Practice Patterns and Outcomes of Hemodialysis in Infants Undergoing Congenital Heart Surgery in the United States.

Hemodialysis after infant congenital heart surgery (CHS) for acute kidney injury (AKI) presents a major challenge due to relatively low utilization, difficult vascular access and small patient size. Therefore, we performed a multi-institutional analysis for infants requiring hemodialysis post-CHS. The Pediatric Health Information System (PHIS) was queried for infants undergoing CHS from 2004 to 2024. Intermittent hemodialysis (iHD) and continuous renal replacement therapy (CRRT) were included, patients undergoing pre-CHS dialysis or post-CHS peritoneal dialysis during admission were excluded.

Workforce Wellbeing and Future Pandemic Preparedness in Emergency Departments: Lessons Learned While Caring for Persons Experiencing Homelessness During COVID-19.

Background/Objective: Research has indicated that providing care during the COVID-19 pandemic negatively impacted healthcare providers' wellbeing and burnout. Much of the literature has focused on emergency department (ED) physicians and nurses, including the distinct challenges they faced caring for persons experiencing homelessness (PEH), including addressing unmet basic needs, limited discharge options, and disruptions to community-based health and social services during the pandemic. Comparatively little research has examined the experiences of other multidisciplinary team members caring f

Long-term impact of 10-valent pneumococcal conjugate vaccine among children <5 years, Uganda, 2014-2021.

Pneumonia is the second leading cause of hospital admissions and deaths among children <5 years in Uganda. In 2014, Uganda officially rolled out the introduction of the pneumococcal conjugate vaccine (PCV) into routine immunization schedule. However, little is known about the long-term impact of PCV on pneumonia admissions and deaths. In this study, we described the trends and spatial distribution of pneumonia hospital admissions and mortality among children <5 years in Uganda, 2014-2021. We analysed secondary data on pneumonia admissions and deaths from the District Health Information System

Utilization of Intensive Care Interventions in Critically Ill Patients With Candidemia Versus Bacteremia: A Multicenter Retrospective Cohort Study.

OBJECTIVES: Candida species are one of the most common causes of ICU-onset bloodstream infection (BSI). Yet, there is no robust guidance on when to initiate empiric antifungals for ICU patients suspected of BSI. We compared patients with ICU-onset candidemia vs. ICU-onset bacteremia to determine which patients may benefit from empiric antifungals. DESIGN: We compared characteristics between patients with bacteremia vs. candidemia at time of culture. We determined risk factors for death and constructed a multivariable regression to determine if candidemia is an independent risk factor for deat

Revisiting risk factors and incidence of postoperative tachyarrhythmias in pediatric cardiac surgery.

Postoperative tachyarrhythmias are a significant complication following pediatric congenital heart surgery. This study revisits the incidence and risk factors for postoperative tachyarrhythmias, focusing on congenital heart disease with a higher propensity for arrhythmias. We aim to determine if surgical and perioperative improvements have mitigated these risks. The findings may inform clinical practices and guide the development of preventive strategies. This retrospective cohort study was conducted at a tertiary university hospital, including pediatric patients under 18 years of age who unde

Initial high-dose β-blockers toleration with reduced ejection fraction heart failure of hospitalized hypertensive patients.

BACKGROUND: β-blockers (BB) are the cornerstone of treatment for heart failure with reduced ejection fraction (HFrEF). However, due to potential adverse effects, guidelines recommend starting with a low dose and gradually titrating upwards. Hypertensive patients with HFrEF tend to have better drug responsiveness and prognosis, but it remains unclear whether they can tolerate initial high-dose BB. Therefore, this study aims to assess the tolerance of this population to initial high-dose BB therapy. METHODS: A retrospective observational study included 307 hypertensive patients with HFrEF who i
