The treatment of distal third humeral diaphyseal fractures: Is there still a place for the external fixation?

INTRODUCTION: The treatment for humeral diaphyseal fractures is still controversial. The purpose of this study was to evaluate the clinical and radiographic outcomes of treating humeral distal third diaphyseal fractures by using external fixation technique. MATERIALS AND METHODS: We retrospectively review 65 cases of diaphyseal humeral fractures (31 type A, 23 type B and 11 type C of the AO/OTA classification) treated with external fixation (Orthofix FAD small) between 2008 and 2013. The mean follow-up was 48 months (24-72 months). There were 12 open fractures; however, no cases of concomitan

Recommendations for fracture management in patients with osteopetrosis: case report.

INTRODUCTION: Osteopetrosis is caused by general increase in bone density and obliteration of the medullary canal. Fractures are a frequent complication and their management is considered a challenge due to increased resistance to reaming and screw positioning; reduction maneuvers have to be done more carefully to avoid intraoperative fractures, and there is an increased risk of drill breakage. There is also a higher risk of infection and malunion, which increases the incidence of surgical revisions in this population. CASE REPORT: 55-year-old male with osteopetrosis and a history of two prev

Tibial plateau fractures: four years review at B & B Hospital.

BACKGROUND: Tibial plateau fractures involve the articular surface of the tibia resulting from a combination of axial loading with varus or valgus stress. Inadequate and inappropriate treatment may result in significant functional loss. OBJECTIVE: The purpose of this study was to determine the outcome of our treatment modalities and to compare with the results of comparable studies. METHODS: The results of treatment of 81 knees were reviewed over the period of five years (1997 to 2002). There were 62 men and 18 women, with an average age of 37 years (15 years to 75 years) at the time of init

[Treatment of Galeazzi's fracture - is the surgical revision of the distal radioulnar joint necessary?].

PURPOSE: Fractures of the radial shaft associated with disruption of the distal radioulnar joint (DRUJ) are termed as Galeazzi-fractures. In Galeazzi's fracture, the aim of treatment is restoring the congruency of the joint and the stability of the DRUJ, thus preventing a loss of pronation or supination. PATIENTS AND METHOD: In this study, we included 24 patients (m = 22, f = 2) with 25 Galeazzi fractures treated between 1980 and 1998. Surgical treatment and the duration of therapy were analyzed retrospectively. The clinical and radiological results of 15 patients were followed up. Two childr

Ultrasound-guided pericapsular nerve group block versus intravenous morphine for pain management in older adults with hip fractures: a randomised controlled trial in the emergency department.

BACKGROUND: Hip fracture pain is severe and is commonly managed with intravenous opioids that cause adverse effects in older adults. Although the use of regional anaesthesia is increasing, high-quality evidence remains limited, with insufficient randomised controlled trial (RCT) evaluating the effectiveness of the pericapsular nerve group (PENG) block in the emergency department (ED). This study aimed to compare the analgesic efficacy of ultrasound-guided PENG block with intravenous morphine for acute hip fracture pain in older adults presenting to the ED. METHODS: This open label, 1:1 parall

Anatomical Augmentation Using Suture Tape for Acute Syndesmotic Injury in Maisonneuve Fracture: A Case Report.

Ankle syndesmosis is crucial to the integrity of the ankle joint and weight-bearing; an injury to this structure can lead to significant disability. The treatment methods for distal syndesmosis injuries are controversial. The representative treatment methods include transsyndesmotic screw fixation and suture-button fixation, and good results with suture tape augmentation have recently been reported. However, an augmentation using suture tape is only possible when the posterior inferior tibiofibular ligament (PITFL) is intact. This study describes the case of an unstable syndesmosis injury, acc

Arthroscopically assisted osteosynthesis of intraarticular scapular fractures.

INTRODUCTION: Intraarticular scapular fractures occur mostly in high-energy injuries in contrast to glenoid fractures, which occur mostly in humeral head dislocations. In addition to open repositioning and osteosynthesis, minivascular osteosynthesis under the control of fluoroscopy and arthroscopy has been also used. Arthroscopy allows debridement of the fracture line in the intraarticular area as well as its repositioning during repositioning maneuvers under direct visualization. Furthermore, arthroscopy allows full control over the placement of osteosynthetic material in the subchondral regi

The effect of a new topographic classification on determining the prognosis of nasal fracture and treatment modality.

BACKGROUND: Classifications of nasal fracture are based on clinical findings or radiological findings. The classification systems of nasal fracture usually determine the type of nasal fracture. It is important that a classification gives information about treatment modality and prognosis rather than determining the type of fracture. The objective of this study was to show the effect of the new topographic classification on determining the parameters of prognosis and deciding on treatment modality of the nasal fracture. METHODS: We reviewed patients with nasal fracture that was referred from e

Bilateral triceps tendon approach is flexible and efficient in the treatment of type C distal humerus fractures.

PURPOSE: The purpose of this study was to assess and compare elbow range of motion, triceps extension strength and functional results of type C (AO/OTA) distal humerus fractures treated with bilateral triceps tendon (BTT) approach and olecranon osteotomy (OO). At the same time, we are also trying to know whether BTT approach can provide sufficient vision for comminuted intra-articular fractures of the distal humerus, and whether it is convenient to convert to the treatment to total elbow arthroplasty (TEA) or OO. METHODS: Patients treated with OO and BTT approaches for type C distal humerus f

Surgical dislocation in the management of pediatric and adolescent hip deformity.

The surgical dislocation approach is useful in assessing and treating proximal femoral hip deformities commonly due to pediatric conditions. We sought to demonstrate the efficacy and problems associated with this technique. Diagnoses included slipped capital femoral epiphysis, Perthes disease, developmental dysplasia of the hip, osteonecrosis, and exostoses. Through this approach, femoral head-neck osteoplasty (22), intertrochanteric osteotomy (eight), femoral head-neck osteoplasty plus intertrochanteric osteotomy (15), femoral neck osteotomy (five), open reduction and internal fixation of an

[Percutaneous fixation of anterior column acetabular fractures--first experience].

PURPOSE OF THE STUDY: Percutaneous fluoroscopy-assisted fixation of acetabular fractures is not a widely used technique. Its advantage lies particularly in percutaneous placement of screws in the anterior column of the acetabulum. The operative procedure is described and the first experience of the authors with this minimally invasive technique at their department is reported. MATERIAL: Between December 2007 and April 2009, 10 fractures of the anterior column of the acetabulum were treated by percutaneous stabilisation in 10 patients. There were six men and four women aged 25 to 76 (average,
