Screening Candidates for Refractive Surgery With Corneal Tomographic-Based Deep Learning.

IMPORTANCE: Evaluating corneal morphologic characteristics with corneal tomographic scans before refractive surgery is necessary to exclude patients with at-risk corneas and keratoconus. In previous studies, researchers performed screening with machine learning methods based on specific corneal parameters. To date, a deep learning algorithm has not been used in combination with corneal tomographic scans. OBJECTIVE: To examine the use of a deep learning model in the screening of candidates for refractive surgery. DESIGN, SETTING, AND PARTICIPANTS: A diagnostic, cross-sectional study was condu…

Small-incision lenticule addition in ex vivo model of ectatic human corneas.

PURPOSE: To investigate the feasibility of intrastromal lenticule insertion to restore corneal shape in a model of ectatic human cornea. METHODS: For this experimental ex vivo study on 34 human corneas unsuitable for transplantation, 17 corneas were thinned by decentralized posterior excimer laser ablation to 200 μm thickness and 6.5 mm diameter and then inflated up to 100 mm Hg to expose the ectasias (recipient corneas). Pachimetry and topography were obtained. Stromal lenticules of the same diameter and thickness as the ectasias were shaped with a femtosecond laser from the remaining 17 don

"Large-bubble" modification of the "big-bubble" technique for performing maximum-depth anterior lamellar keratoplasty.

PURPOSE: To describe a new technique for performing maximum-depth anterior lamellar keratoplasty. METHODS: This was a case series study of a novel method. We combined shallow trephination of the cornea with superficial injection of air in 15 cases. This resulted in formation of a large bubble over Descemet membrane extending to the peripheral cornea. We deflated the bubble at the trephination wound and excised the button from the host completely. The follow-up was from 3 to 8 months. The best spectacle-corrected visual acuity, intraocular pressure, corneal clarity, endothelial cell count, and

Acute corneal hydrops with perforation after LASIK-associated keratectasia.

Herein a case of acute corneal hydrops with perforation in keratectasia 9 years after bilateral laser in situ keratomileusis is presented. A 27-year-old man presented with right eye sudden loss of vision. Right eye best-corrected visual acuity was counting fingers. Left eye best-corrected visual acuity was 6/9. Right eye slit-lamp examination revealed diffuse corneal oedema with central epithelial cystic change. The interface between the flap and residual stroma was open and filled with fluid. This separation extended nearly to the flap margin. Shallow anterior chamber and aqueous leakage were

[Posterior corneal curvature after penetrating keratoplasty before and after suture removal].

PURPOSE: After penetrating keratoplasty (PK), peripheral swelling effects, malapposition of the graft in the recipient bed and suture tension may result in a changed ratio of curvature between anterior and posterior corneal surface. The purpose of this pilot study was to assess the posterior corneal curvature after PK with and without sutures and to compare this data to the posterior curvature of normal individuals. PATIENTS AND METHODS: In this cross-sectional study, 44 eyes after PK with a double running 16-bite diagonal antitorque suture in place (mean age 37 +/- 14 years, spherical equiva

Is raised intraocular pressure a bad prognostic sign in acute corneal graft rejection?

One hundred and forty acute corneal rejection episodes in 94 patients were studied retrospectively. Sixteen episodes in 15 eyes were associated with raised intraocular pressure (IOP) on admission, three of whom had had previously elevated IOP. At six weeks, six (37.5%) still required hypotensive therapy. Five eyes with raised IOP at rejection had lost vision at six weeks. Five of the six eyes with graft failure at review had raised IOP either pre-graft, at rejection or at follow-up. Eyes grafted for herpes simplex keratitis with hypertensive rejection episodes had a higher mean admission IOP,

Ocular complications of atopic dermatitis.

Atopic dermatitis is a common skin disease associated with other allergic diatheses. Ocular complications were seen in 85 (42.5%) of 200 patients. Blepharoconjunctivitis, cataracts, corneal disease and ocular herpes simplex were frequent. Cataract surgery was usually successful, but there were complications of such surgery. The rate of success in attempts to wear contact lenses was surprisingly high for both phakic and aphakic patients.

Treatment of Corneal Ectasia by Implantation of an Allogenic Corneal Lenticule.

PURPOSE: To describe a case in which post-LASIK corneal ectasia was treated using an allogeneic lenticule implanted into the recipient eye. METHODS: A 29-year-old man presented with bilateral ectasia 10 years after LASIK. Manifest refraction was -1.50 -3.25 × 15° in the right eye and -13.50 -6.00 × 10° in the left eye. Because of contact lens intolerance in the left eye, the authors decided to implant an allogenic corneal lenticule, created by small incision lenticule extraction with the VisuMax laser (Carl Zeiss Meditec, Jena, Germany) in a donor eye with a refraction of +5.75 -0.50 × 15°.
