Use of biomedical obstetric care in rural Tanzania: the role of social and material inequalities.

Despite over 20 years of efforts to improve maternal health, complications of pregnancy and childbirth continue to threaten women's lives in many countries of sub-Saharan Africa. To reduce maternal mortality levels and achieve Millennium Development Goal Five, institutions working for safe motherhood are committed to making biomedical obstetric care more available to women during childbirth. However, implementation of this strategy is not reaching women at the lower end of the socioeconomic spectrum for reasons that are not well understood. Using data from fieldwork conducted between September

Mortality among registered tuberculosis patients in Pune Municipal Corporation area, India.

BACKGROUND: Deaths among tuberculosis patients due to varied causes are frequently reported. Usually, the death rate is high during the first six to eight months, i.e., the intensive phase of treatment. The healthcare personnel working under National Tuberculosis Elimination Program are reluctant to ascribe death to tuberculosis and may prefer to attribute the death to other causes. On the other hand, in hospitals, most death are attributed to tuberculosis without careful death audits and reasonable investigations. Comorbidities like diabetes, HIV, drug resistance, and drug-induced toxicity co

Domains of Frailty Predict Loss of Independence in Older Adults After Noncardiac Surgery.

IMPORTANCE: Preoperative frailty has been consistently associated with death, severe complications, and loss of independence (LOI) after surgery. LOI is an important patient-centered outcome, but it is unclear which domains of frailty are most strongly associated with LOI. Such information would be important to target individual geriatric domains for optimization. OBJECTIVE: To assess whether impairment in individual domains of the Edmonton Frail Scale (EFS) can predict LOI in older adults after noncardiac surgery. DESIGN: Retrospective Cohort Study. SETTING: One Academic Hospital. PARTICI

Geriatric Assessment-Guided Care for Older Adults with Cancer: A Clinical Practice Review.

Cancer is a growing global health burden among older adults, with incidence rising most rapidly in low- and middle-income countries. Chronological age alone poorly reflects physiological reserve, comorbidity, frailty, and functional status, which together determine how well an older patient tolerates cancer therapy. This clinical practice review summarizes contemporary geriatric oncology approaches based on international consensus guidelines and current literature. Comprehensive geriatric assessment (CGA), evaluating functional status, comorbidity, falls, cognition, mood, nutrition, polypharma

Educational Inequalities and Obesity: Association and Attenuation After Lifestyle Adjustment in a Cross-Sectional Working-Age Population.

BACKGROUND: Obesity is a major public health concern and shows a clear social gradient, with higher prevalence among individuals with lower socioeconomic position. Educational level is a key indicator of socioeconomic status, but the extent to which lifestyle factors explain its association with obesity remains unclear. OBJECTIVE: To examine the association between educational level and obesity in a working-age population and to evaluate how adjustment for lifestyle factors influences the magnitude of the association between educational level and obesity. METHODS: A cross-sectional study was

Stroke Investigation Group in North and Central London (SIGNAL): cohort profile of a prospective large-scale comprehensive stroke registry.

PURPOSE: Large-scale stroke registries can provide critical insights into disease mechanisms, progression and healthcare needs, informing prevention and care. However, few collect detailed demographic, brain imaging, and comprehensive long-term follow-up data. To address this, we established the prospective Stroke Investigation Group in North And central London (SIGNAL) registry in 2017. PARTICIPANTS: The SIGNAL registry included 3931 adults aged ≥18 years with confirmed acute stroke (cerebral ischaemia or intracerebral haemorrhage (ICH)) admitted to the University College London Hospital hyp

Mental Health and Alcohol Consumption Among University Students in the Post-Pandemic Context: An Exploratory Cross-Sectional Study in Portugal.

Introduction: The COVID-19 pandemic had significant effects on mental health and lifestyle behaviours, especially among university students who experienced academic disruptions, social isolation, and fewer social interactions. Alcohol consumption has long been part of student culture. Still, the influence of post-pandemic academic reintegration on drinking patterns and psychological distress remains relatively unexplored, particularly in countries like Portugal, where student traditions heavily shape consumption habits. This study aimed to describe the prevalence of alcohol consumption, depres

Refining and validation of Family Socioeconomic Status Scale (FSESS) for health research in Egypt.

BACKGROUND: Socioeconomic status (SES) is a key construct in public health and social research, yet its measurement varies across settings and over time. In Egypt, several existing SES scales are outdated and rely heavily on income, which has become increasingly unstable under current economic conditions. This study aimed to refine and psychometrically validate a concise Family Socioeconomic Status Scale (FSESS) that reflects contemporary socioeconomic conditions in Egypt. METHODS: A community-based cross-sectional study was conducted using a multistage stratified cluster sampling design. Four

Do dietary perceptions among patients with chronic musculoskeletal conditions correlate with lifestyle metrics and outcomes of the lifestyle medicine program?

BACKGROUND: Eating a whole-food, plant-predominant diet reduces chronic systemic inflammation, which can improve pain in people with chronic musculoskeletal conditions. However, 85% of people have dietary perceptions that do not accurately reflect their actual dietary quality. OBJECTIVE: To determine whether patients' dietary perceptions align with dietary quality, assess correlations between dietary perception and lifestyle metrics, and evaluate how dietary perceptions relate to outcomes following Lifestyle Medicine (LSM) program completion. DESIGN: Retrospective review of repository data.

Patient-Reported Quality of Life Predicts First-Cycle Pharmacotherapy Survival in Breast Cancer: A Prospective Cohort Study.

OBJECTIVE: Patient-reported outcomes (PROs) provide direct insight into symptom burden and functional status, yet their utility in early pharmacotherapy outcomes in breast cancer remains underexplored. This study investigated whether baseline PROs from the EORTC QLQ-C30 and BR23 questionnaires are associated with and demonstrate discriminative ability for short-term survival during the first pharmacotherapy cycle in breast cancer patients. METHODS: We conducted a prospective cohort study at two secondary referral hospitals in Indonesia from January to October 2025. Women with breast cancer in

Patterns of alcohol and drug screening in trauma patients: understanding housing status as a determinant of care.

BACKGROUND: Traumatic injury remains a leading cause of morbidity and mortality in the United States, with substance use and social determinants of health playing important roles in injury risk and clinical outcomes. People experiencing homelessness are disproportionately exposed to trauma and are screened for alcohol and drugs at higher rates than housed patients, raising questions about whether differences in substance positivity reflect true variation in exposure or disparities in screening practices. This study examines patterns of alcohol and drug screening and positivity among housed and
