A case of prolonged hyperemesis resulting in hepatorenal failure, foetal distress and neonatal mortality.

Hyperemesis gravidarum is defined as a condition emerging during pregnancy, is prominent with severe nausea and vomiting, a water-electrolyte imbalance and liver-kidney function disorder and causes more than 5% loss of body weight. In cases of severe hyperemesis gravidarum, maternal morbidities such as water-electrolyte imbalance, hepatorenal failure, Wernicke's encephalopathy, splenic avulsion, oesophageal rupture, pneumothorax have been reported. We present a case of hyperemesis gravidarum, which continued until the third trimester and developed secondary hepatorenal failure, a water-electro

Sigmoid Volvulus Complicating Postpartum Period.

Background. Sigmoid volvulus is a rare complication of pregnancy and the puerperium. Case. A 19-year-old patient, gravida 1 para 0 at 41 0/7 weeks of gestation, admitted for late-term induction of labor underwent an uncomplicated primary low transverse cesarean delivery for arrest of descent. Her postoperative period was complicated by sudden onset of abdominal pain and the ultimate diagnosis of sigmoid volvulus. Conclusion. Prompt surgical evaluation of an acute abdomen in the postpartum period is essential; delayed diagnosis and treatment can lead to significant maternal morbidity and mortal

Multiple repeat cesareans and the threat of placenta accreta: incidence, diagnosis, management.

Placenta accreta is a significant source of obstetric morbidity and mortality. Its incidence is increasing as a direct consequence of the increasing cesarean section rate, which reflects increased rates of maternal obesity, increased numbers of multiple gestations secondary to assisted reproductive technology, physician concern about litigation for adverse obstetric outcome, and a decline in the use of operative vaginal delivery for both cephalic and breech presentations. Optimum management for most cases requires elective cesarean hysterectomy, ideally performed at about 34 weeks' gestation.

Placement of a vena cava filter in term pregnancy: case report and review of the literature.

During pregnancy there are hemostatic changes that may result in a hypercoagulable state producing thrombotic consequences. This condition may be aggravated in women who are carriers of congenital thrombophilic factors. These factors may increase obstetric complications such as miscarriages, fetal growth restriction, placental abruption and preeclampsia. Trombophilic factors may also cause venous thromboembolism, which is the leading cause of maternal morbidity and mortality. We report a case of a 22-year-old woman with factor V Leiden mutation, whose pregnancy was complicated with deep venous

Motor vehicle crashes, pregnancy loss and preterm labor.

Trauma during pregnancy is remarkably common, and is greatly underestimated in terms of its contribution to both maternal and perinatal morbidity and mortality. Motor vehicle crashes, falls, assaults, including domestic violence, are all important mechanisms of injury, and clinical algorithms have been developed to manage the injured pregnant women. Focus on recognition and management of the most common injuries seen in pregnant women, namely abruptio placentae and uterine rupture are addressed through hemodynamic stabilization and continuous fetal and uterine contraction monitoring in the wom

Pregnancy in patients with mitral valve prolapse.

The obstetrical performances and outcomes of 37 pregnancies in women with mitral valve prolapse between 1979 and 1982 are reviewed. Thirteen patients were diagnosed before pregnancy and 24 patients were detected at antenatal examinations. Three ended in cesarean sections for obstetrical complications and 34 in uneventful vaginal deliveries at term. No cardiac complications occurred in these patients. There was no maternal mortality. Thirty-six babies were born without congenital abnormalities. One baby was hydropic due to haemoglobinopathy and died. Prophylactic antibiotics is recommended in s…

Neural tube malformations: complex segregation analysis and calculation of recurrence risks.

Familial data on neural tube malformations in Great Britain were submitted to segregation analysis under the mixed model. Maternal and fetal factors cannot be discriminated in the absence of substantial bodies of data on spina bifida survivors who reproduce or on half-sibs. Early abortion studies would allow differential mortality in utero to be taken into account. After fitting the mixed and generalised single locus models, it is concluded that the multifactorial model can provisionally be used for calculation of recurrence risks. Pathogenic hypotheses implicating twinning seem to rest on lit

[The avoidable neonatal injuries during delivery (author's transl)].

The authors describe their approach which was designed to meet three imperatives, namely: --to prevent severe foetal distress, --to detect severe foetal distress by systematic use of the cardiotocography, --to avoid unnecessary interventions by supervising the metabolic state of the mother and the foetus which makes it possible to distinguish foetal acidosis by hypoxia necessitating rapid intervention from foetal acidosis of maternal origin for which extraction is not generally necessary. Their action has made it possible to reduce the number of caesarian during labour which achieving extremel

Association between activated partial thromboplastin time and septic shock in pregnant women: a retrospective cohort study.

Maternal sepsis is a life-threatening infectious complication, with septic shock linked to high mortality. Coagulopathy is a key feature, but the association between activated partial thromboplastin time (APTT) and maternal septic shock remains unclear. This retrospective study included 1,631 pregnant women with infection admitted from January 2017 to March 2023. Based on admission APTT levels, patients were divided into three groups: <28 s (n = 697), 28-42 s (n = 915), and >42 s (n = 19). A U-shaped relationship was observed between APTT and septic shock risk. Compared to the reference group

NLRs and preeclampsia: Advances in etiology, pathogenesis, and therapeutic targeting, highlighting NLRP3.

Preeclampsia (PE) is a complex pregnancy disorder. It is characterized by hypertension and multi-organ dysfunction, significantly contributing to maternal and fetal morbidity and mortality. Recent studies underscore the important contribution of NOD-like receptors (NLRs), particularly the NLRP3 inflammasome, to PE development. This review synthesizes the most recent data on NLRs in PE, focusing on their role in disease genesis, pathogenic processes, biomarker potential, and therapeutic consequences. Key characteristics of PE include dysregulated NLR activity, excessive inflammation, oxidative

Non-immune hydrops fetalis (NIHF): diagnostic evaluation and lessons learned from a case series.

BACKGROUND: Fetal hydrops, characterized by abnormal fluid accumulation in two or more fetal compartments, is a critical condition with a poor prognosis, often resulting in significant fetal mortality. The condition itself is not a diagnosis but a clinical sign with many underlying causes, ranging from infections and chromosomal abnormalities to rare maternal conditions such as mirror syndrome. CASE PRESENTATION: This review examines three cases of fetal hydrops, highlighting common etiologies, such as infections and chromosomal disorders, and more unusual causes like maternal complications.

Stroke in pregnancy.

Maternal stroke, while rare, leads to significant maternal morbidity and mortality. Physiologic changes in pregnancy and the postpartum period disrupt cerebrovascular homeostasis, predisposing to pregnancy-specific syndromes and tripling stroke risk. Mechanisms of pregnancy-related stroke are diverse and include cardioembolism, vasculopathies (cervical artery dissections, reversible cerebral vasoconstriction syndrome), intracranial hemorrhage related to vascular lesions or preeclampsia, and cerebral venous thrombosis. Heterogeneity of presentation and limited experience treating pregnant women

Therapeutic potential of extracellular vesicles for treating human pregnancy disorders.

Pregnancy complications such as preeclampsia and fetal growth restriction are major global health concerns, contributing to significant maternal and fetal morbidity and mortality. These disorders also increase the long-term risk of cardiovascular, metabolic, and kidney diseases in both mother and child. Accumulating evidence highlights the important role of placental mesenchymal stromal cell (MSC)-derived extracellular vesicles (EVs) in both healthy and pathological pregnancies. In healthy pregnancies, EVs support placental development and maternal-fetal communication. In contrast, EVs from di

A comprehensive review of the roles of T-cell immunity in preeclampsia.

Preeclampsia (PE) is an obstetrical disorder that occurs after the 20th week of gestation. It is recognized as one of the "Great Obstetrical Syndromes" and principally contributes to maternal morbidity and mortality. PE has been associated with a range of immune disorders, including a preponderance of T helper (Th) 1 over Th2 cells and imbalanced levels of Th17 and T regulatory cells (Tregs). During pregnancy, T cells safeguard the placenta against immune rejection and aid embryo implantation while involved in pregnancy complications, such as PE. Promoting alloantigen-specific Treg cells is a

Current Approaches and Innovations in Managing Preeclampsia: Highlighting Maternal Health Disparities.

Preeclampsia (PE) is a major cause of maternal mortality and morbidity, affecting 3-6% of pregnancies worldwide and ranking among the top six causes of maternal deaths in the U.S. PE typically develops after 20 weeks of gestation and is characterized by new-onset hypertension and/or end-organ dysfunction, with or without proteinuria. Current management strategies for PE emphasize early diagnosis, blood pressure control, and timely delivery. For prevention, low-dose aspirin (81 mg/day) is recommended for high-risk women between 12 and 28 weeks of gestation. Magnesium sulfate is also advised to …

The effect of preterm premature rupture of membranes on neonatal outcomes in low-birth-weight infants: a retrospective study.

INTRODUCTION: This study evaluated the effect of preterm premature rupture of membranes (PPROM) on neonatal outcomes in low-birth-weight infants. METHODS: We included premature infants born in our hospital and divided them into PPROM group and non-PPROM group. Maternal and neonatal data were collected, and follow-up assessments were performed at 3, 6, and 9 months of corrected age. RESULTS: The PPROM group had significantly lower birth weights, gestational ages, higher maternal infection rates, longer hospital stays, and higher complication rate (p < 0.001). PPROM, 5-minute Apgar score <7, a

Burden and determinants of preeclampsia and eclampsia in Ugandan tertiary healthcare facilities: a systematic review.

Preeclampsia and eclampsia, hypertensive disorders of pregnancy, significantly contribute to maternal and perinatal morbidity and mortality in low- and middle-income countries like Uganda. Comprehensive data on their prevalence and determinants in Uganda's tertiary healthcare facilities are limited. A systematic review was conducted searching PubMed, Lens.org, and Google Scholar for observational studies from Ugandan tertiary hospitals published between January 2015 and March 2025. Due to heterogeneity, a narrative synthesis was performed using the Synthesis without Meta-analysis (SWiM) framew

Placenta Previa Major: Prenatal Diagnosis and Uterus-Preserving Surgical Management in a Case Study From the Kingdom of Bahrain.

Placenta previa major is a significant concern in maternal healthcare due to its potential to cause postpartum hemorrhage (PPH), which can lead to morbidity and mortality following childbirth. PPH is generally defined as the loss of 500 ml or more of blood from the genital tract within 24 hours of the birth of a baby. PPH can result from various factors, including uterine atony, retained placental tissue, trauma, and placenta previa. Placenta previa, where the placenta is abnormally positioned over or near the cervical opening, is particularly concerning due to its significant risks for both m
