Atrial Septal Defect Closure: Not Always Straightforward.

A nine-year old boy presented following a chest x-ray for COVID-19 infection that showed cardiomegaly. Transthoracic echocardiogram revealed a large atrial septal defect, dilated right heart, and normal function. Surgical repair was performed eight weeks after his COVID diagnosis. After weaning from cardiopulmonary bypass, pulmonary artery pressures were 2/3 systemic and ectopy was seen, requiring inotropes, nitric oxide, lidocaine, and amiodarone. He was discharged on postoperative day (POD) 5. On POD 6, he presented with acute right foot ischemia. Computed tomography showed a large aortic th

Successful Radiofrequency Ablation of the Right Lateral Accessory Pathway with Modified Carpentier Reconstruction Surgery in a Patient with Ebstein Anomaly Concomitant of Secundum Atrial Septal Defect, Atrial Fibrillation, and Wolff-Parkinson-White Syndrome.

Ebstein anomaly (EA) results from the failure of proper delamination of the tricuspid valve leaflets from the right ventricle (RV) myocardium. The severity of EA occurs on a spectrum that results in varying degrees of tricuspid regurgitation, atrial dilation, RV dilation, and dysfunction. These effects have the potential to create substrates that can give rise to atrial arrhythmia, ventricular arrhythmia, and a greater incidence of Wolff-Parkinson-White (WPW) syndrome Wackel et al. (2018) accounting for 0.5% of all congenital heart diseases (Oh et al. 1985). In the case of atrial fibrillation

Fatal embolic events in childhood.

Lethal embolic events in children are an uncommon occurrence that may be first detected at autopsy. Emboli consist of thrombi, tumours, infective organisms, fat, air and foreign body material. The most significant circulations that may be obstructed are the pulmonary, coronary and cerebral. Rare conditions such as arteriovenous malformations may act as sources for emboli in children, and the brain is at particular risk from paradoxical embolism. The latter is facilitated by right to left communications in the heart such as atrial and ventricular septal defects and also from pulmonary vascular

Accessory mitral valve causing left ventricular outflow tract obstruction.

We studied the clinical characteristics and operative treatment of left ventricular outflow tract obstruction (LVOTO) caused by a congenital accessory mitral valve (AMV). Two patients were admitted to our department. Preoperatively, case 1 was diagnosed as congenital heart disease with severe LVOTO and an anterior mitral valve cleft. The patient in case 2 had a congenital atrial septal defect combined with AMV and mild LVOTO, as well as mild mitral valve regurgitation. In case 1, LVOTO was caused by a type I (fixed) AMV. In case 2, the AMV was type II (mobile type). Both AMV were resected, and

Asphyxiating death due to basal lingual cyst (thyroglossal duct cyst) in two-month-old infant is potentially aggravated after central catheterization with forced positional changes.

There are various causes of possible upper airway obstruction in infants. Particularly, large cysts on the base of tongue may cause severe airway obstruction by a mass effect on the hypopharynx and by displacing the epiglottis. Of these basal lingual cysts, thyroglossal duct cyst is rare but occasionally its remnants can be found at the base of the tongue. Although they are pathologically benign, basal lingual cysts may result in death by asphyxia. There are a few reported cases of asphyxia caused by basal lingual cyst. All died suddenly in the bed or cot while sleeping without any significant

Early sonographic diagnosis of fetal small left heart ventricle with a normal proximal outlet tract: a medical dilemma.

A small left heart ventricle with a normal outflow tract was detected by transvaginal ultrasound in 16 fetuses at 14-16 weeks' gestation. A high incidence of associated anomalies was found in these fetuses, namely coarctation of the aorta (5), abnormal karyotype (4), cystic hygroma (4), and ventricular septal defect (3). The incidence of extracardiac anomalies was probably even higher, since post-mortem and karyotype examinations were not performed in all cases of termination of pregnancy. In patients who decided to continue the pregnancy (7), a normal left ventricle was observed in four cases

Corrective surgery for tricuspid atresia: which modification of Fontan-Kreutzer procedure should be used? A review.

Various modifications of Fontan-Kreutzer procedure have evolved over the years. The available literature was reviewed in an attempt to evaluate whether one of these operations is preferable to others. Although there is no complete agreement, direct atriopulmonary anastomosis in the absence of a subpulmonary ventricular chamber, and atrioventricular connection via a homograft conduit in patients with a good-sized (greater than 30% of normal), well-formed (with both trabecular and outlet portions) right ventricle appear to be reasonable choices. In patients with more than the usual risk or at lo

Surgical treatment of persistent ductus arteriosus at the University College Hospital, Ibadan.

Seventy-seven patients aged from 5 days to 30 years underwent surgical correction of persistent ductus arteriosus (PDA) between July 1975 and December 1985. Forty-seven percent presented before 5 years of age while 20% were aged above 15 years. The male:female ratio was 1:2. Additional cardiovascular anomaly was present in 14 patients, 8 of which were ventricular septal defect. Majority presented with congestive heart failure and or failure to thrive. Surgical closure was effected by ligation in 67 cases, division and suture in 5 and unspecified in 5 cases. Operative mortality was 2(2.1%) case

Systolic time intervals in isolated septal defects before and after corrective surgery in female patients. I. Atrial septal defect.

Systolic time intervals (STI) were evaluated in 19 female patients with uncomplicated ostium secundum atrial septal defect (ASD), before, shortly after (within 2 months) and a long time after the corrective surgery (mean 13.2 years). The measurements were obtained from simultaneous high speed photographic recordings of electrocardiogram, external carotid pulse and phonocardiogram. Before the operation, a significant prolongation of Q-I heart sound interval was detected, together with a less prominent but statistically significant shortening of the left ventricular ejection time; the above alte

Anatomic variations in underdeveloped right ventricle related to tricuspid atresia and stenosis.

In the anatomy of 416 hearts, the seat of tricuspid stenosis or atresia is examined, with special reference to Fontan-like surgical procedures. A classification is offered which includes cases with and without regular or inverted transposition, and with decreased or increased pulmonary flow. The size and thickness of the right atrium, the size and architecture of the right ventricle, the size of the pulmonary tree, the types of atrial and ventricular septal defects, the condition of the mitral valve, and the size and thickness of the left atrium and left ventricle are analyzed. In addition the

Genetic Risk Score Constructed from Polymorphisms in the VEGFA, TBX5, and SMAD7 Genes Provides Novel Insights into the Molecular Mechanisms of the Tetralogy of Fallot and Ventricular Septal Defect (Case-Control Study from the Pakistani Population).

Congenital heart defects are common and complex birth-defect malformations in developed and developing countries. It is a multifactorial disease that involves the interaction of either gene-gene or gene-environment. This comparative study was the first report on the genotypic-phenotypic correlation in the Pakistani population. The single nucleotide polymorphisms (SNPs) were further tested for association with maternal diabetes mellitus or hypertension. In addition, the cumulative genetic risk score (GRS) for low to moderately-associated SNPs was calculated for each study subject, which can ult

Prenatal detection of a 3q29 microdeletion in a fetus with ventricular septum defect: A case report and literature review.

RATIONALE: Chromosomal 3q deletion is a recurrent genomic alternation, which is rarely reported in clinic. PATIENT CONCERNS: A 27-year-old woman underwent amniocentesis for cytogenetic analysis and single nucleotide polymorphism (SNP) array analysis at 27 weeks of gestation, due to ventricular septum defect in prenatal ultrasound findings. DIAGNOSES: G-banding analysis showed the karyotype of the fetus was normal and the couple also had normal karyotypes. However, SNP array detected a 1.71 Mb microdelection in 3q29, which was described as arr[hg19]3q29(194184392-195887205) × 1. There are 12

The association of scoliosis and congenital heart defects.

The effects of cardiac surgery on patients with congenital heart defects and the subsequent development of scoliosis were studied. A group of 998 patients with congenital heart defects who were less than sixteen years old were operated on at the Mayo Clinic during the ten-year period 1950 through 1959. Standing roentgenograms of the spine were made of 377 of the patients ten years or more after surgery. The ages of the patients at follow-up ranged from ten years and seven months to thirty-five years and three months, with a mean age of twenty-one years and four months. The average length of fo
