Interplay of childhood metabolic dysfunction-associated steatotic liver disease and obesity in the development of youth-onset type 2 diabetes.

BACKGROUND: The global increase in childhood and adolescent obesity has significantly contributed to the rising prevalence of metabolic dysfunction-associated steatotic liver disease (MASLD) - a condition now recognized as a key metabolic complication in youth. MASLD significantly increases the risk of youth-onset type 2 diabetes (T2D), particularly among obese individuals. Its asymptomatic progression presents considerable challenges for timely diagnosis and intervention. AIM: To review epidemiology, pathophysiological mechanisms, and management strategies related to pediatric MASLD, explori

Enhanced functional connectivity between the default mode network and executive control network during flow states may facilitate creativity and emotional regulation, and may improve health outcomes.

INTRODUCTION: Flow is characterized by complete immersion and optimal engagement in a task, striking a balance between challenge and skill. Recent neuroimaging studies suggest that flow involves dynamic interactions among large-scale brain networks, particularly the default mode network (DMN) and the executive control network (ECN). This review aims to synthesize current findings on how flow-related DMN-ECN connectivity supports creativity and emotional regulation (ER). METHODOLOGY: Following PRISMA guidelines, we searched PubMed, PsycINFO, and Google Scholar for peer-reviewed neuroimaging st

A Systematic Review of Turmeric/Curcumin for Prevention and Treatment of Oral Mucositis Due to Radiotherapy and/or Chemotherapy for Head and Neck Cancer.

INTRODUCTION: Oral mucositis (OM) is oral mucosal damage caused by cancer treatment, including radiotherapy and chemotherapy. It is a common and debilitating adverse effect with significant impact on the quality of life. Current guidelines recommend various treatments, but the evidence base remains limited, highlighting the need for novel therapeutic approaches. Phytotherapy, including turmeric (Curcuma longa), offers potential due to its antioxidant, anti-inflammatory, antimicrobial, and wound-healing properties, which have shown promise in addressing inflammatory conditions. This systematic

Safety and preliminary efficacy of allogeneic bone marrow-derived mesenchymal stromal cells (BMSC) for painful lumbar facet arthropathy: study protocol for a randomised, double-blinded, placebo-controlled, crossover phase II clinical trial.

BACKGROUND: Lumbar facet arthropathy (LFA) is a degenerative, arthritic joint disorder and a common specific cause of axial low back pain and functional impairment. Currently, no disease-modifying therapies are available for symptomatic LFA. Mesenchymal stem/stromal cell (MSC)-based therapy represents an emerging regenerative treatment modality with the promise to target the underlying pathophysiology of LFA. Despite encouraging clinical evidence supporting the safety and efficacy of MSC therapy in other osteoarthritis, its therapeutic utility in LFA remains largely unexplored. We recently com

HIV post-exposure prophylaxis (PEP) and PEP-in-pocket (PIP) for women: A rodgers' evolutionary concept analysis.

BackgroundHIV prevalence among cis and trans women is increasing, yet prevention medication uptake remains low. Despite extensive literature regarding pre-exposure prophylaxis (PrEP), less is known about post-exposure interventions for women. Among women, non-occupational post-exposure prophylaxis (PEP) is mainly used in sexual assault centres and rarely otherwise. PEP-in-Pocket (PIP) is a self-initiated PEP option for infrequent high-risk exposures (<4/year) and could offer women more agency over their sexual health. PIP, an evolution of PEP endorsed by international guidelines, may better ad

Multiple-family group intervention programming to improve mental health of adolescents living with HIV/AIDS in Ghana: An implementation science study protocol.

BACKGROUND: Adolescents living with HIV/AIDS in sub-Saharan Africa face a disproportionate burden of mental health challenges and suboptimal antiretroviral therapy (ART) adherence, which can have significant consequences for their overall health and well-being. While multiple family group therapy (MFGT) has shown promise in improving outcomes for ALHIV in other settings, the intervention is yet to be wholly adapted and evaluated in the Ghanaian context. This study aims to adapt, pilot, and evaluate a multiple family group therapy (MFGT) intervention to improve mental health and ART adherence a

Early nintedanib deployment in COVID-19 interstitial lung disease (ENDCOV-I): study protocol of a randomised, double-blind, placebo-controlled trial.

INTRODUCTION: In December 2019, the novel SARS-CoV-2 triggered a global pneumonia outbreak, leading to millions of deaths worldwide. A subset of survivors faces increased morbidity and mortality, particularly due to subacute lung injury evolving to chronic fibrosing interstitial lung disease. While nintedanib, a tyrosine-kinase inhibitor, shows promise in treating progressive fibrotic lung disease, limited randomised trial data exists for post-COVID-19-induced lung injury. We hypothesise that treatment with nintedanib may attenuate advancement to the fibrotic stages, offering a potential avenu

Protocol for the process evaluation of a randomised clinical trial of incremental-start versus conventional haemodialysis: the TwoPlus study.

INTRODUCTION: Process evaluation provides insight into how interventions are delivered across varying contexts and why interventions work in some contexts and not in others. This manuscript outlines the protocol for a process evaluation embedded in a hybrid type 1 effectiveness-implementation randomised clinical trial of incremental-start haemodialysis (HD) versus conventional HD delivered to patients starting chronic dialysis (the TwoPlus Study). The trial will simultaneously assess the effectiveness of incremental-start HD in real-world settings and the implementation strategies needed to su

Prolonged versus single dose in penicillin oral challenge testing: protocols for a pilot and definitive randomised controlled trial (PROSPECTOR studies).

INTRODUCTION: Penicillin allergy labels (PALs) are reported in 1 in 10 hospitalised patients globally and associated with inferior patient, hospital and microbiological outcomes; however, the majority are incorrect and should be removed. Direct oral penicillin challenge has been demonstrated to be a safe and effective method for the removal of PALs. However, the question of whether a single dose is sufficient to ascertain true allergy status remains unanswered, with some studies suggesting that extended challenges of 3 or more days are superior for the exclusion of delayed immune reactions. Protocol for a randomised controlled trial of a weight maintenance intervention to promote fat loss in pregnant individuals with obesity.

INTRODUCTION: In pregnancy, people with obesity or excess adiposity are prone to excess gestational weight gain (GWG) and have the highest risks for multiple maternal morbidities. Epidemiological studies suggest that the lowest incidence of adverse maternal and infant outcomes occurs with GWG lower than current recommendations (<5 kg) and with gestational weight maintenance, resulting in fat mass loss, in those with obesity. Data from randomised clinical trials are needed to evaluate the efficacy of a fat mass loss intervention on pregnancy outcomes. The objective of this proof-of-principle ra

Functional Outcomes, Lung health and Livelihood Outcomes among people With Tuberculosis (FOLLOW-TB): study protocol for a multicentre, prospective cohort study in Canada.

INTRODUCTION: Tuberculosis (TB) is a major global cause of morbidity and mortality. Emerging evidence in high-burden settings suggests significant long-term sequelae among people surviving TB; however, evidence from high-income, low-TB burden settings like Canada is lacking. In a person with TB infection, provision of TB preventive treatment (TPT) can prevent TB disease and its sequelae, but remains underused. We propose the Functional Outcomes, Lung health and Livelihood Outcomes among people With Tuberculosis study, a multicentre, prospective cohort study in Canada to help improve our unders

Single-Domain Antibodies as Antibody-Drug Conjugates: From Promise to Practice-A Systematic Review.

BACKGROUND: Antibody-drug conjugates (ADCs) represent potent cancer therapies that deliver highly toxic drugs to tumor cells precisely, thus allowing for targeted treatment and significantly reducing off-target effects. Despite their effectiveness, ADCs can face limitations due to acquired resistance and potential side effects. OBJECTIVES: This study focuses on advances in various ADC components to improve both the efficacy and safety of these agents, and includes the analysis of several novel ADC formats. This work assesses whether the unique features of VHHs-such as their small size, enhanc

GnRH-a-based fertility-sparing treatment of atypical endometrial hyperplasia (AEH) and early endometrial carcinoma (EC) patients: a multicenter, open-label, randomized designed clinical trial protocol.

BACKGROUND: Around 4% of women receive an endometrial cancer diagnosis before turning 40, mainly those without prior childbirth experience and a strong desire to preserve their ability to conceive. Consequently, for young patients diagnosed with atypical endometrial hyperplasia (AEH) or early endometrial carcinoma (EC), a fertility-preserving approach employing high-dose oral progesterone has been adopted. However, previous research has shown a notable relapse rate. Furthermore, the extended use of substantial oral progesterone doses may hinder ovarian function and raise the risk of weight gai

Concomitant Guillain-Barré Syndrome and COVID-19: A Meta-Analysis of Cases.

Background and Objectives: Recent findings demonstrate that the transmigration of severe acute respiratory syndrome Coronavirus 2 (SARS-CoV-2) to the nervous system implicates severe neurotropic pathologies, including the onset of the rare disease called Guillain-Barré syndrome (GBS) which is characterized by immune-mediated polyneuropathy. This study aimed to identify the predisposing factors and the clinical features of coronavirus disease 2019 (COVID-19)-induced GBS. Materials and Methods: We have performed an analysis of 147 cases. The Transitions Clinic Network: Post Incarceration Addiction Treatment, Healthcare, and Social Support (TCN-PATHS): A hybrid type-1 effectiveness trial of enhanced primary care to improve opioid use disorder treatment outcomes following release from jail.

BACKGROUND: In 2016, at least 20% of people with opioid use disorder (OUD) were involved in the criminal justice system, with the majority of individuals cycling through jails. Opioid overdose is the leading cause of death and a common cause of morbidity after release from incarceration. Medications for OUD (MOUD) are effective at reducing overdoses, but few interventions have successfully engaged and retained individuals after release from incarceration in treatment. OBJECTIVE: To assess whether follow-up care in the Transitions Clinic Network (TCN), which provides OUD treatment and enhanced

ETIOSARC study : environmental aetiology of sarcomas from a French prospective multicentric population-based case-control study-study protocol.

INTRODUCTION: Sarcomas are rare tumours of connective tissue. The exact overall incidence of sarcomas is unknown due to diagnostic difficulties and the various histological subtypes (over 80 subtypes). However, the apparent increasing incidence of sarcomas suggests environmental causes such as pesticides. Except for some specific factors (ie, ionising radiation, vinyl chloride, dioxin and genetic predispositions) the scientific knowledge on the aetiology of sarcomas is sparse and inconsistent. France is a particularly appropriate country to set up a study investigating the causes of sarcoma oc
