Discontinuation of denosumab and associated fracture incidence: analysis from the Fracture Reduction Evaluation of Denosumab in Osteoporosis Every 6 Months (FREEDOM) trial.

Osteoporosis is a chronic disease and requires long-term treatment with pharmacologic therapy to ensure sustained antifracture benefit. Denosumab reduced the risk for new vertebral, nonvertebral, and hip fractures over 36 months in the Fracture Reduction Evaluation of Denosumab in Osteoporosis Every 6 Months (FREEDOM) trial. Whereas discontinuation of denosumab has been associated with transient increases in bone remodeling and declines in bone mineral density (BMD), the effect on fracture risk during treatment cessation is not as well characterized. To understand the fracture incidence betwee…

Discriminatory ability of quantitative ultrasound measurements is similar to dual-energy X-ray absorptiometry in a Brazilian women population with osteoporotic fracture.

The discriminating ability and relevance of clinical risk factors, quantitative ultrasound (QUS) variables, X-ray-based bone mineral density (BMD) and hip axis length (HAL) measurements to evaluate the risk of osteoporotic fracture in elderly Brazilian women were examined in this study. QUS at the calcaneus (Achilles +, Lunar), HAL and BMD measurements (DPX-L, Lunar) at several anatomical sites were performed in 275 postmenopausal Caucasian women. Patients with suspected secondary osteoporosis were excluded. One hundred twenty-two (44.4%) women had had previous osteoporotic fracture. All of th

Mortality following periprosthetic and peri-implant femoral fractures: comparative analysis according to previous implant type.

INTRODUCTION: Periprosthetic and peri-implant femoral fractures (PPF) include periprosthetic hip fractures (periPC), periprosthetic knee fractures (periPR), as well as peri-implant fractures associated with an osteosynthesis nail (periCL). Although risk factors for PPF are widely studied, there is scarce evidence on differential risk factors for each subtype. The objective of this study is to compare 30-day and 1-year mortality rates among the three PPF subtypes and identify differential risk factors between them. METHODOLOGY: A retrospective cohort study conducted in a tertiary hospital, inc…

Clinical outcomes of revision arthroplasty with a modular taper fluted titanium stem for the treatment of Vancouver B2 and B3 periprosthetic femur fractures.

BACKGROUND: Existing literature on Vancouver B2 and B3 periprosthetic fracture management with modular tapered fluted stems (MTFS) is limited by lack of patient-reported outcomes, small samples, and unseparated inclusion of multiple implant models. This study examines the outcomes of Vancouver B2 and B3 fractures treated with a single MTFS hip system, the Zimmer Modular Revision (ZMR), with a minimum 1-year follow-up. METHODS: We conducted a retrospective review of all patients who received a femoral revision with a ZMR taper stem to treat a Vancouver B2 or B3 fracture at our centre between 2

Study of the significance of parameters and their interaction on assessing femoral fracture risk by quantitative statistical analysis.

Early assessment of hip fracture helps develop therapeutic and preventive mechanisms that may reduce the occurrence of hip fracture. An accurate assessment of hip fracture risk requires proper consideration of the loads, the physiological and morphological parameters, and the interactions between these parameters. Hence, this study aims at analyzing the significance of parameters and their interactions by conducting a quantitative statistical analysis. A multiple regression model was developed considering different loading directions during a sideways fall (angle [Formula: see text] and [Formu

Catastrophic fracture of a stable metal acetabular component.

Various modes of failure of primary and revision total hip arthroplasty have been well documented in the literature over the past 30 years. Concerns over polyethylene wear, osteolysis, and hypersensitivity reactions leading to component loosening and early revision have been evaluated and reported in the literature. Routine follow-up is important to monitor for postoperative issues that might lead to the subsequent need for revision. This article describes a case of a 64-year-old man who initially presented 11 years prior with an intertrochanteric fracture, which failed secondary to varus align

Total hip arthroplasty in patients 80 years of age and older.

One hundred fifty-seven consecutive patients (162 total hip arthroplasties) 80 years of age and older were observed for one year after total hip arthroplasty (THA). Clinical results and complications were recorded. The indication for surgery was degenerative joint disease (DJD) in one half of the patients and complications to proximal femoral fractures in the other half. The mean age was 83 years old in both groups. In 80% of the patients, no complications were recorded during the first year. Three patients died during the first three months. Two deep infections occurred (1.2%). The dislocatio

LRP5 gene polymorphisms predict bone mass and incident fractures in elderly Australian women.

Postmenopausal osteoporosis and bone mass are influenced by multiple factors including genetic variation. The importance of LDL receptor-related protein 5 (LRP5) for the regulation of bone mass has recently been established, where loss of function mutations is followed by severe osteoporosis and gain of function is related to increased bone mass. The aim of this study was to evaluate the role of polymorphisms in the LRP5 gene in regulating bone mass and influencing prospective fracture frequency in a well-described, large cohort of normal, ambulatory Australian women. A total of 1301 women wer
