A single preoperative dose of gabapentin (800 milligrams) does not augment postoperative analgesia in patients given interscalene brachial plexus blocks for arthroscopic shoulder surgery.

BACKGROUND: Inadequate analgesia is common after shoulder arthroscopy. Both interscalene blocks and gabapentin are effective methods of pain management under various circumstances. We tested the hypothesis that gabapentin augments postoperative analgesia provided by interscalene brachial plexus block in patients having ambulatory arthroscopic shoulder surgery. METHODS: Sixty patients were randomly assigned to receive oral gabapentin, 800 mg, or placebo 2 h before surgery. An interscalene brachial plexus block with 0.3 mL/kg of 0.5% ropivacaine was performed. General anesthesia was maintained

Etiology of pediatric perforating eye injuries in southern Turkey.

In the present study, the records of 242 children, aged 1-14 years, admitted with perforating ocular injury were reviewed retrospectively over a 5-year period. There were 175 boys and 67 girls in the study group. The patients were divided into 3 groups according to their ages. Perforating injuries occurred most frequently in the street in all groups. The second most common place of the injury was at home in the 1-9 year-olds and in the fields in the 10-14 year-olds. The cause of the perforation was a metallic substance in 32.6%, wood in 15.3%, stone in 12.0%, glass in 12.3%, pellets in 12%, an

External fixation of the distal end of the radius: the same technique for different patient populations?

A series of 56 consecutive patients with 59 fractures of the distal end of the radius, treated with a unilateral Hoffmann external fixator between 1980 and 1988, is presented. Forty-seven of these patients underwent clinical re-examination at home. We could distinguish two different groups of patients: an older female group with an average age of 59.2 years and a younger male group with an average age of 40.1 years. The fractures in the women were caused by lower energy trauma than those in the men (P less than 0.05). The men had significantly more complex fractures (Frykman type VII or VIII)

The confounding of occupation and smoking and its consequences.

A strong pattern in smoking behavior can be demonstrated, in which smoking is much more prevalent among those occupational groups (and social strata) that are also more exposed to hazards in the workplace and much less prevalent among those groups less exposed to such hazards. As a consequence, comparing individuals with greater to those with lesser exposure to tobacco also compares groups that differ with respect to occupational exposure to dust, fumes and toxic substances and with respect to occupationally related lifestyle factors. Analyses of the U.S. National Health Interview Survey show

Studies of the teratogenic potential of exposure of rats to 6000-MHz microwave radiation. I. Morphologic analysis at term.

Thirty-six pregnant Wistar strain albino rats were exposed throughout pregnancy to 6000-MHz microwave radiation at a power density level of 35 mW/cm2 or were used as controls. The irradiation did not cause a significant increase in maternal body temperature as measured by a rectal thermocouple. The rats were randomly assigned to one of four groups: home cage control (5), anechoic chamber control (10), sham-irradiated concurrent control (10), and irradiated (11). All animals were killed on the 22nd day of gestation, and maternal tissues were removed and weighed and maternal blood samples were t

DESIGN REFINEMENT AND TAKE-HOME TESTING OF A MOBILE MANUAL STANDING WHEELCHAIR.

Commercially available manual standing wheelchairs are immobile during standing, which may limit their utility for the wheelchair user. This paper describes our refinement of a previously designed manual standing wheelchair that is mobile-in-standing to make it accessible to a wider group of potential users. A powered sit-to-stand system was utilized, which required significant design changes to the wheelchair to allow mobility in standing. In addition to describing these design changes, the paper briefly describes a two-month take-home study of N=11 Veterans with spinal cord injuries and diso

First Fatal Case of Adult Meningoencephalitis Caused by Pasteurella canis: An Autopsy Case Report.

Pasteurella canis is a Gram-negative bacterium commonly found as a commensal organism in the oral cavity of dogs and cats. Human infections are rare and typically occur following skin breaches, with bacteremia reported less frequently. To date, fatal central nervous system infections in adults caused by P. canis have not been reported. We report the case of a 25-year-old immunocompetent man with no significant medical history. The patient developed otitis a few days after his dog licked his ear. Twenty days later, he died at home following a febrile illness with headaches. An autopsy revealed

Hypertension in the Oldest Old.

Hypertension affects 80% of adults 80 years of age or older and is a leading cause of morbidity and mortality through its fundamental role as a risk factor for cardiovascular disease, stroke, kidney disease, and cognitive impairment. Antihypertensive therapy reduces the risk of coronary artery disease, heart failure, atrial fibrillation, stroke, and cognitive decline among patients in this age group, but older patients with advanced frailty, cognitive impairment, complex comorbidities, or nursing home residence have largely been excluded from clinical trials. This article reviews the epidemiol

Differential reinforcement and stimulus fading without escape extinction to teach cooperation with nasal swab tests.

A nasal swab test is commonly used to detect the presence of respiratory viruses, such as SARS-COV2 or influenza. Some individuals with autism spectrum disorder may display challenging behavior during these types of tests, which may interfere with safe and successful test completion. Research has shown that differential reinforcement without escape extinction combined with stimulus fading can be effective to increase cooperation with other types of medical tasks (e.g., blood draws). The purpose of the current study was to systematically replicate the procedures described in Stuesser and Roscoe

Targeting VCAM-1 with chimeric antigen receptor and regulatory T cell for abdominal aortic aneurysm treatment.

Abdominal aortic aneurysm (AAA) is a life-threatening condition characterized by the dilation of the abdominal aorta, leading to a high risk of rupture. Current treatment options are limited, particularly for patients ineligible for surgical interventions. This study explores a novel immunotherapeutic approach using chimeric antigen receptor Treg cells targeting vascular cell adhesion molecule-1 (VCAM-1) to mitigate AAA progression. By leveraging the specificity and regulatory function of CAR-Treg cells, our research aims to modulate the immune response and reduce inflammation in the aneurysma

Oral Health of Children in Foster Care.

The objective of this study was to evaluate the prevalence of dental disease among children in protective custody (eg, foster care) and associations with foster placement characteristics. We analyzed data from 778 patients ≥12 months of age who received oral health examinations by a pediatrician at a foster care clinic in a large urban setting, including 37 who were additionally examined by a dental resident. Examination findings were classified as routine, in need of urgent referral, or in need of emergent referral. Overall, 53% of children in protective custody had at least 1 abnormal dental

Compounding social-ecological crises drive spatial mobility and land abandonment in Morocco's High Atlas.

Drought and water insecurity are escalating crises that shape dryland farmers' mobility and land abandonment decisions. In this study, we appraised the causes of such decisions through 75 qualitative interviews and six group discussions with residents of rural Amazigh communities in Morocco's High Atlas. We identified a multi-faceted crises nexus, where water scarcity exacerbates economic and value-related crises and personal insecurities, leading to a crisis of confidence in traditional rural lifeways. Increasingly multi-local household structures and intersectional attributes mediate actors'

Better understanding fall risk: AI-based computer vision for contextual gait assessment.

Contemporary research to better understand free-living fall risk assessment in Parkinson's disease (PD) often relies on the use of wearable inertial-based measurement units (IMUs) to quantify useful temporal and spatial gait characteristics (e.g., step time, step length). Although use of IMUs is useful to understand some intrinsic PD fall-risk factors, their use alone is limited as they do not provide information on extrinsic factors (e.g., obstacles). Here, we update on the use of ergonomic wearable video-based eye-tracking glasses coupled with AI-based computer vision methodologies to provid
