When a capillary or renal disturbance is not apparent in a woman of reproductive age who presents with edema, a disorder referred to as idiopathic edema should be considered. This disorder, which is often intermittent, causes fluid to accumulate when the patient is upright. Because of this postural relationship, research into pathogenesis has focused on homeostatic adjustments to changes in posture. A number of mechanisms have been proposed, the most tenable of which seem to be increased capillary permeability, increased mineralocorticoid activity, and a faulty "escape mechanism." The diagnosi…

Spinographic parameters of a group of patients with extension facet subluxations and a group with herniated disc nucleus pulposus of the lumbar spine were evaluated pre- and post-manipulative treatment. A statistically significant decrease in the mean disc angle at the involved level in the extension facet subluxation group was seen with the mean post-treatment disc angle more closely resembling that of the controls. Likewise, a statistically significant increase occurred in the mean disc angle at the involved level in the disc herniation group post-treatment to one more closely resembling tha

The in vivo three-dimensional orientation of the vertebral columns in five adolescent idiopathic scoliotic subjects with double curves were examined in standing, supine, and supine positions with Cotrel traction postures using a biplanar radiographic technique. This analysis was compared with that obtained from a previous study on five healthy adolescent subjects to assess the postural changes and mechanical factors relating to the development of double-curve scoliotic deformity. Statistical analysis of vertebral orientation, rotation, disc distraction, and wedging between the normal and scoli…

Parkinson's disease is a progressive neurodegenerative disorder that commonly results in gait dysfunction, balance impairment, postural instability, and reduced functional independence. Freezing gait (FOG) is among the most disabling motor manifestations of the disease and is associated with an increased risk of falls, impaired mobility, and reduced quality of life. Although exercise-based rehabilitation has demonstrated beneficial effects in individuals with Parkinson's disease, detailed reports describing the response of patients with long-standing disease and significant freezing gait remai

Human postural control system has the capacity to adapt to balance-challenging perturbations. However, the characteristics and mechanisms of postural adaptation to continuous perturbation under the sensory conflicting environments remain unclear. We aimed to investigate the functional role of oscillatory coupling drive to lower-limb muscles with changes in balance control during postural adaptation under multisensory congruent and incongruent environments. We combined a platform moving sinusoidally (0.24 Hz) along the anterior-posterior (AP) axis and a virtual scene moving sinusoidally (0.24 H

BACKGROUND CONTEXT: Postural misalignment and compromised balance are major contributors to fall risk in the elderly, leading to significant physical injuries, reduced quality of life (QOL), and increased healthcare burdens. Evaluating postural stability is critical for fall prevention. The cone of economy (CoE) concept explores the range of motion of the center of mass and head required to maintain a stable upright posture, yet its measurement remains challenging due to the limitations of existing methods.
PURPOSE: This study introduces a novel apparatus for real-time measurement of both ext…

BACKGROUND: Although deep brain stimulation of the subthalamic nucleus (STN-DBS) induces motor benefits in people with Parkinson's disease (PwPD), its effect on motor axial symptoms (e.g., postural instability, trunk posture alterations) and gait impairments (e.g., freezing of gait) is still ambiguous. Physical therapy (PT) effectively complements pharmacological treatment to improve postural stability, gait performance, and other dopamine-resistant symptoms (e.g. freezing of gait) in the general population with PD. Despite the positive potential of combined PT and STN-DBS surgery, scientific …

INTRODUCTION: 3D Non-Contact surface scanning (3DSS) is used in both biomechanical and clinical studies to capture accurate 3D images of the human torso, and to better understand the shape and posture of the spine-both healthy and pathological. This study sought to determine the efficacy and accuracy of using 3DSS of the posterior torso, to determine the curvature of the spinal column in the lateral lying position.
METHODS: A cohort of 50 healthy adults underwent 3DSS and Magnetic Resonance Imaging (MRI) to correlate the contours of the external spine surface with the internal spinal column. …

BACKGROUND: Exercises with motor complexity induce neuroplasticity in individuals with Parkinson's disease (PD), but its effects on freezing of gait are unknown. The objective of this study was to verify if adapted resistance training with instability - exercises with motor complexity will be more effective than traditional motor rehabilitation - exercises without motor complexity in improving freezing-of-gait severity, outcomes linked to freezing of gait, and brain function.
METHODS: Freezers were randomized either to the adapted resistance training with instability group (n = 17) or to the

BACKGROUND: Recently, interactive video games (IVGs) have been used as a health-care intervention that provides both exercise and cognitive stimulation. Several studies have shown that IVGs can improve postural control, gait, cognition, and functional independence in elderly people and patients with neurological disease. However, there is a lack of evidence about the effects of IVGs on frail and pre-frail elderly people. The aim of this study was to evaluate the feasibility, safety, and acceptability of playing Nintendo Wii Fit Plus™ (NWFP) interactive video games, and the functional outcomes

BACKGROUND: Glenohumeral pain and rotator cuff tendinopathy (RCT) are common musculoskeletal complaints with high prevalence among working populations. The primary proposed pathophysiologic mechanisms are sub-acromial RC tendon impingement and reduced tendon blood flow. Some sleep postures may increase subacromial pressure, potentially contributing to these postulated mechanisms. This study uses a large population of workers to investigate whether there is an association between preferred sleeping position and prevalence of: (1) shoulder pain, and (2) rotator cuff tendinopathy.
METHODS: A cro…

INTRODUCTION: Obstructive sleep apnea (OSA) is widely accepted to improve during slow wave sleep (SWS) compared to lighter stages of NREM sleep. However, supporting data to establish the magnitude and prevalence of this effect is lacking. Consequently, we examined this phenomenon, controlling for posture, in a large group of patients investigated for OSA at an academic clinical sleep service.
METHODS: A detailed retrospective analysis was conducted on data obtained from each 30-sec epoch of sleep in 253 consecutive full-night diagnostic polysomnography studies performed over a 3-month period.…

A patient reported bilateral hand tremors when writing but not with other tasks. These "task specific" tremors are considered subcategories of essential tremor. Primary writing tremor, in which the tremor occurs only with writing, is probably the most common. The important teaching point is that the "standard" tremor assessment, watching the patient holding a sustained posture and touching his finger to the examiner's and then back to the nose is not adequate. Patients should be tested doing the activity that causes them the most difficulty.

Predominant orientation of local muscular load to proximal or distal portions of shoulders and upper limbs considerably influences features of changes in brain functions and blood supply. Characteristic for plasterers, localization of myogeloses in upper part of trapezium muscles causes not only impaired vertebrobasillar circulation, but also venous cerebral stasis and caudal medullar dysfunction. Clinical symptoms are vestibular disorders, depressed functions of hearing, chronic venous cerebral congestion, posture-tonic disorders. Local loads on distal portions of limbs have other mechanisms …

It has been known for many years that weightlessness induces changes in numerous physiological systems: the cardiovascular system declines in both aerobic capacity and orthostatic tolerance; there is a reduction in fluid and electrolyte balance, hematocrit, and certain immune parameters; bone and muscle mass and strength are reduced; various neurological responses include space motion sickness and posture and gate alterations. These responses are caused by the hypokinesia of weightlessness, the cephalic fluid shift, the unloading of the vestibular system, stress, and the altered temporal envir…

Post-traumatic syringomyelia is now a well known entity and occurs months or years after a spinal cord injury. The presenting symptoms are usually pain, progressive motor weakness, sensory changes, and increased spasticity. Profuse sweating or hyperhidrosis can be a symptom of the post-traumatic syrinx or can occur in autonomic dysreflexia provoked by peripheral stimuli. We present two patients with cervical spine fractures whose presenting symptom of post-traumatic syringomyelia was hyperhidrosis affected by posture. The pathophysiology involved and the management of these patients is discuss…

Spinal stenosis is a common problem in the elderly population. Thanks to better knowledge it is being diagnosed more frequently and in many cases successfully treated by surgery. Clinically, patients with a lumbogluteal and in later stages bilateral sciatic pain syndrome can be differentiated from patients with monoradicular pain. Typically, the posture deteriorates with walking distance. Spinal stenosis must be differentiated from peripheral vascular disorders and compressive disc herniation. The possibilities of conservative therapy are limited and their effect is lasting only in mild cases.

We present seven cases of tremor caused by mild head injury without loss of consciousness. The interval between head trauma to onset of symptoms was 1 to 4 weeks. A posture and kinetic tremor of the hands and head occurred unassociated with other neurologic signs. Myoclonic-like jerking was frequently present. Neuroimaging studies were normal. Clonazepam administration resulted in tremor reduction in three patients and propranolol decreased tremor in one patient. A tremor, similar to essential tremor, can be a rare complication of head trauma.
