Challenges toward achieving universal health coverage in Ghana, Kenya, Nigeria, and Tanzania.

Although many sub-Saharan African countries have made efforts to provide universal health coverage (UHC) for their citizens, several of these initiatives have achieved little success. This study aims to review the challenges facing UHC in Ghana, Kenya, Nigeria, and Tanzania, and to suggest program or policy changes that might bolster UHC. Routine data reported by the World Bank and World Health Organization, as well as annual reports of the national health insurance schemes of Ghana, Kenya, Nigeria, and Tanzania, were analyzed. The data were supplemented by a review of published and gray liter

Australian women and heart disease: trends, epidemiological perspectives and the need for a culturally competent research agenda.

Heart disease commonly manifests as acute coronary syndromes (unstable angina pectoris, or myocardial infarction) and heart failure (HF). These conditions are major causes of morbidity and mortality in Australia and internationally. Australian faces particular challenges in health care delivery given the cultural and ethnic diversity of society and unique issues related to rurality. These factors have significant implications for health care delivery. Following an acute cardiac event women have poorer outcomes: higher mortality rates, higher incidence of complications and greater psychological

The value of community participation in disease surveillance: a case study from Niger.

A team of researchers, including one behavioral scientist (S.M.N.) and three epidemiologists (L.Q., O.S. and S.N.) conducted community analyses to assess the social and cultural factors that affect the detection and reporting of disease cases in a surveillance system, using acute flaccid paralysis (AFP) surveillance in Niger as a case study. Over a 60-day period in the country, the research team reviewed written field reports and interviewed epidemiologists, nurses, community members and persons in governmental and non-governmental organizations. Overall, we found that the logistical difficult

Allostatic Load and Racial and Rural Disparities in Breast Cancer Survival.

IMPORTANCE: Racial and geographic disparities in breast cancer survival persist. Biological consequences of chronic stress, measured by allostatic load (AL), may contribute to these disparities, but their role in breast cancer prognosis is not well understood. OBJECTIVE: To evaluate the association between AL and overall survival among patients with breast cancer and assess the contribution of AL to racial and rural disparities in survival. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from an institutional electronic health record and cancer registry at the University of Vi

Feasibility and Acceptability of the Cardiac Rehabilitation Especially for Women (CREW) Web-Based Program in Rural and Remote Communities: A Mixed-Method Study.

BACKGROUND & AIMS: Women remain under-represented in cardiac rehabilitation, particularly those in rural or remote communities. Geographic distance and the lack of women-focused programs are major barriers. Home-based cardiac rehabilitation using telehealth and gender-tailored approaches may help address these challenges. This study aimed to evaluate the feasibility and acceptability of a co-designed, web-based CREW (Cardiac Rehabilitation Especially for Women) program tailored for women in rural and remote communities. METHODS: This was a mixed-methods study, comprising an observational pros

Intangible cultural heritage as school-based psychosocial support for left-behind adolescents in rural China: a framework and preliminary pilot study.

Rural left-behind adolescents in China experience elevated risks of loneliness, negative affect, weakened belonging, and limited access to acceptable psychosocial support. Conventional school counselling models often depend on self-referral, verbal disclosure, and identification with a help-seeking role, conditions that may not fit the social and cultural context of many rural students. This manuscript develops and preliminarily evaluates a culturally grounded schoolbased psychosocial support programme using locally rooted intangible cultural heritage (ICH) activities, including paper cutting,

Triatoma venosa and Panstrongylus geniculatus challenge the certification of interruption of vectorial Trypanosoma cruzi transmission by Rhodnius prolixus in eastern Colombia.

Reactivation of Trypanosoma cruzi transmission by native vectors with different domiciliation capabilities is a major concern for Chagas disease control programs. T. cruzi transmission via intra-domestic Rhodnius prolixus was certified as interrupted by the Pan American Health Organization in Miraflores municipality (Boyacá, Colombia) in 2019. However, Triatoma venosa, a native vector infected with T. cruzi has been increasingly found inside human dwellings across rural areas. In this study, the aim was to describe the eco-epidemiological aspects of T. cruzi transmission in the rural area of M

Social isolation and loneliness prevention among rural older adults aging-in-place: a needs assessment.

INTRODUCTION: The adverse effects of social isolation and loneliness (SI/L) have been documented among older adults in rural communities and contribute to poor health outcomes, premature disability and mortality, and increased burden on the healthcare system. The identification of factors contributing to SI/L among older adults can build the foundation for rural policymakers and leaders to allocate resources and develop tailored strategies more efficiently. The purpose of this article is to describe findings from a needs assessment designed to understand local factors that contribute to SI/L a

Lessons learned from a rural, community-based cervical cancer screen-and-treat pilot study in Malawi.

OBJECTIVES: Invasive cervical cancer (ICC) is the leading cause of cancer-related death among women in Malawi. Barriers to screening for ICC in Malawi, such as long distances to health facilities and lack of public education about ICC, have hindered participation of women in ICC prevention programs. Given the burden of disease and barriers to screening, we implemented a community-based ICC screen-and-treat pilot study and present its successes and challenges. STUDY DESIGN: This study was a screen-and-treat pilot study using Visual Inspection with Acetic acid (VIA) for screening and same-day t

A prospective analysis of pinch grafting of chronic leg ulcers in a series of elderly patients in rural Cameroon.

BACKGROUND: Chronic leg ulcers (CLUs) pose serious public health concerns worldwide. They mainly affect the elderly population. Pinch grafting (PG) could be used to treat a variety of CLUs. However, in Cameroon, there is scarce data on the outcome of PG of CLUs in elderly patients in rural hospitals where most of these patients seek for medical attention and where clinicians rely on unconventional wound dressing methods to treat CLUs. Our objective was to describe the outcome of PG of CLUs in elderly patients in rural Cameroon. METHODS: This was a prospective study conducted in a rural hospit
