Physician Perspectives on Real-World Evidence for Prescribing to Older Patients.

BackgroundAvailable evidence for many prescription drugs comes from clinical trials that often exclude older patients. Real-world evidence (RWE) generated post-approval can provide new evidence on effectiveness and safety for these populations. We sought to understand physicians' perceptions of clinical trial and RWE for prescribing decisions. To frame the study, we used examples of drugs for heart failure and anticoagulation, which are more like to be prescribed to older adults.MethodsWe conducted an interview study of 15 US physicians specializing in geriatrics, cardiology, or primary care a

CMV-GLA: contrastive multi-view graph layer attention for predicting phosphorylation site-disease associations.

Protein phosphorylation is a critical posttranslational modification involved in cell signaling and metabolic regulation, and phosphorylation dysregulation is closely associated with various diseases. However, studies on phosphorylation site-disease associations remain scarce, and existing methods often fail to fully leverage complementary information across heterogeneous data sources, resulting in suboptimal predictive performance. We present CMV-GLA (Contrastive Multi-View Graph Layer Attention Network), a novel framework integrating protein sequences, disease semantics, and known associatio

DeepAden: an explainable machine learning method for predicting the substrate specificity of nonribosomal peptide synthetases.

Microbial nonribosomal peptides (NRPs) exhibit remarkable structural diversity and are important sources of lead compounds for clinical drug development. The biosynthesis of NRPs relies on nonribosomal peptide synthetases (NRPSs), in which adenylation (A) domains define the core structure by selectively recognizing and activating amino acid substrates. Accurately predicting the substrate specificities of A-domains is thus essential for understanding the core structural and biosynthetic logic of NRPs. Here, we present DeepAden, a two-stage deep learning model. In the first stage, a graph attent

Exploring the Antiparasitic Activity of the Sea Cucumber Isostichopus sp. aff. badionotus From the Northern Coast of Colombia Against Trypanosoma cruzi.

Chagas disease, caused by Trypanosoma cruzi, remains a neglected tropical infection with limited therapeutic options. Sea cucumbers are recognized as rich sources of bioactive sulfated metabolites with potential antiparasitic properties; however, information on the chemistry and trypanocidal activity of Caribbean Isostichopus species is still scarce. In this study, the trypanocidal activity of solvent fractions from Isostichopus sp. aff. badionotus was evaluated, together with the chemical characterization and molecular docking analysis of their major metabolites. The methanolic extract, obtai

Mesenchymal vs. induced pluripotent stem cells: potential for spiral ganglion neuron regeneration in auditory neuropathy.

Auditory neuropathy is a distinct form of sensorineural hearing loss characterized by dysfunction or degeneration of primary auditory neurons, also known as spiral ganglion neurons (SGNs), which transmit acoustic information from the cochlea to the brain. Increasing evidence indicates that SGNs are particularly susceptible to degeneration induced by noise exposure, ototoxic agents, genetic mutations, or aging, often preceding the loss of cochlear mechanosensory hair cells, and thus represents a critical target for regenerative intervention for auditory neuropathy. Stem cell-based approaches ha

Questions and issues of nonventilated hospital-acquired pneumonia: an opinion document.

BACKGROUND: Nonventilated hospital-acquired pneumonia (NV-HAP) is a significant cause of morbidity and mortality within healthcare settings. Despite advancements in diagnostic and therapeutic approaches over recent decades, the supporting evidence for diagnostic criteria remains limited. Information regarding the aetiology of this condition is scarce, and many methodological aspects related to the collection and analysis of respiratory samples are still under debate. Furthermore, recommendations from leading scientific societies on these topics vary and have not been updated in many instances.

Attitudes, Strategies and Value Frameworks Guiding Risk Communication in Isolated REM Behaviour Disorder: A Scoping Review of Perspectives From Patients and Healthcare Professionals.

Idiopathic/isolated REM sleep behaviour disorder (iRBD) is widely recognized as a key prodromal condition associated with a high risk of progression to α-synucleinopathies. Despite this, disclosure of risk related to such early signs of neurodegeneration remains controversial in clinical practice. This scoping review aims to systematically analyse literature regarding the risk disclosure, focusing on attitudes and preferences of patients as well as healthcare professionals, communication strategies used, emotional and behavioural processes that emerge pre, during and post disclosure in differe

Out of area placements for people with "Personality Disorder": Making the case for a local intensive psychotherapeutic alternative.

OBJECTIVES: The use of Out of Area (OoA) psychiatric placements for people with "Personality Disorder" (PD) is widespread in the UK. An innovative local intensive psychotherapeutic service, adapted to the transdiagnostic presentations of the most complex PD patients, likely to be placed out of the area, was devised in the English County of Devon. This paper reports the findings of a Freedom of Information (FOI) request to commissioners attempting to quantify PD OoA placements in England and the cost offset of the local therapeutic alternative to OoA placements in Devon. Scaling Up Evidence-Based Supported Employment in the United States.

Implementing evidence-based practices has become a standard policy in U.S. behavioral health care. However, current penetration remains inadequate, and longitudinal studies of scaling up these practices are rare. This article examined the steady, 3-decade growth of individual placement and support (IPS), the evidence-based model of employment services for people with psychiatric disabilities. The number of IPS programs in the United States has increased to >1,000. Nevertheless, coverage remains limited, and challenges to further growth are substantial. Facilitators of IPS growth over the past
