Workforce Detachment in Persons with CKD.

KEY POINTS: In this study, we identified individuals in the workforce with estimated kidney function based on sequential serum creatinine measurements. Individuals with CKD were more likely to experience workforce detachment. The risk of workforce detachment was graded according to CKD severity. These findings were persistent irrespective of age, sex, diabetes status, and level of education. BACKGROUND: Impaired kidney function is associated with mortality, cardiovascular events, and hospitalization as well as diminished functional capacity and health-related quality of life. We hypothesized

Humanistic care in the ICU: effects on patient dignity, delirium and family satisfaction - a non-randomised clinical trial protocol.

INTRODUCTION: Critical care patients face varying degrees of physical and psychological stress; in recent decades, the increasing focus on technologies and clinical processes has led to dehumanisation. In contrast, humanistic care has been introduced as an essential care paradigm, emphasising comprehensive care and considering all stakeholders in the care process (patient, family and staff). In addition to improving clinical outcomes, this approach focuses on family satisfaction, preserving the human dignity of patients and reducing the incidence of acute cognitive disorders such as delirium;

Barriers and facilitators of mental health app use among young women - a multimethod study.

BACKGROUND: Prevalences of psychological distress among young women are high and rising. Despite a vast number of potentially helpful mental health apps (MHA), dissemination is limited. The current study aims to inform user-centered development and implementation of mental health apps for young women by examining mental health problems in the target population, existing help-seeking patterns and their gaps, perceived barriers to both conventional and digital care, and preferred MHA characteristics. METHODS: 521 women between the ages of 18 and 35 participated in a cross-sectional online surve

Development of an artificial intelligence prediction model for moderate-to-severe COPD exacerbations using continuous multiple unobtrusive sensors: protocol of a multicentre prospective observational study.

INTRODUCTION: Exacerbations, impaired health-related quality of life (HRQoL) and reduced exercise capacity increase the risk of hospitalisations and death in chronic obstructive pulmonary disease (COPD). However, their monitoring relies on in-person assessments, potentially delaying early care. While smart sensing technologies can enable remote monitoring, their use in predicting disease worsening remains limited. The TOLIFE Clinical Study A (CSA) aims to develop an artificial intelligence (AI) model integrating clinical data with smart sensing devices data to predict exacerbation onset and ch

Effects of pain neuroscience education combined with blood flow restriction training on chronic postoperative pain following total knee arthroplasty: a randomized controlled trial protocol.

INTRODUCTION: Chronic postsurgical pain (CPSP) remains a major clinical challenge following total knee arthroplasty (TKA), with psychological maladaptation and persistent neuromuscular dysfunction considered key contributors to ongoing pain and disability. Pain neuroscience education (PNE) may improve maladaptive pain cognitions, whereas blood flow restriction training (BFRT) can enhance muscle function and induce exercise-related hypoalgesia. Although both interventions have shown potential benefits in chronic pain rehabilitation, no randomized controlled trial has investigated the combined e

Effects of non-invasive spinal cord stimulation on autonomic function in individuals with subacute spinal cord injury: A pilot clinical trial protocol.

INTRODUCTION: Spinal cord injury (SCI) at or above the thoracic sixth spinal cord level disrupts descending sympathetic and parasympathetic control, leading to severe autonomic dysfunctions including cardiovascular and pelvic organ function. These complications adversely affect the quality of life and are associated with increased morbidity and mortality after SCI. Transcutaneous spinal cord stimulation (tSCS) may offer therapeutic benefits for these functions. The safety of tSCS in subacute SCI, however, remains unknown. Therefore, this study aims to evaluate the feasibility of tSCS for auton

Non-inferiority of saline versus corticosteroid injections for trapeziometacarpal osteoarthritis: a protocol for a pilot and feasibility randomised controlled trial.

INTRODUCTION: Trapeziometacarpal osteoarthritis (TMO) is among the most prevalent forms of upper extremity osteoarthritis. It is frequently associated with significant levels of pain and disability, particularly among ageing women. The typical care pathway for TMO relies on non-surgical approaches for up to 2 years, before surgery is considered. One of the most common non-surgical approaches is an intra-articular cortisone injection. However, these are not universally recommended due to their safety profile and unclear efficacy compared with saline injections. Recent evidence suggests that sal

FinnDiane LifeOne Study: influence of ageing on people with type 1 diabetes - a prospective observational cohort study protocol.

INTRODUCTION: Life expectancy for people with type 1 diabetes has increased due to improved treatment of diabetes and its comorbidities, allowing many to reach old age. Still, we lack knowledge of how individuals with type 1 diabetes age. On one hand, those who reach older age can be considered survivors, but on the other hand their long-standing diabetes might still exhibit negative impacts on their health and functional ability. Healthy ageing is the WHO's priority for this decade. The focus has shifted from chronological age to functional ability, which reflects the ability of individuals t

Efficacy of high-flow nasal cannula versus conventional face mask oxygen therapy on ipsilateral cerebral haemodynamics during the immediate post-extubation phase following carotid endarterectomy: protocol for a randomised controlled trial.

INTRODUCTION: Cerebral hyperperfusion syndrome (CHS) is a severe complication following carotid endarterectomy (CEA). During emergence from general anaesthesia, residual anaesthetics induce mild respiratory depression and carbon dioxide accumulation, exacerbating hyperperfusion risks via hypercapnia-induced cerebral vasodilation. High-flow nasal cannula (HFNC) effectively reduces excessively elevated pressure of arterial carbon dioxide via multiple mechanisms including anatomical dead space washout. This trial aims to evaluate the effects of HFNC versus conventional mask oxygen therapy on ipsi

Psychological Versus Somatic Correlates of Adolescent Suicide Attempts: A 21-Year National Time-Series and Path-Analytic Study in South Korea (2005-2025).

Background: South Korea reports one of the highest adolescent suicide rates among OECD countries. Both psychological factors (perceived stress, depression) and somatic/behavioral factors (atopic disease, obesity, diet, physical activity) have been proposed as correlates of adolescent suicidality, but the extent to which their co-occurring long-term national trends reflect genuine, independent associations-rather than a shared secular (time) trend-has rarely been tested at the population level. Methods: We compiled national annual prevalence estimates (2005-2025) from the Korea Youth Risk Behav

Serum and CSF cytokine profile impact clinical phenotype in pediatric MOGAD.

BACKGROUND: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) has a broad spectrum of clinical phenotypes, including acute disseminated encephalomyelitis (ADEM), optic neuritis (ON), transverse myelitis (TM), and cerebral cortical encephalitis (CCE). Although these conditions exhibit diverse outcomes and require different treatment options for relapse prevention, no biomarkers have been identified to distinguish the clinical phenotypes. This study aimed to investigate the properties of cytokines and chemokines in the serum and cerebrospinal fluid (CSF) of patients with MO

TIDES 2.0: A protocol for examining the prevalence and longitudinal course of depression, anxiety and behaviour problems in children with cystic fibrosis under 12 years of age.

INTRODUCTION: Integration of mental health screening and treatment into cystic fibrosis (CF) care has been driven by the finding of elevated rates of depression and anxiety in The International Depression/Anxiety Epidemiological Study (TIDES), international mental health guidelines and CF Foundation support to screen adolescents and adults in all US CF care centres. Benefits of screening include earlier identification, greater access to care, reduced stigma and positive uptake from the CF community. However, TIDES did not include children under 12 years. Depression and anxiety have increased d

ITARA research programme: Investigating Integrated Tuberculosis And Respiratory care in Africa using transdisciplinary methods.

INTRODUCTION: Pulmonary tuberculosis (PTB) and chronic respiratory diseases (CRDs) are closely linked. Affected groups present with similar symptoms and share many risk factors (eg, poverty-related factors, smoking, occupational exposures). PTB is itself an independent risk factor for chronic lung disease. However, in many high TB-incidence settings health services for these conditions are provided separately, with little integration of prevention, diagnosis or care. METHODS AND ANALYSIS: We describe a transdisciplinary programme of research investigating strategies for integrated TB-CRD care

From Organoids to Organ-on-Chip: Advancing Human-Relevant Models for Viral Pathogenesis and Antiviral Drug Discovery.

Organoid and organ-on-chip technologies are rapidly evolving platforms for viral research that integrate stem cell biology, tissue engineering, and microfluidics to recapitulate key structural, mechanical, biochemical, and cellular features of human and animal physiology. By incorporating multicellular organoids into perfused microfluidic systems, these models can provide complex, dynamic, and physiologically relevant micro-environments for investigating virus-host interactions that are difficult to capture in conventional two-dimensional cultures and static organoids. Controlled flow, shear s

Utilization of Prognosis Assignment: A Cross-Sectional Survey of Military Periodontists.

INTRODUCTION: Prognosis is a forecast of the likely course of a disease and its future implications on treatment outcomes. The significance of periodontal prognosis in clinical practice is evident by its influence on treatment planning, communication, and in the retrospective evaluation of treatment results. While there are multiple well-known prognosis systems reported in the periodontal dental literature, there is an absence of data on the actual use of prognosis assignment among practicing clinicians. The purpose of this study was to survey military periodontists regarding their utilization

iHEART trial: study protocol for a German multicentre randomised controlled trial on the feasibility and acceptance of an internet-based preoperative intervention to optimise patient expectations and improve outcomes after heart surgery.

INTRODUCTION: Treatment expectations are a key mechanism of placebo effects in clinical trials. In a previous study (PSY-HEART-I), preoperative expectation optimisation improved quality of life 6 months postcardiac surgery. However, barriers such as travel distance, staffing shortages and COVID-19 limited participation. This study evaluates the feasibility and acceptability of iEXPECT, a brief internet-based intervention designed to optimise expectations before heart surgery. METHODS AND ANALYSIS: In this three-arm, multicentre randomised controlled trial, 160 patients undergoing elective cor

Examining the effect of direct-from-blood bacterial testing on antibiotic administration and clinical outcomes: a protocol and statistical analysis plan for a pragmatic randomised trial.

INTRODUCTION: Patients with suspected bacterial infection frequently receive empiric, broad-spectrum antibiotics prior to pathogen identification due to the time required for bacteria to grow in culture. Direct-from-blood diagnostics identifying the presence or absence of bacteria and/or resistance genes from whole blood samples within hours of collection could enable earlier antibiotic optimisation for patients suspected to have bacterial infections. However, few randomised trials have evaluated the effect of using direct-from-blood bacterial testing on antibiotic administration and clinical
