Several factors influencing the colonization of group B streptococci--rectum probably the main reservoir.

Women (1 268) attending a gynaecological clinic or a venereal disease clinic had positive cultures of group B streptococci from urethra in 27.8% and from rectum in 29.4%. The colonization rate was highest in patients from the venereal disease clinic (p less than 0.001). In a study of 405 of the females at different parts of the menstrual cycle group B streptococci were detected during the last half of the cycle in specimens from cervix in 18%, from urethra in 29% (p less than 0.005), and from rectum in 35% (p less than 0.001). The correlation between contraceptive methods and isolation of grou

[Colonization of the pregnant woman and the newborn by group B streptococcus: therapeutic and preventive implications].

In a search for group B streptococci, vagina and urethra of 196 pregnant women entering the delivery room were cultured using a selective medium. Sixteen (8.2%) were positive. Fifteen (15%) of 101 nonpregnant women cultured similarly during a clinical consultation were positive. Esogastric aspiration fluid of 199 neonates from the former group of women was also cultured: 8 (4.0%) were positive. Thus, maternal genital colonization was accompanied by neonatal contamination in 7 of 16 cases (43%). Typing of the streptococci revealed identity between all maternal and neonatal isolates tested. Thes

Group a beta-hemolytic streptococcal pneumonia: clinical course and complications of management.

Three childhood cases of beta-hemolytic streptococcal pneumonia are presented with a review of the literature. The disease, though uncommon in childhood, results in severe local and systemic effects. Hematologic fluid and electrolyte problems are frequent. Pericardial disease and secondary bacterial invasion are added dangers encountered in management. Early and vigorous drainage of the pleural cavity is essential to successful outcome. It appears that predisposing viral disease, including influenza, is essential for acquisition of this form of bacterial pneumonia.

Acute gangrenous colitis proximal to obstructive cancer of the sigmoid colon.

Two cases of massive gangrenous inflammation of the colon proximal to obstructive carcinomatous lesions, the so-called obstructive colitis, are reported. Because of the rarity of this condition, the etiological factors responsible for this entity are not yet clear. The detailed pathological and microbiological studies at the time of operation are essential to their further clarification.

[Role of anaerobic streptococci in human pathology. Report I. Etiological significance of anaerobic streptococci in chronic tonsillitis].

The authors describe a method of isolation of anaerobic streptococci from the blood. A high seeding efficiency of anaerobic streptococci from the depth of the tonsils (75.1%) and from their surface (59.7%) and also from the blood (36%) pointed to the leading role of these microbes in the etiopathogenesis of chronic tonsillitis. In healthy individuals anaerobic streptococci are revealed (by culture method) on the surface of the tonsils but rarely (6%). Str. micros are the most incident in chronic tonsillitis (in 85.3% of cases-from the depth of the tonsils and in 77.5%-from the surface), wherea

[Septic endocarditis in cardiosurgical clinical practice].

The authors analyze the experience of the treatment of eight patients with primary acute septic endocarditis (SE), 35 patients with rheumatic heart disease complicated by secondary subacute SE, 68 patients with early postsurgery SE including 25 who had had closed mitral comissurotomy, and 43 with prosthetic SE. Surgical treatment consisting in the removal of the infection source (the impaired valve or prosthesis) and the implantation of a new prosthesis should be carried out before the development of irreversible alterations of the internal organs.

Bacterial and candidal cutaneous infections in the neonate.

The skin and oral cavity of the neonate are colonized by a variety of organisms during the first few days of life. Some of these organisms constitute normal flora, but others are true pathogens or are capable of pathogenicity when host or environmental factors are altered in their favor. Cutaneous bacterial and yeast infections that afflict the newborn are discussed in terms of clinical presentation, laboratory evaluation, and appropriate management.

The effect of prophylactic tinidazole on the anaerobic vaginal flora in patients undergoing gynaecological surgery.

The effects of prophylactic tinidazole on the vaginal carrier state of anaerobic bacteria and the development of postoperative infections were studied in 50 patients undergoing gynaecological surgery. The results are based on a double-blind trial, using tinidazole (Fasigyn; Pfizer) and placebo. Although no postoperative infections other than urinary tract infections were observed in our series, this study shows conclusively that the isolation of anerobic bacteria from the vagina decreased markedly in the tinidazole group compared with the placebo group.

Early splenomegaly in homozygous sickle-cell disease: An indicator of susceptibility to infection.

135 children with homozygous sickle-cell (SS) disease diagnosed at birth have been followed for 1--5 years. Severe bacterial infections were confined to those in whom the spleen was first palpable at or before 1 year of age and were commonest in those in whom the spleen was first palpable at or before age 6 months. Regular follow-up of children with SS disease diagnosed at birth will identify children particularly at risk of severe infections.

Maternal body mass index and the risk of early-onset Group B Streptococcus disease in newborns: A systematic review and meta-analysis.

BACKGROUND: Early-onset group B Streptococcus disease (EOGBS) remains a leading cause of neonatal morbidity and mortality. The incidence can be substantially reduced by intrapartum antibiotic prophylaxis in women with defined risk factors. However, the role of high prepregnancy body mass index (BMI) as a risk factor remains unclear. This systematic review and meta-analysis therefore aimed to evaluate the association between maternal BMI and the risk of EOGBS, as well as related proxy outcomes, including intrapartum vaginal GBS colonization and rectovaginal or urinary GBS colonization before te
