Sickle retinopathy in a person with hemoglobin s/new york disease.

A patient who presented with sickle retinopathy and hemoglobin electrophoresis results compatible with sickle cell trait was found, on further investigation, to be a compound heterozygote with hemoglobin S and hemoglobin New York disease. This recently reported form of sickle cell disease was not previously known to cause retinopathy and surprisingly was observed in a non-Asian individual. The ophthalmological findings, the laboratory diagnosis, and possible pathophysiology of this disorder are discussed. Persons diagnosed with sickle cell trait who present with symptoms of sickle cell disease

Strabismus and poor stereoacuity associated with Kabuki syndrome.

Kabuki syndrome is characterized by long palpebral fissures, large ears, a depressed nasal tip, and skeletal anomalies associated with postnatal dwarfism and mental retardation. There have been few prior detailed descriptions of strabismus or stereopsis in these patients. We report a patient with Kabuki syndrome who showed small-angle strabismus and poor stereopsis. This case illustrates the need for patients with a diagnosis of Kabuki syndrome to have an ophthalmologic evaluation. Strabismus associated with Kabuki syndrome may have a small angle that can be easily overlooked.

Bupropion-induced diplopia in an Iranian patient.

We present a case report of bupropion-induced diplopia in a 26-year-old woman who suffered from atypical depression. After four weeks of taking bupropion (225 mg per day) she complained of headaches, blurred vision, and diplopia. No neurological and ophthalmologic abnormal signs were found. Her brain magnetic resonance imaging (MRI) and magnetic resonance venography (MRV) were normal as well. After tapering off and discontinuing bupropion, her diplopia resolved within a week.

[Limited value of ophthalmoscopy in arterial hypertension].

The background for the ophthalmoscopic classification of retinal changes in arterial hypertension, hypertensive retinopathy grade I-IV is summarised. It is concluded that ophthalmoscopy has no significance for the diagnosis and treatment of arterial hypertension in persons older than 40-50 years. In future, computerized image analysis of digital images of the retina might be used as a marker of cardiovascular and other systemic risk factors. However, this epidemiological tool has no predictive value for the individual patient. Hopefully, this paper may contribute to a limited number of superfl

[Neonate with linear rows of vesiculobullous lesions].

We report the case of an 8-days-old patient with linear rows of vesiculobullous lesions from birth. Along with these lesions, the neonate had eosinophilia, which made us arrive to the clinical diagnosis of Incontinentia Pigmenti. Incontinentia Pigmenti or Bloch Sulzberger disease is a multisystemic neuroectodermal disorder that affects mostly women, which includes as initial manifestation cutaneous lesions that evolve by outbreaks. Occasionally, it is associated with neurologic, ophthalmologic, and dental compromise. This must be considered as a differential diagnosis when a neonate presents v

[Transient visual loss: a practical approach].

Transient vision loss results from a variety of disorders including benign dry eye as well as vision- or life-threatening disorders such as giant cell arteritis. The goal of the initial evaluation is to determine the most likely mechanism based on the past medical history, characteristics of visual loss, and ophthalmologic and neurologic examinations. The ophthalmologist is often the first physician consulted and plays an essential role. Indeed, identification of the mechanism of transient visual loss allows the care-taker to properly decide on the work-up, its timing (emergency or otherwise),

Effects of some ophthalmic medications on pupil size: a literature review.

Ophthalmological pharmacology is a rapidly expanding field aimed at achieving the safest and most effective treatment results. Physicians must be aware of the side-effect profiles, both beneficial and harmful, of medications currently used. This review highlights the available data on the effect of some ophthalmic medications on pupil size; it was limited to all reports or studies describing topical ophthalmic agents not originally designed or indicated to alter pupil diameter. This awareness will protect patients from unwanted drug-induced side effects and will improve clinical management and

Serum eye drops, amniotic membrane and limbal epithelial stem cells--tools in the treatment of ocular surface disease.

The advent of serum eye drops, amniotic membrane and limbal stem cell grafts, have transformed the treatment of diseases which result in ocular surface failure. This article provides an overview of ocular surface anatomy and failure, and how the use of serum eye drops, amniotic membrane and limbal epithelial stem cell transplantation has influenced ophthalmological practice both historically and in recent years. This review focuses on the rationale for the use of these emerging tools, how they are prepared, clinical applications, limitations, and speculates on future directions.

Subfoveal choroidal neovascularization in a patient with Fabry's disease.

INTRODUCTION: Fabry disease (FD) is an X-linked, inherited disorder caused by a deficiency of the enzyme alpha-galactosidase A, with progressive accumulation of glycosphingolipids within several tissues and organs, including the eye. Ophthalmological manifestations include conjunctival vessel tortuosity, cornea verticillata, lens opacity, and retinochoroidal vessel abnormalities. REPORT: In FD, the presence of macular choroidal neovascularization (CNV) has never been previously described. DISCUSSION: We report the case of a FD patient who developed an early-onset CNV, when he was still in hi

Combined central retinal artery and vein occlusion complicating orbital cellulitis.

Orbital Cellulitis is a dreaded ophthalmologic disease. It may destroy vision and the eye and may even become life threatening. Often visual loss is the result of exposure and subsequent destruction of ocular tissue commonly the cornea and the uvea. We report a case of combined central retinal artery and vein occlusion complicating orbital cellulitis in a 35 year old patient who was 37 weeks pregnant resulting in loss of vision in the affected eye. There have been few case reports of this type of complication of orbital cellulitis.

Bilateral posterior dislocation of the crystalline lens after a head injury sustained during a seizure.

This article presents a case of bilateral posterior dislocations of the crystalline lens in a patient with epilepsy who presented with reduced vision and anisocoria 2 weeks after having sustained head injuries during a seizure. The possibility of lens dislocation was raised only at this time, and subsequently confirmed on computed tomography. Such patients may require prompt referral to the ophthalmologist to treat complications such as functional aphakia, uveitis and more seriously pupillary block glaucoma. This case highlights the importance of ocular examination of head injuries to rule out

[Amaurosis fugax with elevated acute phase proteins].

Case report of a 66-year-old woman with episodes of amaurosis fugax and hemicranic headache with otherwise normal ophthalmologic and neurological examinations and normal imaging. While ESR was in the normal range for patient's age, acute phase proteins (C-reactive protein and fibrinogen) were elevated. Giant cell arteritis was proved by temporal artery biopsy. Giant cell arteritis should be considered as an important differential diagnosis of amaurosis fugax even in patients with normal ESR. Acute phase protein testing can give relevant diagnostic information.

Genetics of hereditary vitreoretinal degenerations.

The hereditary vitreoretinal disorders have variable vitreoretinal and other ocular and skeletal abnormalities. Some of these conditions represent a spectrum of clinical disease. This, along with the phenotypic variability, often leads to diagnostic difficulties. In this context, genetic testing is of valuable diagnostic value. Molecular genetic studies have helped distinguish conditions that were previously grouped together and proven others to represent spectrum of the same clinical entity. Accurate diagnosis is important in order to offer effective screening and genetic counseling and appro

Botulinum toxin: clinical use.

Since its development for the use of blepharospasm and strabismus more than 2.5 decades ago, botulinum neurotoxin (BoNT) has become a versatile drug in various fields of medicine. It is the standard of care in different disorders such as cervical dystonia, hemifacial spasm, focal spasticity, hyperhidrosis, ophthalmological and otolaryngeal disorders. It has also found widespread use in cosmetic applications. Many other indications are currently under investigation, including gastroenterologic and urologic indications, analgesic management and migraine. This paper is an extensive review of the

The red reflex.

Eliciting the red reflex is a useful clinical test that can be easily performed in the pediatric emergency room. A direct ophthalmoscope is the only tool needed to perform this test. The test is non-invasive, making it a quick screening tool for even the most apprehensive child. A normal red reflex can rule out intraocular pathology; an abnormal reflex can indicate need for urgent ophthalmologic referral. The differential diagnoses for an absent (or black) reflex; an abnormally sized, shaped or positioned reflex; leukocoria; as well as a non-homogenous reflex are listed.

Endemic treponematosis: review and update.

Despite major efforts to eradicate these disorders, yaws, bejel, and pinta (endemic treponematosis) remain serious health issues in many regions of the world. Aside from prominent skin manifestations, these diseases may also lead to significant osseous, neurologic, and ophthalmologic complications. Although progress has been made in differentiating the causative species in a research setting, a simple, specific, and sensitive diagnostic test remains elusive. Parenteral penicillin, in appropriate dosage, is the treatment of choice; alternative antibiotics such as tetracycline and erythromycin m

Congenital retinal dystrophy and corneal opacity in trisomy 8 mosaicism.

Trisomy 8 mosaicism can present with a wide variety of systemic and ophthalmologic manifestations. We report a patient with an extinguished electroretinogram and diffuse retinal pigment epithelial abnormalities suggestive of a rod-cone dystrophy, which has not been previously reported in association with trisomy 8. A finding that has been described in several patients with trisomy 8 is a central corneal opacity, but there are no data regarding the natural history of this finding; the corneal opacity in our patient spontaneously improved. This case expands the clinical spectrum of this rare ent

Three cases of focal dermal hypoplasia (Goltz syndrome).

Focal dermal hypoplasia or Goltz syndrome is a rare genodermatosis, characterized by multiple abnormalities of ectodermal and mesodermal origin. It is found predominantly in females and is characterized by hypoplasia of skin and papillomas. Three cases of focal dermal hypoplasia in infancy with unusual inheritance patterns are reported. Cutaneous features were atrophic reticulated scars involving the trunk and extremities following the lines of Blaschko. Papillomas were present on the genitalia and in a periorificial distribution. Skeletal abnormalities included syndactyly, polydactyly and lob

[Occlusion of the retinal artery].

The occlusion of the retinal artery usually leads to sudden and, in most cases, continual visual impairment of the affected eye. This is an uncommon illness with a variable progression. To date it has not been possible to develop a generally recommended therapy using prospective, randomised studies, as is the case for circulatory impairment in other organs. Due to this uncertainty, the treatment of a retinal arterial occlusion is regarded as an ophthalmological emergency and a challenge for the attending practitioner. In this overview, the actual state of our knowledge on the pathogenesis of o
