Exertional leg pain: teasing out arterial entrapments.

Vascular causes of exertional lower extremity pain are relatively rare, but may be the answer in athletes refractory to treatment for the more common overuse syndromes of the lower extremities. It is important to differentiate these vascular causes from chronic exertional compartment syndrome (CECS), medial tibial stress syndrome (MTSS), and stress fractures in order to develop appropriate treatment plans, avoid complications, and return athletes to play expeditiously. Important vascular etiologies to be considered are popliteal artery entrapment syndrome (PAES), endofibrotic disease, poplitea…

Polyamine deficient diet to relieve pain hypersensitivity.

There is a compelling body of evidence that N-methyl-d-aspartate receptors (NMDA-R) play a critical role in the development and maintenance of pain hypersensitivity. However, long-term treatments with NMDA-R antagonists are limited by unacceptable side effects. Since polyamines modulate the functioning of NMDA-R and mainly originate from normal dietary intake and bacterial metabolism in the gut, we developed a nutritional therapy based on dietary polyamine deficiency. Here, we reported that a polyamine deficient diet (PD diet) for 7 days prevented the enhancement of tyrosine phosphorylation of…

[Vulvar pain syndrome (vulvodynia)--dilemmas in terminology].

BACKGROUND: In recent years there has been increasing awareness of chronic vulvar pain, both in the medical and lay communities. The etiology of vulvar pain is largely unknown. Furthermore, there is no worldwide consensus regarding the terminology and classification of this condition, which makes it difficult to compare the results of different treatments. In 2003, following more than three decades of uncertainty, the terminology and classification of vulvar pain was re-established. AIM: To review the development of nomenclature for vulvar pain and the increasing understanding of its pathophy…

Outpatient pediatric pain management practices for fractures.

OBJECTIVES: Describe the pattern of utilization and effectiveness of outpatient fracture pain medication. METHODS: A cross-sectional survey of caregivers of children with an isolated extremity fracture at a hospital-based pediatric orthopedic clinic during initial follow-up. RESULTS: Surveys were completed by 98 (79.2%) of 125 caregivers. Mean age of children was 9 years (range, 1-18 years). Fracture sites include arm (36%), wrist (24%), hand (6%), leg (14%), ankle (9%), and foot (6%). Pain was reported "worst" at the time of injury in 45.3% patients (95% confidence interval [CI], 35.0%-55.8…

Pain related sexual dysfunction after inguinal herniorrhaphy.

To determine the incidence of pain related sexual dysfunction 1 year after inguinal herniorrhaphy and to assess the impact pain has on sexual function. In contrast to the well-described about 10% risk of chronic wound related pain after inguinal herniorrhaphy, chronic genital pain, dysejaculation, and sexual dysfunction have only been described sporadically. The aim was therefore to describe these symptoms in a questionnaire study. A nationwide detailed questionnaire study in September 2004 of pain related sexual dysfunction in all men aged 18-40 years undergoing inguinal herniorrhaphy between…

Upper-extremity pain disorders in breast cancer.

UNLABELLED: Upper-extremity pain is a common and debilitating problem for patients with breast cancer. Although there is considerable literature describing symptoms, little is available on the specific disorders responsible for pain and debility in these patients. Cervical radiculopathy, brachial plexopathy, neuropathy, rotator cuff tendonitis, adhesive capsulitis, epicondylitis, postmastectomy syndrome, swelling, and bone metastases are among the common disorders responsible for upper-extremity pain in breast cancer patients. OVERALL ARTICLE OBJECTIVE: To discuss common upper-extremity pain …

[Low back pain and degenerative disc disease].

INTRODUCTION: Various clinical conditions can cause low back pain, and in most cases it is of a degenerative origin. Degenerative disc disease is a common condition which affects young to middle-aged men and women equally. Changes in the mechanical properties of the disc lead to degenerative arthritis in the intervertebral joints, osteophytes, and narrowing the intervertebral foramen or the spinal canal. PATHOPHYSIOLOGY: Degenerative cascade, described by Kirkaldy-Willis, is the widely accepted pathophysiologic model describing the degenerative process as it affects the lumbar spine in 3 phas…

Managing pain after shingles: a nursing perspective.

Post-herpetic neuralgia (PHN) is the neuropathic pain syndrome that may develop following an attack of shingles. While for many the symptoms subside, there can be long-term pain problems for up to 40% of those affected with PHN, and for 3% of these, symptoms can be severe (Dworkin and Portenoy, 1996). Knowledge and understanding of the symptoms and various treatments and approaches available is important to enable nurses and patients to work together in partnership to achieve the best outcomes. Realizing that more than one approach may be needed can allow for treatments which are complementary…

Psychosomatic pain: new insights and management strategies.

At least 40 to 60 percent of women and at least 20 percent of men with chronic pain disorders report a history of being abused during childhood and/or adulthood. This incidence of abuse is two to four times higher than in the general population. Patients with more severe or frequent abuse, usually during childhood and worse if sexual in nature. often develop specific syndromes or combinations of syndromes. These syndromes include posttraumatic stress disorder, fibromyalgia, and other conditions characterized by repression, somatization, and increased utilization of medical care. Psychosomatic …

Post-lumbar surgery complex regional pain syndrome.

BACKGROUND: This case report identifies a patient with complex regional pain syndrome Type 2 (causalgia) with sympathetically maintained pain in a distal extremity associated with an anterior sacroiliac fusion with local bone graft. CASE DESCRIPTION: A sacroiliac fusion was undertaken due to chronic back pain that was temporarily relieved with percutaneous injections to the joint space and lower lumber facets. A 65 year-old man underwent a left sacroliac arthrodesis for continued lumbosacral pain. The successful surgery with allograft had no intraoperative complications. The patient remained …

[Cancer pain management--the palliative care approach].

Combining pharmacological pain management to other therapeutic options relieves most of cancer pain. The pharmacological approach lies on the WHO analgesic three steps. The first step (paracetamol, NSAIDs) is used for weak pain, and the second (weak opioids) for moderate pain. The third one, involving strong opioids, is indicated in moderate to severe pain. If morphine is still considered the first choice, other opioids can be useful in case of unacceptable side effects, inadequate antalgy, or renal failure. The use of co-analgesics can further optimize analgesia in certain cases. Their approp…

Persistent shoulder pain: epidemiology, pathophysiology, and diagnosis.

Persistent shoulder pain is a very common condition that often has a multifactorial underlying pathology and is associated with high societal cost and patient burden. In 2000, the direct costs for the treatment of shoulder dysfunction in the United States totaled $7 billion. Persistent shoulder pain can result from bursitis, tendinitis, rotator cuff tear, adhesive capsulitis, impingement syndrome, avascular necrosis, glenohumeral osteoarthritis (OA), and other causes of degenerative joint disease or from traumatic injury, either in combination or as a separate entity. Rotator cuff disorders, a…

Acceptability of severe pain among hospitalized adults.

BACKGROUND: It is unclear why some hospitalized adults experiencing severe pain report that this degree of pain is acceptable to them. METHODS: A 25% random sample of cognitively intact patients admitted to nine medical/surgical units in a New York City hospital were enrolled. Patients were interviewed daily, Monday to Friday from April 1, 2001 through February 14, 2003, to collect information on the presence of pain, pain intensity, analgesic use, and acceptance of pain. Patients were asked to rate their current level of pain using a four-point scale. RESULTS: Three hundred ninety-two of 12…
