Beyond Opioids: A Review of Suzetrigine for Acute Pain Management.

The ongoing opioid epidemic in the United States highlights the need for novel analgesics with reduced risk of misuse, dependence, and adverse central nervous system effects. Suzetrigine (trade name Journavx), a first-in-class selective Nav1.8 (a sodium channel expressed in peripheral nociceptors) inhibitor, was approved by the FDA in early 2025 and marketed as the first non-opioid analgesic in over two decades. Current studies have shown that suzetrigine has potential in treating acute pain perioperatively for minimally to moderately painful ambulatory procedures. However, suzetrigine appears

Posterior Approach to Sacroiliac Joint Fusion as a Salvage Procedure: A Case Report.

BACKGROUND: Sacroiliac joint (SIJ) pain is a prevalent cause of chronic low back pain (LBP), affecting many adults in the United States. The SIJ provides stability and proper weight distribution from the trunk. Degenerative disruption to this joint can result in shearing and tension that can lead to significant pain and force imbalances. Previous treatment often failed to provide complete and long-term pain relief. CASE REPORT: This article follows a 72-year-old woman with a medical history of chronic right-sided LBP. The patient received a corticosteroid injection, a T10-S1 fusion, conventio

An Unusual Challenging Case of Complex Regional Pain Syndrome Involving all 4 Limbs: A Case Report.

BACKGROUND: Complex regional pain syndrome (CRPS) is characterized by disproportionate pain along with autonomic signs and trophic changes. In rare instances, it can spread to other extremities. CASE REPORT: We report an unusual case of gradual progression of CRPS to all 4 extremities in a patient who developed CRPS following a traumatic event. Despite receiving adequate medical management, her pain pattern spread to her other extremities, causing significant functional impairment, and she eventually had to undergo minimally invasive procedures, namely stellate ganglion blocks (SGB) and spina

Efficacy of Pudendal Nerve Blocks and Ultrasound-Guided Superior Hypogastric Plexus Blocks for the Management of Refractory Interstitial Cystitis: A Case Series.

Interstitial cystitis/bladder pain syndrome (IC/BPS) is characterized by chronic pelvic, perineal, or bladder pain in addition to lower urinary tract symptoms. The etiology of this condition is not fully understood, which presents a challenge for effective therapeutic intervention. Current treatment guidelines recommend the use of multimodal pain management strategies including behavioral/non-pharmacologic, oral medications, bladder instillations, procedures, and major surgery. However, the safety and efficacy of these modalities vary, and there is currently no optimal treatment for the manage

Consensus Guidelines on Interventional Therapies for Knee Pain (STEP Guidelines) from the American Society of Pain and Neuroscience.

Knee pain is second only to the back as the most commonly reported area of pain in the human body. With an overall prevalence of 46.2%, its impact on disability, lost productivity, and cost on healthcare cannot be overlooked. Due to the pervasiveness of knee pain in the general population, there are no shortages of treatment options available for addressing the symptoms. Ranging from physical therapy and pharmacologic agents to interventional pain procedures to surgical options, practitioners have a wide array of options to choose from - unfortunately, there is no consensus on which treatments

Pain and overcorrection in clubfeet treated by Ponseti method.

Long-term outcomes of Ponseti casting have consistently shown improvement over soft-tissue release. The incidence of foot pain and overcorrection in clubfeet treated by Ponseti method has not been reported. We studied the rate of overcorrection and its association with pain in clubfeet treated with Ponseti casting. A retrospective review of clubfoot patients treated with Ponseti method with at least 8 years of follow-up was carried out. Patient charts were reviewed for demographic data, recurrence, type and number of procedures, and patient-reported complaints of foot pain. Pedobarographs were

The use of lumbar epidural injection of platelet lysate for treatment of radicular pain.

BackgroundEpidural steroid injections (ESI) are the most common pain management procedure performed in the US, however evidence of efficacy is limited. In addition, there is early evidence that the high dose of corticosteroids used can have systemic side effects. We describe the results of a case series evaluating the use of platelet lysate (PL) epidural injections for the treatment of lumbar radicular pain as an alternative to corticosteroids.MethodsRegistry data was obtained for patients (N = 470) treated with PL epidural injections presenting with symptoms of lumbar radicular pain and MRI f

Primary ulnar superficial slip resection in complicated trigger finger.

Long standing trigger finger, often present for 6 months to years before consulting, can result in a lack of extension, which is often painful and may persist after section of the A1 pulley. The ulnar superficial slip resection (USSR) was introduced by Le Viet to address this problem. It is hypothesised that primary section of the A1 pulley with USSR resolves both extension lack and pain in longstanding trigger fingers with a painful extension lack. This retrospective study reviewed the results of primary USSR for longstanding trigger fingers (6 months or more) with painful extension lack in 1

Pain after suburethral sling insertion for urinary stress incontinence.

INTRODUCTION AND HYPOTHESIS: Pain after suburethral sling insertion is an uncommon, but troublesome complication of continence surgery; in some cases it can cause significant distress and disability. It is often not recognised, poorly diagnosed and treated by gynaecologists and urologists unfamiliar with the condition. METHODS: A literature review was performed and the current evidence on pain after suburethral sling insertion reviewed. RESULTS: The tension-free vaginal tape-obturator (TVT-O) procedure is associated with an increased incidence of early groin pain, but this often resolves wit

Do topical anesthetics reduce periareolar injectional pain before sentinel lymph node biopsy?

BACKGROUND: Topical anesthetics have been used in various procedures. The purpose of this study was to evaluate efficacy of lidocaine/prilocaine cream in decreasing the pain of injection for sentinel lymph node biopsy. METHODS: A prospective, randomized, placebo-controlled study was conducted on female breast cancer patients undergoing periareolar injection for sentinel lymph node isolation. Subjects applied lidocaine/prilocaine cream or a placebo cream before injection and completed a survey postoperatively. RESULTS: Twenty treatment and 19 control patients were studied. There was a trend f

Heart rate variability in response to pain stimulus in VLBW infants followed longitudinally during NICU stay.

The objective of this longitudinal study, conducted in a neonatal intensive care unit, was to characterize the response to pain of high-risk very low birth weight infants (<1,500 g) from 23 to 38 weeks post-menstrual age (PMA) by measuring heart rate variability (HRV). Heart period data were recorded before, during, and after a heel lanced or wrist venipunctured blood draw for routine clinical evaluation. Pain response to the blood draw procedure and age-related changes of HRV in low-frequency and high-frequency bands were modeled with linear mixed-effects models. HRV in both bands decreased d

Pain management and regional anesthesia for the dental patient.

Current standards of care in veterinary medicine dictate an adequate level of pain control for our patients. Effective pain control uses a proactive, multimode approach that starts with preoperative medications, includes the anesthetic protocol selected, and continues into the postoperative period. A basic understanding of the physiology of pain assists in selecting those agents and modalities best suited for individual patients. Analgesic drug selection and local anesthesia are both integral parts of pain control when performing surgery in the oral cavity. Local (regional) anesthesia plays an

Management of late posttraumatic kyphosis with anterior Z-plate instrumentation.

Failed treatment of thoracolumbar spine fractures may lead to late posttraumatic kyphosis (LPK), and LPK treatment is challenging. The aim of this retrospective study was to investigate whether anterior reduction and Z-plate instrumentation constitute feasible treatment for LPK (> 30 days after injury). Twenty patients who developed LPK after a thoracolumbar fracture were treated with the Z-plate anterior thoracolumbar plating system. Sixteen patients were followed for a mean of 35 months. Ten of 15 patients with a thoracotomy had persistent postthoracotomy pain. Mean back pain decreased signi

Neuropathic pain after C7 spinal nerve transection in man.

Various animal models of neuropathic pain have been developed which involve creating a lesion in a spinal root. We describe a human correlate in which patients developed a neuropathic pain syndrome after having one spinal nerve surgically divided. In some patients with brachial plexus lesions, the C7 spinal nerve from the opposite side is divided and used as a nerve transfer to re-innervate the injured brachial plexus. Of five patients that underwent this procedure, one went on to develop a transient but significant neuropathic pain problem. Extensive sensory testing in this patient 2 months a

The efficacy of continuous cryotherapy on the postoperative shoulder: a prospective, randomized investigation.

This prospective, randomized investigation evaluated the efficacy of cryotherapy on subjective responses after both open and arthroscopic procedures on the shoulder. Seventy patients were randomly assigned to one of two study groups: (1) continuous cryotherapy group and (2) age-matched control group. Visual analog scales were used to assess subjective responses on postoperative days 1, 7, 14, and 21. On day 1, patients receiving cryotherapy reported significantly less pain during sleep and significantly more comfort in bed and rated their sleep as more restful than the control subjects. During

Heel pain--operative results.

In 6 years through 1982, the authors performed 34 operative cases in 26 patients with recalcitrant heel pain. The operative procedure involves an isolated neurolysis of the mixed nerve supplying the abductor digiti quinti muscle as it passes beneath the abductor hallucis muscle and beneath the medial ridge of the calcaneus. The deep fascia of the abductor hallucis muscle is released routinely, and an impinging heel spur or tight plantar fascia is partially removed or released if it is causing entrapment of the nerve. The biomechanical pathogenesis of heel pain in relation to pes planus and pes

The Evolution and Advancement of Endoscopic Foraminal Surgery: One Surgeon's Experience Incorporating Adjunctive Techologies.

BACKGROUND: Endoscopic spine surgery has evolved gradually through improvements in endoscope design, instrumentation, and surgical techniques. The ability to visualize and treat painful pathology endoscopically through the foramen has opened the door for the diagnosis and treatment of degenerative conditions of the lumbar spine (from T10 to S1). Other endoscopic techniques for treating a painful disc have been focused on a posterior approach and has been compared with micro-lumbar discectomy. These procedures have not been more effective than open microdiscectomy but are less invasive, have le
