Brain-spine interface: an exploration of a potential future treatment for spinal cord injury.

Spinal cord injuries (SCIs) profoundly impact millions globally, leading to loss of motor and sensory functions below the injury site. Brain-spine interfaces (BSIs) represent an early-stage neuroprosthetic strategy that attempts to restore functional communication between cortical motor-intention signals and spinal sensorimotor circuits below the level of injury. Although early preclinical and highly selected clinical studies have shown encouraging motor outcomes, the evidence remains preliminary, and routine clinical use is limited by questions regarding safety, durability, patient selection,…

Genetic and Molecular Biomarkers for Predicting Outcomes in Traumatic and Nontraumatic Spinal Cord Injury.

Spinal cord injury and degenerative cervical myelopathy cause significant neurologic morbidity with highly variable outcomes that are difficult to predict using conventional clinical and imaging assessments. This review synthesizes evidence from 52 studies on genetic and molecular biomarkers, including neuroaxonal proteins, inflammatory mediators, metabolic markers, and genetic variants. Neurofilament light chain, glial fibrillary acidic protein, and apolipoprotein E ε4 emerged as shared prognostic markers, while etiology-specific signatures reflect distinct pathophysiology. Multimodal panels …

Development and Validation of a Sitting-Balance Confidence Scale for Individuals with Spinal Cord Injury.

Background/Objectives: Participation in sport and daily activities among individuals with spinal cord injury (SCI) is influenced not only by physical capacity but also by psychological factors such as balance confidence. Existing measures primarily assess performance or fear of falling, leaving a gap in evaluating how perceived balance impacts activity choice. This study aimed to (1) develop a Patient-Reported Sitting Balance Confidence (PR-SBC) interview instrument to enable therapists to assess and evaluate non-ambulatory individuals with SCI, (2) incorporate participation-based activities r…

Is MR Imaging of the Cervical Spinal Cord Sufficient for Patients with Suspected Multiple Sclerosis?

PURPOSE: Lesions in the spinal cord (SC) can be found in up to 83% of patients with multiple sclerosis (MS). As they are mainly located in the cervical segment, many centers exclude the thoracic part from SC imaging. The aim of our study was to quantify the decrease in sensitivity resulting from this approach. METHODS: MR images (3T) of 543 consecutive patients with clinically isolated syndrome (CIS) (n: 37) and MS (n: 506) were analyzed retrospectively. Lesions were segmented semi-automatically on axial T2-weighted images of the whole SC using BrainSeg3D. The volume of lesions was related to…

Quantitative Susceptibility Mapping of the Cervical Spinal Cord at 3T and Application to Multiple Sclerosis.

Quantitative susceptibility mapping (QSM) is an advanced MRI technique that links phase variations to the local tissue susceptibility. In multiple sclerosis (MS), QSM has shown promise in characterizing brain lesions by assessing chronic inflammation and myelin content. However, in the spinal cord (SC), the question remains as to whether QSM can classify MS lesions as demyenilated/remyenilated and detect chronic inflammation. SC QSM poses novel challenges due to the small size, mobility and curvature of the cord, as well as magnetic field inhomogeneities caused by nearby vertebrae and physiolo…

Intraoperative Baseline Motor Signals as a Predictor of Recovery in Complete Spinal Cord Injury.

STUDY DESIGN: Retrospective observational study. OBJECTIVE: The purpose of the study was to investigate the association between the presence of baseline sensory and motor intraoperative neuromonitoring and follow-up functional improvement in complete SCI. SUMMARY OF BACKGROUND DATA: During surgery for complete spinal cord injury (SCI), a subset of patients may have present sensory and motor signals at baseline, suggesting that neuronal tracts may be intact as a target for novel therapeutics or even as a prognostic tool for recovery. METHODS: One hundred nineteen complete SCI patients who ha…

Genetic Markers and Mutations in Primary Spinal Cord Tumors and Their Impact on Clinical Management.

Primary spinal cord tumors are rare neoplasms representing 2-4% of central nervous system tumors. Despite their low incidence, their impact on neurological function is profound. Historically, tumor classification and management have relied primarily on histopathology. However, advances in molecular diagnostics have highlighted the critical role of genetic alterations in tumor behavior, prognosis, and treatment response. This narrative review summarizes current evidence on genetic mutations in primary intramedullary spinal cord tumors, focusing on their prognostic value and implications for cli…

The Effect of Cefazolin on the Gut Microbiome of Female Rats After Spinal Cord Injury.

Spinal cord injury (SCI) is a devastating neurological state that could lead to motor, sensory, and autonomic dysfunction. In addition to its direct impact on the central nervous system, SCI exerts systemic effects, including disruption of gut homeostasis and alterations in the gut microbiota, which can contribute to sustained inflammation and hinder functional recovery. While antibiotic administration during the acute phase of SCI is clinically indicated, it may exacerbate microbial dysbiosis. In this study, we investigate the combined effects of SCI and cefazolin treatment on the gut microbi…

Characterisation of MRI Appearance of the Spinal Cord Syrinx in the Whole Spectrum of Chiari Malformation.

Chiari malformations constitute a spectrum of hindbrain anomalies characterized by downward herniation of posterior fossa structures through the foramen magnum and are frequently associated with spinal cord syrinx, a fluid-filled intramedullary cavity that can result in progressive neurological deficits. With the advent of MRI, accurate, non-invasive characterization of Chiari malformations and associated syrinx has become possible, particularly in pediatric populations where early detection is crucial. This hospital-based cross-sectional study was conducted between February 2020 and July 2021…

State-of-the-Art in Robotic Rehabilitation for Spinal Cord Injury Patients: A Literature Review.

Spinal cord injury (SCI) causes severe motor, sensory, and functional impairments, often leading to long-term disability. Conventional rehabilitation is labor-intensive and resource-demanding, whereas robotic rehabilitation enables repetitive, task-specific, and intensive training that promotes neuroplasticity. Evidence from clinical and randomized studies shows that upper limb robotic systems, including end-effector and exoskeleton types, improve motor outcomes, especially in the subacute phase. Moreover, hybrid approaches that integrate functional electrical stimulation or spinal cord stimul…

Can Spinal Cord Stimulation be Considered as a Frontier for Chronic Pain in Diabetic Foot?

Chronic pain in the diabetic foot (DF) is a common complication of diabetes, bringing a significant burden to patients, their families, and even society. There is no very effective treatment for it, traditional treatments such as medication, lumbar sympathetic nerve block, and alternative therapies are often not very effective and have more adverse effects. The emergence of neuromodulation technology has brought new hope for the treatment of DF, among which spinal cord stimulation (SCS) is a hotspot in current research and has achieved remarkable efficacy in the study of DF treatment by blocki…

Management of Pregnancy in Patients With an Implanted Spinal Cord Stimulator: A European Expert Survey.

BACKGROUND: Spinal cord stimulation (SCS) is a surgical treatment for severe chronic neuropathic pain and is a common procedure in women of child-bearing age. Theoretical concerns about the safety of active SCS treatment during pregnancy have led to a widespread practice of recommending deactivation of SCS in pregnant women. The presence of an SCS device also has implications for anesthetic management during labor. MATERIALS AND METHODS: An online questionnaire-based expert survey was conducted among centers in the Nordic countries, Benelux, and Germany, a territory with a population of 140 m…
