Initial supplementary oxygen concentration for moderate-late preterm infants receiving respiratory support in the delivery room: study protocol for the multicenter, cluster-randomized, crossover AIROPLANE trial.

BACKGROUND: Moderate-late preterm infants born at 32-35 completed weeks' gestation constitute a large proportion of all preterm births (< 37-week gestation), yet they are not well represented in the newborn resuscitation literature. Preterm infants often receive respiratory support in the delivery room, and recommendations exist to guide the use of supplemental oxygen when providing this support. However, there are minimal data regarding the best initial supplementary oxygen concentration for moderate-late preterm infants requiring respiratory support at birth, resulting in practice variation.

Cerebral monitoring of very preterm infants with anterior cerebral artery resistive index and early NIRS.

BACKGROUND: The prediction of adverse conditions in the preterm neonatal brain might be improved by cerebral monitoring using combined measures of cerebral function, including oxygenation and blood flow parameters. To perform the consecutive measurements of the resistive index (RI) from the anterior cerebral artery (ACA) within the first week of life and to evaluate the association of these measurements with cerebral oxygen saturation (Csat) detected by near-infrared spectroscopy (NIRS). METHODS: This prospective cohort study enrolled very preterm infants, <32 weeks of gestational age, admitt

Increased newborn NICU admission for evaluation of hypoxic-ischemic encephalopathy during COVID-19 pandemic in a public hospital.

BACKGROUND: Prenatal and perinatal care of pregnant mothers has been adversely affected during the COVID-19 pandemic. Hypoxic-ischemic encephalopathy (HIE) is a leading cause of neonatal death and long-term neurological disabilities. Therapeutic hypothermia is effective for neonatal HIE. This study evaluated the effect of the pandemic on neonatal HIE. METHODS: This retrospective single-center study compared neonatal HIE evaluation and hypothermia treatment between pre-COVID-19 pandemic (1 January 2018-31 December 2019) and COVID-19 pandemic (1 January 2020-31 December 2021) periods. Adverse effects and short-term developmental outcomes of infants exposed to atypical antipsychotics during breastfeeding.

BACKGROUND: Postpartum period in women is vulnerable to the occurrence and exacerbation of psychiatric disorders. Mothers with postpartum psychosis or bipolar disorder need treatment with psychotropic medications, especially atypical antipsychotics. However, many mothers and families will have reservations about the use of psychotropics during the perinatal period, particularly during breastfeeding because of its presumed side effects and adverse developmental outcomes of the child. Since there are limited data in this area, the present study aimed to examine the adverse effects, if any, and t

Comparing First- and Second-year Palivizumab Prophylaxis in Patients With Hemodynamically Significant Congenital Heart Disease in the CARESS Database (2005-2015).

BACKGROUND: Respiratory syncytial virus hospitalization (RSVH) rates in children <2 years of age with hemodynamically significant congenital heart disease (HSCHD) are 2- to 4-fold higher compared with healthy term infants. Pediatric recommendations differ as to whether palivizumab is beneficial beyond 1 year of age. The objective of this study was to determine whether differences exist in respiratory-related illness hospitalization (RIH) and RSVH in HSCHD infants receiving palivizumab during the first year versus second year of life in the Canadian Registry of Palivizumab. METHODS: The Canadi

Parenteral amino acid and metabolic acidosis in premature infants.

BACKGROUND: Aggressive parenteral nutrition (PN) including amino acids is recommended for low-birth-weight infants to prevent energy and protein deficit. Their impact on acid-base homeostasis has not been examined. METHODS: We investigated the impact of dose and duration of parenteral amino acids, with cysteine, on acid-base parameters in 122 low-birth-weight infants. Premature infants <or=32 weeks, <or=1850 g, and receiving parenteral amino acids at 1.5 g/kg/d for an extended period (>24 hours), or 3 g/kg/d for a short (5 hour), extended (24 hour), or prolonged (3-5 days) duration were inclu

Partial pressure of carbon dioxide in extremely low birth weight infants supported by nasal prongs continuous positive airway pressure.

OBJECTIVE: Traditionally, delivery room management of extremely low birth weight (ELBW) infants consisted of immediate intubation and mechanical ventilation. There have been recent reports of success using nasal prongs continuous positive airway pressure (NCPAP) in this population. Data on the partial pressure of carbon dioxide (PCO(2)) in spontaneously breathing ELBW infants is very limited. The objective of this study was to determine the trend of the average PCO(2) in the spontaneously breathing ELBW infants, while on NCPAP, during the first week of life and to determine whether a brief per

Swallowing dysfunction in infants less than 1 year of age.

BACKGROUND: Gastroesophageal reflux (GER) occurs frequently in infants and children and is implicated as a trigger for reactive airways disease. While evaluating patients for GER by upper gastrointestinal studies (UGI), we frequently noticed laryngeal penetration or tracheal aspiration in infants < 1 year of age. We conducted this prospective study to assess the incidence of swallowing dysfunction in infants with vomiting or respiratory symptoms. METHODS: Between February 1994 and August 1997, 1,003 UGI in infants < 1 year of age were performed by two experienced pediatric radiologists. Fluor

Experiencing acute genomic care: perspectives from parents in the neonatal and paediatric intensive care units towards rapid genomic sequencing.

Rapid genomic sequencing (rGS) is increasingly used in neonatal and paediatric intensive care units (ICUs) to inform diagnosis and guide management of critically ill infants and children. Although rGS has a high diagnostic yield and potential to influence treatment and care planning decisions, little is known about how families experience rGS in the ICU and the emotional and contextual factors influencing their testing-related decisions. We conducted semi-structured interviews with twenty-three parents of infants who consented to rGS in an ICU at two tertiary hospitals in Toronto, Ontario, Can

Conservative Management of Extensive Type V Aplasia Cutis Congenita in a Term Neonate Following Fetus Papyraceus: A Case Report and Guide for Outpatient Pediatric Management.

BACKGROUND: Type V aplasia cutis congenita (ACC) associated with fetus papyraceus presents management challenges for general pediatricians. While surgical intervention is often considered for extensive defects, conservative management may be effective with appropriate patient selection and protocols. To our knowledge, this is one of the most extensive cases of Type V ACC managed conservatively in the outpatient literature, and it provides a reproducible wound care protocol where none currently exists. CASE: We report a female infant born at 37 weeks gestation with extensive Type V ACC involvi

Maternal contact with Child Protective Services around childbirth and postpartum contraception.

In the U.S., child protective services (CPS) frequently intervenes with mothers and infants around the time of childbirth, as newborns are particularly vulnerable to adverse exposures and peripartum women and their infants are highly visible to mandated reporters, especially health care providers. In this study, we explored whether involvement with CPS around childbirth is related to whether mothers receive contraception in the postpartum period. Using population-level administrative data, we conducted discrete-time survival analyses to assess the association between maternal CPS involvement a

INTRODUCTION: Inflammatory bowel disease (IBD), which includes Crohn's disease and ulcerative colitis, often affects individuals during their peak reproductive years. Female patients with IBD face unique challenges across the reproductive years, from fertility and conception to delivery and lactation. Despite increasing awareness, knowledge gaps remain regarding optimal management during pregnancy and postpartum. This review outlines these challenges and provides a practical, evidence-based approach across reproductive stages. AREAS COVERED: This review summarizes care for IBD in reproductive

Influenza Vaccination Coverage During Pregnancy - Selected Sites, United States, 2005-06 Through 2013-14 Influenza Vaccine Seasons.

Seasonal influenza vaccine is recommended for all pregnant women because of their increased risk for influenza-associated complications. In addition, receipt of influenza vaccine by women during pregnancy has been shown to protect their infants for several months after birth (1). As part of its case-control surveillance study of medications and birth defects, the Birth Defects Study of the Slone Epidemiology Center at Boston University has recorded data on vaccinations received during pregnancy since the 2005-06 influenza vaccination season. Among the 5,318 mothers of infants without major str
