Pancreas preserving distal duodenectomy: A versatile operation for a range of infra-papillary pathologies.

AIM: To investigate the range of pathologies treated by pancreas preserving distal duodenectomy (PPDD) and present the outcome of follow-up. METHODS: Neoplastic lesions of the duodenum are treated conventionally by pancreaticoduodenectomy. Lesions distal to the major papilla may be suitable for a pancreas-preserving distal duodenectomy, potentially reducing morbidity and mortality. We present our experience with this procedure. Selective intraoperative duodenoscopy assessed the relationship of the papilla to the lesion. After duodenal mobilisation and confirmation of the site of the lesion, t

Justified skepticism about Apgar scoring in out-of-hospital birth settings.

BACKGROUND: The Apgar score is used worldwide to assess the newborn infant shortly after birth. Apgar scores, including mean scores and those with high cut-off scores, have been used to support claims that planned home birth is as safe as hospital birth. The purpose of this study was to determine the distribution of 5 min Apgar scores among different birth settings and providers in the USA. METHODS: We obtained data from the National Center for Health Statistics of the US Centers for Disease Control birth certificate data for 2007-2010 for all singleton, term births of infants weighing ≥2500

Estimates of the population prevalence of injection drug users among hispanic residents of large US metropolitan areas.

Little information exists on the population prevalence or geographic distribution of injection drug users (IDUs) who are Hispanic in the USA. Here, we present yearly estimates of IDU population prevalence among Hispanic residents of the 96 most populated US metropolitan statistical areas (MSAs) for 1992-2002. First, yearly estimates of the proportion of IDUs who were Hispanic in each MSA were created by combining data on (1) IDUs receiving drug treatment services in Substance Abuse and Mental Health Services Administration (SAMHSA)'s Treatment Entry Data System, (2) IDUs being tested in the Ce

Microbial leakage of MTA, Portland cement, Sealapex and zinc oxide-eugenol as root-end filling materials.

Dysthymic disorder is a chronic depressive condition occurring in 0.6-4.6% of children and 1.6-8.0% of adolescents. Although symptoms are less severe than those observed in major depression, childhood-onset dysthymic disorder is characterised by a persistent and long-term depressed or irritable mood (mean episode duration 3-4 years), a worse outcome than major depression and, frequently, comorbid disorders (in around 50% of patients). Long-lasting depressive symptoms seem responsible for long-term disabling consequences on social skill learning, psychosocial functioning and consequent professi

Transillumination and optical coherence tomography for the detection and diagnosis of enamel caries.

BACKGROUND: Caries is one of the most prevalent and preventable conditions worldwide. If identified early enough then non-invasive techniques can be applied, and therefore this review focusses on early caries involving the enamel surface of the tooth. The cornerstone of caries detection and diagnosis is a visual and tactile dental examination, although alternative approaches are available. These include illumination-based devices that could potentially support the dental examination. There are three categories of illumination devices that exploit various methods of application and interpretati

Comparative Effectiveness of Sacubitril/Valsartan Versus Sodium-Glucose Cotransporter 2 Inhibitors among Patients with Heart Failure with Reduced Ejection Fraction with or without Diabetes: A Cohort Study.

BACKGROUND: Sodium-glucose cotransporter 2 inhibitors (SGLT2i) and sacubitril/valsartan (SAC/VAL), an angiotensin receptor-neprilysin inhibitor, are both US Food and Drug Administration-approved and guideline-recommended therapies for heart failure with reduced ejection fraction (HFrEF). While each has demonstrated significant reductions in cardiovascular morbidity and mortality, direct comparative evidence remains limited, particularly in relation to patients' diabetes status. OBJECTIVE: To compare the effectiveness of SAC/VAL versus SGLT2i in preventing HF-related and all-cause hospitalizat

Fiber Supplementation during and after Glucagon-Like Peptide-1 Receptor Agonists Treatment: A Perspective on Clinical Benefits.

BACKGROUND: Pharmaceutical advancements with glucagon-like peptide-1 receptor agonists (GLP-1RA) treatment have dramatically shifted medical treatment for obesity and diabetes. The mechanism of GLP-1RA can lead to unwanted side effects, with nearly half of patients discontinuing treatment within 1 year. There is currently limited evidence on weight maintenance after treatment, but available data suggest that sustained benefits require continued use of GLP-1RA. A comprehensive position article encourages fiber intake during and after GLP-1RA treatment, but consuming high-fiber diets can be chal

Impact of a diagnostic strategy for appendicitis in children with acute abdominal pain in primary care: study protocol for a hybrid type 1 cluster randomised controlled trial.

BACKGROUND: Children with acute abdominal pain pose a diagnostic challenge for general practitioners (GPs), as it can be difficult to distinguish appendicitis from self-limiting conditions due to overlapping symptoms. To support GPs, a diagnostic strategy for appendicitis was developed that integrates an externally validated prediction rule with C-reactive protein point-of-care testing (CRP POCT) and risk-based management advice. This study will compare the impact of this diagnostic strategy to usual care in children with acute abdominal pain in primary care in terms of: (i) clinical effective

Knowledge, attitude, and practice of dentists in India regarding geriatric oral healthcare and polypharmacy management: a cross-sectional study.

BACKGROUND: The global population is ageing rapidly, presenting significant challenges to oral healthcare systems. Geriatric patients frequently present with complex oral conditions compounded by polypharmacy and systemic comorbidities. Despite widespread treatment of elderly patients, whether dentists possess adequate knowledge and systematic application of evidence-based practices remains unclear. This study assessed the knowledge, attitudes, and practices (KAP) of Indian dentists concerning geriatric oral healthcare and polypharmacy management, using the KAP theoretical model as a conceptua

Assessing the implementation of national sodium reduction policies in Nigeria: an interim qualitative evaluation of stakeholder perspectives.

INTRODUCTION: Population-wide dietary sodium reduction is a global priority and a highly cost-effective strategy to lower blood pressure and reduce cardiovascular disease risk. In response, the Nigerian government launched the National Multi-sectoral Action Plan (NMSAP) in 2019, recommending evidence-based policy actions including limiting sodium in processed foods, restricting food advertising especially for children, promoting public awareness, integrating nutrition education in schools, and front-of-package labeling. We conducted a qualitative study to examine progress, barriers, and facili

The Effects of Orexin Receptor Antagonism During Early Withdrawal in Stimulant Use Disorder: Protocol for a Proof-of-Concept Study.

BACKGROUND: Stimulant use disorders (StimUDs), including cocaine and methamphetamine use disorders, remain a major public health concern in the United States with no US Food and Drug Administration (FDA)-approved pharmacological treatments. The early recovery period following stimulant use is marked by sleep disturbances, heightened stress reactivity, and dysregulated reward processing, contributing to high rates of resumption of use. Evidence from animal and human studies supports the orexin neuropeptide system as a promising therapeutic target for minimizing these disruptive mechanisms. Suvo

Occurrence and Spatial Distribution of Antibiotic-Resistant Bacteria and Antibiotic Resistance Genes in Surface and Groundwater of the Kelani River Basin, Sri Lanka.

Antimicrobial resistance (AMR) in aquatic environments represents a critical global public health threat, yet comprehensive environmental surveillance data remain scarce in low- and middle-income countries. The Kelani River Basin, supplying approximately 80% of Colombo's potable water and Sri Lanka's most industrially impacted waterway, lacks a basin-wide characterization of antibiotic-resistant bacteria (ARB) and antibiotic resistance genes (ARGs) across spatial and seasonal gradients. This study provides the first comprehensive basin-wide assessment of ARB distribution, ARG occurrence, minim

Visualizing Persistent Inflammation and Occult Infections in Nonrecovering ICU Patients with [18F]FDG PET/CT: VIPER Study Protocol.

The use of [18F]FDG PET/CT for detection of infectious and inflammatory foci in patients admitted to the intensive care unit (ICU) has gained interest as previous retrospective studies have shown promising results. [18F]FDG PET/CT in these studies was predominantly performed in patients with prolonged organ failure. These patients often acquire an immune dysregulated state, resulting in infectious complications impairing organ recovery. Early detection of infectious or inflammatory foci in this population may improve outcomes and reduce health care costs. We aim to explore the value of [18F]FD

A Fellow-Led Quality Improvement Initiative to Increase Disease Activity Measurement in a Busy Academic Rheumatology Clinic.

BACKGROUND: The preferred management of rheumatoid arthritis (RA), recommended by the American College of Rheumatology (ACR) and international organizations like the European Alliance of Associations for Rheumatology (EULAR), is the treat-to-target (T2T) strategy, due to its association with improved long-term patient outcomes. Successful implementation requires consistent assessment and documentation of disease activity, commonly using tools such as the Clinical Disease Activity Index (CDAI). However, variability in the use and documentation of disease activity measures exists in academic rhe

Bridging inpatient and community treatment: feasibility, acceptability, and short-term outcomes of step care, a virtual CBT-E pathway for adults with severe eating disorders.

BACKGROUND: Transitions between inpatient and community care carry a high risk of disengagement or relapse for adults with severe eating disorders. Step Care was developed to address this gap by adapting intensive enhanced cognitive behavioural therapy (CBT-E) for remote, multidisciplinary delivery in patients' homes. The virtual service operates through three pathways, Starting Well, Staying Well, and Working Towards Recovery, and aims to improve engagement, reduce hospitalisation, and prevent relapse. METHOD: This paper describes the Step Care service innovation and reports a prospective se

Clinical impact of metagenomic next-generation sequencing on pathogen detection and outcomes in non-immunocompromised patients with severe pneumonia supported by veno-venous extracorporeal membrane oxygenation.

BACKGROUND: Timely pathogen identification is crucial for guiding antimicrobial therapy in severe pneumonia requiring veno-venous extracorporeal membrane oxygenation (vv-ECMO). Compared with slow and often insensitive conventional culture, metagenomic next-generation sequencing (mNGS) enables more rapid and comprehensive pathogen detection and may improve clinical management. This study aimed to evaluate the clinical impact of mNGS compared with conventional culture in non-immunocompromised patients undergoing vv-ECMO for severe pneumonia. METHODS: The retrospective study explored non-immunoco
