Echocardiographic evaluation of diastolic function in mouse models of heart disease.

BACKGROUND: Mouse models of heart disease are extensively employed. The echocardiographic characterization of contractile function is usuall … trial area increased in all three chronic models while mitral E/A was difficult to quantify at high heart rates. Isovolumic relaxation time (IVRT) and Doppler E/E' increased significantly and the peak longitudinal strain rate during early filling (peak reverse longitudinal strain rate) decreased significantly after aortic constriction, with the changes being …

Hot flashes and cardiac vagal control during women's daily lives.

OBJECTIVE: The physiology of menopausal hot flashes is not well understood. The autonomic nervous system may play a role in hot flashes, but the current understanding is limited. We previously demonstrated in the laboratory that decreases in high-frequency heart rate variability, an index of cardiac vagal control, occur during hot flashes relative to preceding and following periods. In the present study, we tested whether we would observe a similar phenomenon in the ambulatory setting. We additionally considered respiratory rate in these associations. METHODS: Twenty-one perimenopausal and po…

Differential effects of autonomic blockade on sinus and atrioventricular nodal function in normals and in intrinsic sinus node dysfunction.

The effect of pharmacologic total autonomic blockade on sinus and atrioventricular nodes was studied in 10 normals and 21 patients with sick sinus syndrome with abnormal intrinsic corrected sinus node recovery time. In normals the intrinsic heart rate (113.3 +/- 11.6 beats/min) was higher than the resting heart rate (87.3 +/- 12 beats/min; P less than 0.001). The AH interval at an identical paced rate decreased from 119 +/- 36 msec to 93 +/- 17.6 msec after autonomic blockade (P less than 0.05). Mean atrial paced cycle length at AH Wenckebach block was not different during control and after dr…

Thermal plasticity of adrenaline-mediated, frequency-dependent calcium homeostasis in rainbow trout ventricular cardiomyocytes.

The effect of global warming on rising aquatic temperatures is producing ever-steeper thermoclines. Fish encountering these sharp changes in water temperature might experience an acute-warming stress. Temperature is the most dominant environmental factor affecting heart function in fish, and without compensatory mechanisms as temperatures rise (e.g. higher heart rate), it could imperil cardiovascular performance. To enhance heart function during acute warming, fish release adrenaline to boost Ca2+ influx in heart cells (cardiomyocytes). However, the relationship between acute warming, elevated…

[Fascicular ventricular tachycardia in a 49-year-old patient].

We report a 49-year-old patient who presented with tachycardia in our emergency room. The 12-lead ECG showed a wide complex tachycardia with a heart rate of 234 beats per minute. After structural heart disease was excluded via echocardiography, coronary angiography and magnetic resonance imaging, an electrophysiological study was performed. During programmed ventricular stimulation, a fascicular tachycardia was induced, which was successfully treated by radiofrequency ablation. Fascicular ventricular tachycardia is a rare tachycardia that occurs in patients without structural heart disease. Ra…

[Anesthetic and perioperative management of 1000 cases of congenital heart surgery--massive vasodilator therapy and early extubation].

We report our anesthetic and perioperative management of 1000 cases of congenital heart surgery in Okayama University Medical School over the last six and a half years. Most of these patients were anesthetized by fentanyl and all patients were managed in our ICU postoperatively. Although hospital death rate in all patients was 4.5%, almost half of dead patients were patients with hypoplastic left heart syndrome. Neonatal mortality rate has been still high, 18.9%, which has been our main problem. We tried to extubate patients with complex congenital heart diseases earlier postoperatively for th

[Valve replacement in patients with huge heart].

In general, valve replacement for patients with big heart (cardiac-thoracic rate greater than 0.80) has been considered a contraindication. Mitral valve replacement was done in 11 patients with a C/T rate of 0.81 to 0.97. In addition, aortic valve replacement was performed in 2 patients and tricuspid valvuloplasty in 5. One patient died from low cardiac output after operation. The heart function of the survivor was improved from III or IV to II degree after operation. We consider that active pre- and postoperative management for preserving myocardial function and preoperative technique are imp

[A new mechanical sensor for activity controlled and body position dependent frequency adaptive pacemaker stimulation].

In the past, the most important feature in the development of rate-responsive pacemaker systems was ensuring an adequate rate response to physical exercise. But there are other variables that modulate the rate of the healthy heart; examples include postural changes in heart rate in order to prevent orthostatic reactions, and the nightly decrease in intrinsic pacemaker rate. Therefore, we developed a sensor that could distinguish, not only rest and activity, but also discriminate between a supine and an upright position. This sensor is a multicontact tilt switch, which contains a small mercury …

Gradient over Melody valve in left AV valve position reduces with beta blockade.

The Melody valve, designed for implantation into the pulmonary outflow tract, can also be used to treat the pathology of atrioventricular (AV) valves. Increasing gradients are seen as an indication for re-dilating the valve. Our case demonstrates the heart rate dependency of the gradient across a Melody implanted in the left AV valve position in an infant. Beta blockers were used to lower both heart rate and gradient.

Unsuccessful methotrexate treatment of a tubal pregnancy with a live embryo.

Two cases with unsuccessful local and systemic methotrexate (MTX) therapy of tubal pregnancy with fetal heart rate activity are reported. The three modes of therapy, the first with local potassium chloride solution injection, the second with local MTX injection, and the third with systemic MTX injection, failed in the resolution of viable ectopic pregnancy. Therapy of MTX in cases of tubal pregnancy with demonstrable fetal heart rate beats, should be reconsidered.

Sleep disturbance in association with elevated pulse rate for prediction of mortality--consequences of mental strain?

OBJECTIVES: Sleep deprivation has experimentally been shown to adversely influence glucose metabolism, endocrine function and sympathovagal balance in young men without known serious disease. We investigated the impact of sleep problems and resting heart rate in a large sample of self-reported, healthy middle-aged men and women on long-term mortality. METHODS: In all 22,444 men and 10,902 women participated in a population-based health screening (71% mean attendance), including blood sampling and examination of blood pressure (BP) and pulse rate after 10 min supine rest, as well as a self-adm…

Pet-induced stress buffering depends on touch: Evidence from brain and cardiac measures.

While there is much evidence for the wellbeing benefits of pet-ownership, the mechanisms through which these benefits emerge have been left uncertain. Here, we pursued the possibility that friendly physical contact with one's pet dampens stress and furthers homeostasis. Sixty-four pet owners visited our lab with their dog. They completed two experimental blocks each comprised of a resting baseline, a stress-inducing cognitive task, and a recovery period. The dog was present only during recovery, with physical contact being possible during one block but not the other. Relative to rest, the cogn

The role of mindfulness in attenuating the adverse effects of daily negative events: An experience sampling study.

Mindfulness is robustly associated with psychological and physiological well-being. To date, studies have primarily focused on trait mindful … mindfulness levels, positive mood, and negative mood, while wearing Fitbit devices to monitor their heart rate. The registered analyses mainly demonstrated main effects (but no buffering effects) for state mindfulness. Specifically, state mindfulness predicted higher positive mood and lower heart rate among women. No association was found with negative mood, …

The effect of self-compassion versus mindfulness interventions on autonomic responses to stress in generalized anxiety disorders.

OBJECTIVE: Although research on psychological interventions in generalized anxiety disorder (GAD) has provided evidence of their effectiveness regarding self-reported outcomes, few studies have examined their psychophysiological effects. Heart rate is emerging as a potential biomarker of efficacy in anxiety disorders. This study aimed to investigate the effects of a self-compassion intervention versus a mindfulness intervention on physiological arousal in response to induced stress. METHODS: Forty-seven patients with GAD had heart rate data collected during a stress task before and after a 2-…

Cerebellar Asymmetry of Motivational Direction: Anger-Dependent Effects of Cerebellar Transcranial Direct Current Stimulation on Aggression in Healthy Volunteers.

No main effects of tDCS on aggressive behavior, heart rate and HRV were found. Higher state anger before and during the PSAP task was associated with increased aggressive behavior in th …

QTc interval prolongation due to spinal anesthesia in patients with and without diabetes: an observational study.

BACKGROUND: Spinal anesthesia and autonomic neuropathy (caused by diabetes) prolong the QTc interval. Changes in the duration of the QTc interval following subarachnoid blockade in patients with diabetes have not been evaluated. We hypothesized that after subarachnoid blockade, QTc interval prolongation would be greater in patients with diabetes than in those without. Accordingly, we compared the QTc interval, T wave peak-to-end interval (Tp-e interval), blood pressure, heart rate, and heart rate variability before and after spinal anesthesia in patients with and without diabetes. METHODS: Th…
