thus confirming the diagnosis of crowned dens syndrome. Because this rare syndrome is frequently misdiagnosed, nuclear physicians should be aware of the signs and symptoms of this condition, which may call for the use of PET/CT imagery.

Giant plexiform neurofibroma in a patient with neurofibromatosis type I.

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder primarily affecting the development and growth of nerve cell tissues. Plexiform neurofibroma is considered an uncommon skin tumor. The involvement of the genitourinary tract or the lower limb is rare, with bladder, upper urinary tract and genital involvement reported in decreasing order of frequency. The management of patients with plexiform neurofibroma is not well defined and aiming mostly at controlling symptoms. We present a case of a giant genitourinary plexiform neurofibroma associated with lower limb gigantism in a patient

Cherubism combined with epilepsy, mental retardation and gingival fibromatosis (Ramon syndrome): a case report.

Cherubism is an inherited, autosomal dominant disorder that characteristically affects the jaws of children. The disease typically manifest as a bilateral swelling with associated submandibular lymph node enlargements and usually regresses as age advances. The disease is microscopically indistinguishable from other giant cell lesions and is essentially a clinical diagnosis. The association of cherubism with gingival fibromatosis, epilepsy, mental retardation, stunted growth, and hypertrichosis is referred as Ramon syndrome. We report a case of Ramon syndrome in an 8 year old girl.

Vasculitis in the geriatric population.

Vasculitis represents an uncommon but important group of disease entities that may affect older patients. The most common systemic vasculitic disease in humans is giant cell arteritis, a disease process seen almost exclusively in patients over the age of 50. Vasculitic disease in geriatric patients presents unique challenges with regard to diagnosis and treatment. A thorough understanding of the vasculitic disease entities that may affect older patients as well as their diagnosis and management is essential in minimizing disease and treatment related morbidity and mortality.

[Polymyalgia rheumatica (arteritica)].

Polymyalgia rheumatica occurs mainly in patients older than 60 years of age. Differential diagnostically, the condition can be identified on the basis of a number of laboratory parameters (ESR, CRP, CK, autoantibodies) and, if the findings are not unequivocal, the additional use of electromyography and muscle biopsy. Some 40-50% of the patients have concomitant giant cell arteritis. Treatment of polymyalgia rheumatica is glucocorticoids are applied, usually for a period of at least one year. The initial and the tapered dose is dependent on whether an associated arteritis is present.

[Giant pleomorphic adenoma of the submaxillar gland: a case report].

The authors are reporting of an exceptional case of giant cell tumor of the submaxillar gland in a lady of 76 years old. She presents a progressively slow evolution within a period of 40 years. After a standard check up, a submaxillectomy was carded out. The weight of the operated mass was 3 kg before the pathological fixation. The post-operative period was without any problems and the patient was discharged after fifteen days. The anatomo-pathological examination concluded a pleomophic adenoma developed by the salivery gland.

Retrospective study of porcine circovirus 2 infection in Japan: seven cases in 1989.

Retrospectively, we demonstrated that seven pigs necropsied in 1989 had characteristic porcine circovirus 2 (PCV-2) lymphoid tissue lesions of severe lymphoid depletion, syncytial giant cell formation, and intracytoplasmic inclusion bodies in macrophages. Immunohistochemically, PCV-2 antigen was detected in these tissues and the distribution of positive staining corresponded to the distribution of inclusion bodies. Electron microscopy demonstrated viral particles compatible with porcine PCV-2. Therefore, the disease occurred in Japan as early as 1989.

Histopathology of ossicular implants.

Ossicular and cortical bone grafts maintain their morphologic structure for long periods of time and show varying amounts of replacement of nonviable bone by new bone through a slow process of creeping substitution. Cartilage grafts develop chondromalacia, lose stiffness, and tend to be resorbed over time. Plastipore prostheses elicit foreign body giant cell responses with microscopic biodegradation of the implants. There is a great need for the study of well-documented human temporal bone cases with in situ ossicular implants.

Aneurysmal bone cyst: report of a case mimicking orbital tumor.

Aneurysmal bone cysts are rare bone tumors which normally affect the vertebral region or long bones. Involvement of facial bones is uncommon. Although nonneoplastic, these lesions are expansile and may cause local destruction of bone and soft tissues. When making the diagnosis, it is imperative to consider the clinical, radiologic, and histologic features of the entity to avoid confusion with other possibly malignant giant cell tumors. We report an unusual case involving the orbit in a 2 1/2-year-old girl.

[Hypophysitis mimicking a pituitary adenoma (report of 3 cases)].

Three cases of rare chronic hypophysitis found by surgery are reported, with a review of the related literature. Two main types of inflammatory lesions of the pituitary gland are described: lymphocytic hypophysitis (LCH) and giant-cell granulomatous hypophysitis (GGH). These present clinically as a pituitary tumor but are characterized by distinct hypopituitarism with a slightly enlarged sella turcica. Only a few cases present as hyperprolactinemia. The inflammatory process may be resolved with hormonal replacement alone for LCH, while surgical intervention is proposed for GGH.

Inverted heart-shaped, interradicular radiolucent area of the anterior maxilla.

A variety of odontogenic, nonodontogenic, and inflammatory conditions may occur between the roots of the maxillary canine and lateral incisor. A list of differential diagnoses might include: odontogenic keratocyst, central giant cell granuloma, odontogenic myxoma, primordial cyst, lateral periodontal cyst, ameloblastoma, and radicular cyst. A diagnosis of globulomaxillary cyst should be noticeably absent from the list, however, and should be considered only after clinical radiographic, and microscopic correlation has effectively ruled out all other possibilities.

Temporal arteritis-like presentation of carotid atherosclerosis.

A 68 year-old woman presented with a two-week history of amaurosis fugax, ipsilateral fronto-temporal headache and jaw claudication suggesting carotid giant cell arteritis. However, this syndrome proved to be due to atherosclerosis causing complete occlusion of the external carotid artery at its origin and narrowing of the internal carotid artery. Combined external and internal carotid endarterectomy relieved the symptoms. The symptom complex of temporal arteritis may be rarely mimicked by carotid atherosclerotic occlusive disease.

[Rhizomelic pseudopolyarthritis].

Clinically, polymyalgia rheumatica is characterized by bilateral hip and shoulder pain predominant in the shoulder girdle, with a varying degree of alteration of the general condition. The erythrocyte sedimentation rate is often markedly increased. The dramatic effect of corticosteroids in low dosage confirms the diagnosis. The main problem is the relationship between the syndrome and giant cell arteritis, or Horton's disease. The therapeutic approach is strongly influenced by the risk of complications, notably ocular involvement. Another prognostic factor is the duration of the disease, which
