A case is reported in which excision of the jugular bulb during glomus tumor surgery resulted in the development of postoperative benign intracranial hypertension. The pathogenesis of this previously unreported complication of glomus tumor surgery is discussed, and the principles of management are reviewed.

We studied changes in intestinal microvascular permeability resulting from chronic arterial hypertension. Normotensive dogs and dogs made chronically hypertensive utilizing the one-kidney, one clip Goldblatt technique were used to obtain values for: arterial pressure, portal pressure, intestinal lymph flow, and the lymph-to-plasma protein concentration ratio (CL/Cp). The accuracy and reliability of quantitative gamma camera renography in screening for functionally significant renovascular hypertension was investigated. Renal perfusion, total and relative renal function, and parenchymal transit times were measured in 141 patients with recently diagnosed or severe hypertension. The effects on blood pressure, the renin-angiotensin-aldosterone and the kallikrein-kinin systems were investigated in 32 patients with primary hypertension WHO stage I-II treated with captopril. Infarct-kidney hypertension was induced in congenital athymic nude rats, and in their immunologically normal haired littermates. Percutaneous transluminal angioplasty was successfully used in the treatment of renovascular hypertension that resulted from fibromuscular hyperplasia in a 26-year-old woman who was four weeks' pregnant. Results of medical and surgical treatment of renovascular hypertension were compared retrospectively in 28 patients. Renal function remained normal throughout the study in all patients. After six months of follow-up, systolic and diastolic blood pressures were significantly lower in the 15 patients treated surgically than in the 13 patients treated medically. A radiological opacity of the posterior and inferior mediastinum should evoke a lesion of vascular origin. The venous nature of the mass can be easily confirmed by morphological changes apparent during the respiratory cycle and changes in position. Investigations to establish etiology of this venous ectasia arising from the azygos vein require more extensive radiological examinations, and computed tomography may be of value. Portal hypertension is one cause of increased venous blood flow in the azygos systems, and should be systematically considered.

A boy was found on the day of birth to have hypertension and radiological evidence of calcification of the left renal vein. Persistent hypokalaemia and hyper-reninaemia in the presence of a small left kidney and normal right kidney led to the decision to perform a left nephrectomy. The biopsied specimen showed old calcified renal vein thrombosis with accompanying medullary necrosis. Postoperatively hypertension resolved.
