The Role of Arthroscopic Soft Tissue Reconstruction for Failed Bristow-Latarjet Procedure.

PURPOSE: To analyze the functional results after unipolar or bipolar arthroscopic soft tissue stabilization in the treatment of recurrent anterior instability after a coracoid bone block procedure. METHODS: We studied a retrospective series of 41 patients (33 male, 8 female) with recurrent anterior shoulder instability after Bristow (n = 7) or Latarjet (n = 34) coracoid bone block treated with unipolar (isolated Bankart, n = 22) or bipolar (Bankart + Hill-Sachs remplissage, n = 19) arthroscopic stabilization. RESULTS: The mean follow-up was 72 (25-208) months. Severe glenoid erosion (>25%) w

Arthroscopic Latarjet procedure with double-button fixation: short-term complications and learning curve analysis.

BACKGROUND: The arthroscopic Latarjet with double-button fixation is a guided procedure recently proposed to treat anterior shoulder instability with glenoid bone loss. The goal of this study was to report intraoperative and early postoperative complications and to analyze the learning curve. METHODS: This was a prospective, nonrandomized study that included 88 patients. Intraoperative or postoperative complications as well as adverse events and operative time were recorded. Clinical outcomes were evaluated at 2 weeks, 1.5 months, and at the last follow-up. Radiologic analysis was based on an

Bipolar Bone Loss in Patients With Anterior Shoulder Dislocation: A Comparison of Adolescents Versus Adult Patients.

PURPOSE: To compare bipolar bone loss by evaluating the degree of glenoid bone loss, Hill-Sachs lesion size, and glenoid track in adolescents and adults with shoulder dislocations. METHODS: We performed a retrospective review between 2012 and 2016 of surgical and nonsurgical patients with a history of anterior shoulder dislocations (primary or recurrent) who underwent magnetic resonance imaging of the affected shoulder. The exclusion criteria included multidirectional instability, prior surgery, and posterior dislocation. Patients were grouped into 2 groups: adolescents (aged 10-19 years) and

Open Bankart Repair for the Treatment of Anterior Shoulder Instability without Substantial Osseous Glenoid Defects: Results After a Minimum Follow-up of Twenty Years.

BACKGROUND: Neglected osseous glenoid defects are thought to be one of the reasons for the reported high rates of recurrent instability at long-term follow-up after Bankart repair. We hypothesized that open Bankart repair for the treatment of anterior glenohumeral instability in the absence of a substantial osseous glenoid defect would result in a lower long-term recurrence rate than has been reported in previous long-term studies. METHODS: Forty-seven patients were treated with a primary modified open Bankart repair for recurrent anterior shoulder instability between 1989 and 1994. Double-co

Effect of Hypertonic Saline in Intra-Articular Hydraulic Distension for Adhesive Capsulitis.

BACKGROUND: Maintaining the integrity of the capsule along with infusing a sufficient amount of fluid is 1 of the therapeutic concepts in intra-articular hydraulic distension (IHD) for adhesive capsulitis. It has been known that hypertonic saline solution decreases tissue edema and increases the fluid volume within the epidural space, causing microdissection, in epidural adhesiolysis. OBJECTIVE: To investigate the effect of hypertonic saline solution in capsule-preserving intra-articular hydraulic distension (CPIHD) for adhesive capsulitis. DESIGN: Prospective randomized controlled trial. S

Postoperative stiff shoulder after open rotator cuff repair: a 3- to 20-year follow-up study.

BACKGROUND AND AIMS: Stiffness after a rotator cuff tear is common. So is stiffness after an arthroscopic rotator cuff repair. In the literature, however, postoperative restriction of passive range of motion after open rotator cuff repair in shoulders with free passive range of motion at surgery has seldom been recognized. We hypothesize that this postoperative stiffness is more frequent than recognized and slows the primary postoperative healing after a rotator cuff reconstruction. We wondered how common is postoperative restriction of both active and passive range of motion after open rotato

Re-evaluation of pectoralis major height as an anatomic reference for humeral height in fracture hemiarthroplasty.

BACKGROUND: Shoulder replacement may be indicated for complex proximal humeral fractures. The primary reason for disappointing results is the nonanatomic position of the prosthesis because the normal anatomic landmarks are disrupted when fractured. An anatomic reference outside of the zone of injury may facilitate proper positioning of fracture arthroplasty reconstructions. It is unknown whether the measurement from the top of the pectoralis major tendon (PMT) to the top of the humeral head is related to patient height. MATERIALS AND METHODS: PMT measurements were performed on 21 pairs of cad

Role of arthroscopy in treating anterior instability of the athlete's shoulder.

INTRODUCTION: Shoulder arthroscopy continues to use minimally invasive techniques to diagnose and treat the athlete's shoulder. Instability can result from trauma or repetitive forceful movements. Arthroscopy stabilization includes multiple quadrant surgery, proper capsular tensioning, and allows dynamic evaluation of the anatomic repair without disruption of the subscapularis. METHODS/MATERIALS: Bone deficits may limit soft tissue stabilization procedures. The arthroscope is helpful in determining the amount of glenoid bone loss, Hill-Sachs impression, positions of engagement, and associated

Panacryl synovitis: fact or fiction?

PURPOSE: It has been suggested from anecdotal experience that Panacryl suture (Ethicon, Johnson & Johnson, Westwood, MA) may be responsible for an exaggerated inflammatory response. Our goal was to test the null hypothesis that Panacryl causes aseptic synovitis when used on anchors in the case of arthroscopic Bankart and rotator cuff repair. TYPE OF STUDY: Retrospective review of the postoperative outcome, specifically looking for clinical criteria that are associated with aseptic synovitis. METHODS: Eighty-three arthroscopic Bankart and 33 arthroscopic rotator cuff repairs were performed us

Three-dimensional characterization of glenoid defects in failed shoulder arthroplasties.

BACKGROUND: Glenoid vault defects are often observed in revision shoulder arthroplasty, and improper management of glenoid bone loss during revision to reverse total shoulder arthroplasty (rTSA) increases the risk of complications. However, there is limited understanding of the types of glenoid vault defects that occur in patients. Therefore, the purpose of this study was to three-dimensionally characterize the underlying glenoid morphology of patients with failed shoulder arthroplasties subsequently undergoing revision arthroplasty to rTSA. METHODS: Computed tomography scans of 100 patients,

Revision reverse total shoulder arthroplasty: clinical and radiographic outcomes compared to primary reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has surpassed anatomic total shoulder arthroplasty in popularity due to its superior outcomes in patients with rotator cuff deficiency and other complex shoulder pathologies. While primary rTSA demonstrates excellent functional improvements, revision rTSA remains challenging, with limited data available on outcomes. Previous studies on revision total shoulder arthroplasty have primarily focused on anatomic total shoulder arthroplasty, with fewer studies evaluating revision rTSA outcomes. The purpose of this study is to compare the clinical

Radiological and electrodiagnostic insights into suprascapular nerve dysfunction: a key predictor of poor functional outcomes in shoulder hemiarthroplasty.

BACKGROUND: Suprascapular nerve (SSN) dysfunction has emerged as an underexplored factor influencing functional outcomes after shoulder hemiarthroplasty (SHA) for proximal humerus fractures. Despite achieving optimal tuberosity healing and prosthesis alignment, some patients continue to experience poor functional recovery. This study investigates the role of SSN dysfunction as a key determinant of unfavorable outcomes following SHA. METHODS: This retrospective study analyzed 38 patients who underwent SHA for Neer type III or IV proximal humerus fractures. Inclusion criteria included radiograp

Subcoracoid impingement exacerbates pain and scapular dysfunction in full-thickness rotator cuff tears.

BACKGROUND: Subcoracoid impingement co-occurs with subacromial impingement in 19% of the patients with rotator cuff tears. The influence of subcoracoid impingement in patients with full-thickness rotator cuff tear (FT-RCT) remains unknown. Therefore, it is worthwhile to investigate potential differences between FT-RCT patients with and without subcoracoid impingement. METHODS: Twenty-two participants were categorized as having an FT-RCT combined with subcoracoid impingement, while 25 others were diagnosed with an FT-RCT without subcoracoid impingement. Characteristics and tear sizes were reco

Shoulder kinematic and muscle recruitment pattern changes in upper trapezius myofascial trigger points: a controlled observational study.

BACKGROUND: Myofascial trigger points (MTrPs) in the upper trapezius are prevalent in individuals with shoulder pain and may interfere with neuromuscular control and kinematics during shoulder movements. The purpose of this study was to compare shoulder muscle activation patterns and kinematic parameters, particularly scapulohumeral rhythm (SHR), between individuals with upper trapezius MTrPs and the healthy group during shoulder abduction. METHODS: An observational study was conducted with participants assigned to either a trigger point group (n = 15) or a healthy group (n = 13). Surface ele
