Attention Gate Based Dual-Pathway Network for Vertebra Segmentation of X-Ray Spine Images.

Automatic spine and vertebra segmentation from X-ray spine images is a critical and challenging problem in many computer-aid spinal image analysis and disease diagnosis applications. In this paper, a two-stage automatic segmentation framework for spine X-ray images is proposed, which can firstly locate the spine regions (including backbone, sacrum and ilium) in the coarse stage and then identify eighteen vertebrae (i.e., cervical vertebra 7, thoracic vertebra 1-12 and lumbar vertebra 1-5) with isolate and clear boundary in the fine stage. A novel Attention Gate based dual-pathway Network (AGNe…

Outcomes With a Mobile Digital Health Platform for Patients Undergoing Spine Surgery: Retrospective Analysis.

BACKGROUND: Digital health solutions have been shown to enhance outcomes for individuals with chronic medical illnesses, but few have been validated for surgical patients. The digital health platform ManageMySurgery (MMS) has been validated for spine surgery as a feasible method for patients along their surgical journey through in-app education and completion of patient-reported outcomes surveys. OBJECTIVE: The aim of this study is to determine the rates of 90-day emergency room (ER) visits, readmissions, and complications in patients undergoing spine surgery using MMS compared to patients us…

Lumbosacral Transitional Vertebra Contributed to Lumbar Spine Degeneration: An MR Study of Clinical Patients.

We aimed to comprehensively characterize degenerative findings associated with various types of lumbosacral transitional vertebra (LSTV) on magnetic resonance images. Three hundred and fifty patients with LSTV (52.3 ± 10.9 years), including 182 Castellvi type I, 107 type II, 43 type III, and 18 type IV, and 179 controls without LSTV (50.6 ± 13.1 years), were studied. Discs, endplates, and posterior vertebral structures were assessed and compared to those of controls for the most caudal three discs on MRIs. There were no differences in degenerative findings between patients with type I LSTV and

Do Adolescent Idiopathic Scoliosis Patients With Vitamin D Deficiency Have Worse Spine Fusion Outcomes?

BACKGROUND: Prior research has shown that patients with adolescent idiopathic scoliosis (AIS) have a higher prevalence of vitamin D deficien … tive AIS patients were prospectively recruited during routine follow-up visits, 2 to 10 years after spine fusion. Vitamin D levels were measured by serum 25-hydroxyvitamin D (ng/mL). Patients were categorized based on vitamin D level: deficient (<20 ng/mL), insufficient (20 to 29 ng/mL), or sufficient (≥30 ng/mL). The correlation between vitamin D level …

Thoracolumbar surgery for degenerative spine diseases complicated with tethered cord syndrome: A case report.

BACKGROUND: Tethered cord syndrome (TCS) secondary to split cord malformation (SCM) is rare in adulthood. There is as yet no consensus about the optimal treatment method for adult patients with SCMs and degenerative spine diseases such as lumbar stenosis, spondylolisthesis and ossification of the ligamentum flavum (OLF). The tethered cord poses a great challenge to the decompression and fusion procedures for the intraoperative stretching of the spinal cord, which might lead to deteriorated neural deficits. Here, we report on a case to add our treatment experience to the medical literature. CA…

Biportal endoscopic spine surgery for cervical disk herniation: A technical notes and preliminary report.

Biportal endoscopic spine surgery (BESS) for cervical disk herniation (CDH) has been rarely reported. The aim of the article is to describe a novel BESS as a posterior approach for CDH and report the preliminary outcomes and complications. This single-centered retrospective chart review included 109 consecutive patients who underwent BESS for symptomatic single-level CDH. Working and viewing portals were created in each unilateral paravertebral area at the target disk level. Endoscopic exploration allowed for effective and minimally invasive decompression via safe access to the medial foramen …

Spinal Radiology associated with Redundant Nerve Roots of cauda equina in lumbar spine stenosis.

OBJECTIVES: Redundant Nerve Roots (RNR) is a common radiological observation of elongated tortuous appearance of cauda equina nerve roots in sagittal MRI. It is considered a poor prognostic factor, but the associated spinal morphometry remains unfamiliar. METHODS: A retrospective cohort study was conducted at the Aga Khan University Hospital, Pakistan. Patients, aged 18 or above, undergoing decompressive spinal surgery due to degenerative Lumbar Spinal Canal Stenosis (LSCS) in 2015 were included. Patients were divided in groups as per presence of RNR and assessed for spinal morphometric param

Near-miss Thoracic Spine Solitary Plasmacytoma with Neurological Deficit during Pregnancy: A Case Report.

Solitary plasmacytoma (SPC) account for only 5% of plasma cell neoplasms, and the literature hardly reports spinal SPC with a neurological deficit. Furthermore, spinal surgical intervention during pregnancy is rarely encountered and often requires multidisciplinary collaboration and management. The objective of this case report is to highlight this near-miss diagnosis and spinal surgical intervention during pregnancy. A 31-year-old woman with 24 weeks gestation presented with sudden paralysis and incontinence, with an underlying history of chronic backpain over a two-month period. Initially, s

Transient paraplegia in lumbar spine surgery-a potential complication following erector spinae plane block.

PURPOSE: Erector spinae plane block (ESPB) has gained popularity over recent years and is being increasingly used in spine surgery for pain management. To date, no major neurological complications have been reported. We present here two patients who developed transient postoperative paraplegia and discuss the possible causes of this phenomenon. METHODS: Patients, who underwent preoperative bilateral ESPB as an adjunct to general anesthesia for elective lumbar spine surgery at our institution between January 2017 and December 2020, were retrospectively identified. Among them, only patients who…

A deep cascaded segmentation of obstructive sleep apnea-relevant organs from sagittal spine MRI.

PURPOSE: The main purpose of this work was to develop an efficient approach for segmentation of structures that are relevant for diagnosis and treatment of obstructive sleep apnea syndrome (OSAS), namely pharynx, tongue, and soft palate, from mid-sagittal magnetic resonance imaging (MR) data. This framework will be applied to big data acquired within an on-going epidemiological study from a general population. METHODS: A deep cascaded framework for subsequent segmentation of pharynx, tongue, and soft palate is presented. The pharyngeal structure was segmented first, since the airway was clear

Surgery for metastatic epidural spinal cord compression in thoracic spine, anterior or posterior approach?

BACKGROUND: The most commonly encountered tumour of the spine is metastasis, and thoracic spine is the most commonly metastatic spine. Controversy exists regarding the optimal surgical approach for this kind of patient. The author conducted a study to assess the differences between anterior thoracotomy and a posterior approach in patients with malignant epidural cord compression in the thoracic spine. METHODS: Between January 2004 and December 2017, 97 patients with metastatic thoracic lesion were stratified into two groups by approach method to the lesion site: Group A - mean anterior thorac…

Candida Spondylitis Considered as Microbial Substitution After Multiple Surgeries for Postoperative Lumbar Spine Infection.

Candida spondylitis is a relatively rare disease. The primary risk factor is an immunocompromised status. Here, we report an immunocompetent patient who developed Candida spondylitis. The patient was a 70-year-old male. After multiple surgeries, he developed a fever and was diagnosed with chronic pyogenic spondylitis of the lumbar spine, which was treated by long-term antimicrobial therapy. However, his back pain persisted and the inflammatory response was prolonged. We performed posterior thoracolumbar pelvic fixation with a percutaneous pedicle screw system to stabilize the infected vertebra…

Overgrowth After Pediatric Tibial Spine Repair with Symptomatic Leg Length Discrepancy: A Case Report.

CASE: A 12-year-old girl sustained a right-sided tibial spine fracture while jumping on a trampoline. Postoperative course was complicated initially by arthrofibrosis requiring manipulation under anesthesia and subsequent leg length discrepancy attributed to posttraumatic overgrowth necessitating femoral epiphysiodesis. Ten years after initial injury, she reported her knee to be 63% of normal and an International Knee Documentation Committee score of 63.2. Symptomatic overgrowth requiring epiphysiodesis after tibial spine repair has not been previously reported to our knowledge. CONCLUSION: T…

Blood preservation techniques in complex spine surgery: Illustrative case and review of therapeutic options.

BACKGROUND: Complex spine surgery predisposes patients to substantial levels of blood loss, which can increase the risk of surgical morbidity and mortality. CASE DESCRIPTION: A 29-year-old achondroplastic male required thoracolumbar deformity correction. However, he refused potential allogeneic blood transfusions for religious reasons. He, therefore, underwent pre-operative autologous blood donation and consented to the use of the intraoperative cell salvage device. Immediately prior to the incision, he underwent acute normovolemic hemodilution. Throughout the case, we additionally utilized m…

Incidental Finding Prevalences in 3-Tesla Brain and Spine MRI of Military Pilot Applicants.

INTRODUCTION: Incidental findings in brain and spine MRI are common. In aerospace medicine, pilot selection may be affected by improved sensitivity of modern MRI devices. We investigated the occurrence of medically unfit rates caused by incidental findings in military pilot applicants using a 3-Tesla scanner as compared to the outcomes of a lower field strength 1-Tesla device based on similar screening protocols.METHODS: A total of 3315 military pilot applicants were assessed by a standardized German Air Force Imaging Screening Protocol and retrospectively subdivided into two cohorts, one of w…
