Patient with Progressive Supranuclear Palsy Initially Diagnosed as Anxiety and Depression.

INTRODUCTION: Progressive Supranuclear Palsy (PSP) encompasses a broad spectrum of syndromes related to movement and behavioral disorders. The most common symptoms include motor disturbances such as frequent falls, rigidity, bradykinesia, and ocular movements. Behavioral changes may precede these symptoms, complicating the diagnosis and delaying treatment. This case aims to highlight symptoms that may raise suspicion for PSP diagnosis and facilitate timely treatment. CLINICAL CASE: The patient is a 58-year-old woman with neuropsychiatric symptoms such as disinhibition and depressive symptoms,

Acute and Long Term Treatment of Manic Episodes in Bipolar Disorder.

Bipolar disorder is a disabling psychiatric disorder which causes premature death and loss of quality of life. Despite the developments, novel treatments are partially effective and insufficient responses to treatment may cause loss of quality of life. Contemporary approaches to treatment planning involve taking the current symptoms and the personal treatment history of the patient into account and tailoring them for the treatment of each patient, i.e. individualized treatment. In this article, effects and side effects of antipsychotics, mood stabilizers and sedative hypnotic medications are r

Aripiprazole-induced Hepatitis: A Case Report.

Aripiprazole is an atypical antipsychotic that acts as a partial agonist of dopamine type 2 receptors as well as 5-HT1A receptors. It is used in the treatment of schizophrenia and in type 1 bipolar disorder for mania. Because aripiprazole is well tolerated with few side effects it is used off-label in other psychotic disorders. The prevalence of abnormal liver function tests with antipsychotic use is 32%, with clinically significant effects in 4% of cases. No cases of aripiprazole-induced liver injury have been published. We report a 28-year-old female who presented with non-affective first-ep…

Prevalence of violent victimisation in severe mental illness.

BACKGROUND: Since de-institutionalisation, much has been written about the risk posed to the community by those with severe mental illness. However, violent victimisation of people with mental illnesses has received little attention. AIMS: To establish the 1-year prevalence of violent victimisation in community-dwelling patients with psychosis and to identify the socio-demographic and clinical correlates of violent victimisation. METHOD: A total of 691 subjects with established psychotic disorders were interviewed. The past-year prevalence of violent victimisation was estimated and compared …

Psychotropic drugs in acute intermittent porphyria.

Acute intermittent porphyria is one of a group of metabolic diseases called the porphyrias that may lead to symptoms of the central nervous system during an acute exacerbation. Certain drugs such as barbiturates are known to precipitate attacks of acute intermittent porphyria, but unfortunately there is little information regarding the safety of many psychotropic drugs in this disorder, especially the newer antidepressants and atypical antipsychotics. We report a case of an elderly patient with acute intermittent porphyria who was treated with a variety of psychotropic agents for a severe depr

Depression in the elderly.

Depression is a common psychiatric problem in old age. The magnitude of the problem will become greater as this segment of the population grows. Depression is often overlooked in the elderly, contributing to a greater risk of suicide in this group than in younger patients. Depression may be masked by hypochondriasis or somatization. Special problems in diagnosis include bipolar disorder, pseudodementia, pathological grief, and organic mood disorder as related to medications or physical illness. Pharmacokinetic changes with aging need to be kept in mind by physicians treating the elderly with a

Psychiatric hospitalization of homeless patients over 55 years old. Retrospective study in the largest French psychiatric hospital.

To date, there have been only a few scientific studies about characteristics of homeless patients hospitalized in psychiatric wards, and even fewer about the elderly. This is despite the fact that France, like all Western countries, is facing a general aging of its population and an increasing prevalence of homelessness. The objective of this single-centre retrospective study is to compare the sociodemographic data, psychiatric and somatic diagnoses, and hospitalization characteristics of patients over 55 years of age consecutively admitted to the Groupe Hospitalo-Universitaire Paris Psychiatr

Efficacy, safety and predictors of response to electroconvulsive therapy in Lewy body disease.

BACKGROUND AND OBJECTIVES: Severe neuropsychiatric symptoms, specifically affective, psychotic, and catatonic symptoms, are common in Lewy body disease (LBD). As pharmacotherapy is often limited by poor efficacy and intolerance, alternative treatments are needed. While electroconvulsive therapy (ECT) has shown promise, systematic data on its use in LBD remain scarce. This study aimed to evaluate the efficacy and safety of ECT in LBD and identify predictors of response. METHODS: We compared 40 patients with LBD with 33 with schizophrenia and 24 with affective disorders who received ECT between

Identifying reversible psychiatric dementia mimics in new memory clinic outpatients.

BACKGROUND: Timely identification of reversible conditions that mimic dementia is critical in memory clinic practice. However, psychiatric conditions as potential dementia mimics have not been studied as thoroughly as neurological ones, and detailed data on their reversibility remain limited. OBJECTIVE: To identify reversible psychiatric dementia mimics. METHODS: A retrospective chart review was conducted on 749 new outpatients to investigate etiologies, progression rates, a neuropsychological assessment, cognitive and functional levels, and potential reversibility, categorized by psychiatri

How variants in inflammatory mediator genes influence symptom severity of psychiatric disorders: Findings from the PsyCourse study.

Alterations in glial cell function and cytokine levels in the central nervous system may be influenced by neuroinflammatory processes, which have a pathogenic role in psychiatric disorders. Variability in genes that encode inflammatory mediators is associated with risk of developing mental disorders. Therefore, by analyzing data from the transdiagnostic PsyCourse Study, we aimed to investigate whether variations in inflammatory mediator genes are associated with current symptom severity. We used cross-sectional data from 1320 individuals with a psychiatric disorder and 466 neurotypical individ

Long-term risk of psychiatric disorder and psychotropic prescription after SARS-CoV-2 infection among UK general population.

Despite evidence indicating increased risk of psychiatric issues among COVID-19 survivors, questions persist about long-term mental health outcomes and the protective effect of vaccination. Using UK Biobank data, three cohorts were constructed: SARS-CoV-2 infection (n = 26,101), contemporary control with no evidence of infection (n = 380,337) and historical control predating the pandemic (n = 390,621). Compared with contemporary controls, infected participants had higher subsequent risks of incident mental health at 1 year (hazard ratio (HR): 1.54, 95% CI 1.42-1.67; P = 1.70 × 10-24; differenc
