Optimizing dural opening and closure in endonasal transsphenoidal surgery: Technical considerations for incision design and clinical utility.

Dural opening and closure represent a crucial yet under-discussed aspect of transsphenoidal surgery (TSS), where the operative field is narrow and cerebrospinal fluid (CSF) leakage remains a significant complication. In 2010, we abandoned the conventional cruciate dural incision and began developing optimized designs that provide wide exposure, minimal interference, and facilitate watertight closure. After investigating various incision types, we empirically established a modified H-shaped dural incision in 2011. This design minimizes the interdural gap, enabling effective dural flap tenting a

Comparative analysis of facial nerve outcomes in petroclival versus posterior petrous meningioma surgery: A systematic review and meta-analysis of 2884 patients.

BACKGROUND: Even conservative resection of petroclival (PC) and posterior petrous (PP) meningiomas may cause facial nerve (FN) morbidities. While inconsistent classifications by tumor's dural origin circulate among different cohorts, a comprehensive assessment of postoperative FN outcomes is lacking. We systematically evaluate early and long-term FN outcomes related to surgical extirpation of PC and PP meningiomas. METHODS: A PRISMA-tailored survey of PubMed, Embase, Ovid/Medline, and Scopus was used to assess pooled perioperative FN deficit rates in PC and PP meningiomas. Outcomes were dicho

Significance of the open A2 plane in determining the side of approach for microsurgical treatment of anterior communicating artery aneurysms.

OBJECTIVE: Choosing the side of approach (SoA) in the microsurgical treatment of anterior communicating artery (AComA) aneurysms remains a topic of debate. The aim of this study was to investigate the effectiveness of the open A2 plane as the decisive factor in choosing the SoA. METHODS: This retrospective cohort study analyzed data from 59 patients who underwent microsurgical treatment for AComA aneurysms from 2017 to 2024 at a single institution. For 19 patients, the SoA was dictated by an accompanying pathology such as another aneurysm or intracerebral hemorrhage. For 40 patients, the open

Nomogram to predict postoperative complications after surgical resection in adult patients with carotid body tumors.

Background: Surgical resection remains the primary treatment for carotid body tumors (CBTs), but it carries a considerable risk of postoperative complications. Although several predictors have been identified, no validated tool currently exists to estimate the individualized risk of complications in CBT patients. This study aimed to develop a nomogram for predicting postoperative complications following CBT resection. Methods: We retrospectively analyzed adult patients who underwent surgical resection for CBTs at our institution between 2016 and 2022. Potential predictors of postoperative comp

Comprehensive analysis of surgical treatments for meralgia paraesthetica: Case series and meta-analysis encompassing results of decompression vs neurectomy.

BACKGROUND: Meralgia paraesthetica (MP) is caused by lateral femoral cutaneous nerve (LFCN) compression. Surgical intervention is considered for refractory cases, with LFCN decompression being the most commonly performed procedures. In this study, we present the clinical outcomes of LFCN neurectomy performed at a single neurosurgical department. Our findings are juxtaposed meta-analysis assessing the success rates of different surgical strategies. MATERIAL AND METHOD: 14 LFCNs neurectomies were performed in 13 MP patients at our Department between 2015 and 2022. Follow-up evaluations were con

Median medullary perforators: Anatomical, surgical, and clinical significance in foramen magnum meningiomas.

The median medullary perforators, a group of perforator vessels arising from the vertebrobasilar junction, remain underrecognized. There is scarce knowledge regarding their clinical significance and relevance in foramen magnum surgery. This study elucidates the anatomy of these perforators and their clinical and surgical relevance, in analogy to the well-known vital basilar tip perforators. We highlight their potential contribution to selective pyramidal infarct or ischemia during foramen magnum tumor resection, and rarely their implication in isolated ischemic strokes of the medullary pyramid

[Frontal aslant tract: Anatomy and implications in brain glioma neurosurgery].

The frontal aslant tract (FAT) connects the supplementary motor area (SMA) with the pars opercularis. Its role in language and its implications in glioma surgery remain under discussion. We present an anatomosurgical study of three cases with surgical resolution. Three patients with gliomas in the left frontal lobe were operated on using an awake patient protocol with cortical and subcortical mapping techniques, conducting motor and language evaluations. Tractography was performed using DSI Studio software. All three patients showed intraoperative language inhibition through subcortical stimul

Neurophysiological identification and differentiation between the motor and sensory roots in pediatric spinal cord lipoma surgery.

BACKGROUND: Radical resection of spinal cord lipomas reduces the rate of re-tethering. Current conventional neurophysiological mapping techniques are not able to differentiate between crucial motor nerve roots and sensory roots. Enhanced differentiation could contribute to complete resection. We present our experience with a double-train paradigm to differentiate between motor and sensory roots. METHODS: In children undergoing spinal cord lipoma resection, the double-train mapping paradigm was used with an inter-train interval of 60 ms. Given the longer recovery time due to the H-reflex, a si

Planned Surgical Trajectory Affects Clinical Motor Outcome in Deep Brain Stimulation Targeted at Subthalamic Nucleus for Parkinson's Disease.

BACKGROUND AND OBJECTIVES: Surgical planning is critical to achieve optimal outcome in deep brain stimulation (DBS). The relationship between clinical outcomes and DBS electrode position relative to subthalamic nucleus (STN) is well investigated, but the role of surgical trajectory remains unclear. We sought to determine whether preoperatively planned DBS lead trajectory relates to adequate motor outcome in STN-DBS for Parkinson's disease (PD). METHODS: In 49 participants who underwent bilateral STN-DBS for PD using a Leksell® frame, we coregistered the frame and participant MRI images to obt

Outcome measures used in peripheral nerve surgery for symptomatic neuroma in upper extremity amputations: A scoping review.

Novel surgical treatments for painful neuromas are increasingly used, but determining which provides the greatest benefit has been difficult due to the inconsistent use of outcome measures. We mapped the current literature of outcome measures used to evaluate peripheral nerve surgery for the management of symptomatic neuromas in patients who underwent an adult-acquired upper extremity amputation (UEA). Medline, Embase, Cochrane, and CINAHL were searched for primary research written in the English language from inception to February 2023. The search yielded 1137 articles, of which 35 were inclu

Fluorescein-Guided Surgery for Arteriovenous Malformation.

Microscopes with fluorescence modality for videoangiography (VAG) using indocyanine green or sodium fluorescein (FL) have been used in cerebrovascular surgeries as tools for brain blood flow assessment in vascular diseases, especially in brain aneurysms, extracranial-intracranial bypass, and arteriovenous malformations (AVMs). Indocyanine green-VAG is a well-documented tool frequently employed as an adjunct to microsurgery for AVM treatment. Nevertheless, it's worth noting that the use of FL-VAG has been significantly underrepresented in medical literature, with only a few studies addressing i

Intraoperative neuromonitoring in Chiari I malformation surgery: a systematic review and meta-analysis.

Chiari Malformation Type I (CMI) is a prevalent neurosurgical condition characterized by the descent of cerebellar tonsils below the foramen magnum. Surgery, aimed at reducing symptomatology and syrinx size, presents risks, making intraoperative neuromonitoring (IONM) a potentially vital tool. Despite its widespread use in cervical spine surgery, the utility of IONM in CMI surgery remains controversial, with concerns over increased operative time, cost, restricted anesthetic techniques and tongue lacerations. This systematic review and meta-analysis followed the Cochrane Group standards and PR

Anterior interhemispheric vs. pterional approach in the microsurgical management of anterior communicating artery aneurysms: a comparative analysis employing a novel multidimensional matching-tool.

The surgical management of anterior communicating artery aneurysms (AcomA) is challenging due to their deep midline position and proximity to complex skull base anatomy. This study compares the pterional craniotomy with the interhemispheric approach based on the specific aneurysm angulation. A total of 129 AcomA cases were analyzed, with 50 undergoing microsurgical clipping via either the pterional or interhemispheric approach. All selected cases had computed tomography-angiography with sagittal imaging slices and 2D-angiography. Using an interactive tool, 14 cases treated via the interhemisph

Single-Center Experience in Microsurgical Resection of Acoustic Neurinomas and the Benefit of Microscope-Based Augmented Reality.

Background and Objectives: Microsurgical resection with intraoperative neuromonitoring is the gold standard for acoustic neurinomas (ANs) which are classified as T3 or T4 tumors according to the Hannover Classification. Microscope-based augmented reality (AR) can be beneficial in cerebellopontine angle and lateral skull base surgery, since these are small areas packed with anatomical structures and the use of this technology enables automatic 3D building of a model without the need for a surgeon to mentally perform this task of transferring 2D images seen on the microscope into imaginary 3D im

Intracranial Arachnoid Cyst in Children: Clinical Presentation and Risk Factors for Surgical Intervention.

INTRODUCTION: Intracranial arachnoid cysts (IAC) in children are a common incidental finding on imaging. Most IACs are asymptomatic and can be monitored; however, a small percentage may enlarge and require surgical intervention. This study aimed to identify clinical risk factors in patients with IAC who underwent surgery versus those who did not. METHODS: We conducted a retrospective chart review from 2009 to 2021 at a free-standing children's hospital. A total of 230 patients diagnosed with an IAC aged 0-21 years of age were included in the study. Data on demographics, imaging, and neurologi
