BACKGROUND: Rotator cuff tears involving the subscapularis are less common than those involving the superior aspect of the rotator cuff. The purpose of the present study was to report the results of repair of isolated tears of the subscapularis.
METHODS: The records on eighty-four shoulders that had undergone open repair of the subscapularis tendon were reviewed. The mean age of the patients at the time of surgery was 53.2 years. The mean interval from the onset of symptoms to the time of surgery was 12.5 months. Fifty-seven tears were traumatic, and twenty-seven were degenerative. Twenty-thr

OBJECTIVE: Hamstring strains are one of the most common muscle strains in athletes; however, complete rupture of the proximal hamstring origin is rare and results from significant trauma. The objective of this paper is to present our experience of management of complete ruptures where surgical repair resulted in good results in both acute and delayed cases.
METHODS: Two water skiers and two bull riders sustained complete rupture of the proximal origin of the hamstring muscles. All underwent repair of the hamstring origin and sciatic nerve neurolysis. A post operative hamstring rehabilitation

Optimal magnetic resonance (MR) imaging of the wrist requires a high field strength magnet and a dedicated wrist coil to achieve high-resolution images. Using current MR sequences, detailed images of articular cartilage and the supporting ligaments and tendons can be obtained. Evaluation of the triangular fibrocartilage as well as the extrinsic and intrinsic ligaments of the wrist is possible with thin-slice three-dimensional volumetric gradient recalled sequences. Fast inversion recovery sequences, used to achieve fat suppression in peripheral joints such as the wrist, allow for detection of

PURPOSE OF THE STUDY: The anterior or subcoracoïd impingement is often mentioned but remains unprecise as far as clinical, radiological or even anatomical lesion are concerned. The purpose of our work was to study the different factors influencing the subcoracoïd space in case of cuff tear.
METHODS: Our study was based on 206 shoulders operated for full-thickness rotator cuff tear. The SubCoracoïd Space (SPS), measured in millimeters on pre-operative arthro-CT-scan, was defined by the shortest distance between the coracoïd process and the humeral head. Muscular statement of the rotator cuff c

Experience with flexor tendon repairs has suggested the superiority of the augmented Becker (MGH) technique for strength, toughness, and gap resistance. In an effort to apply these findings to the extensor tendons, 3 four-strand extensor tendon repair techniques were biomechanically tested in fresh human cadaver limbs: modified Bunnell, modified Krackow-Thomas, and MGH. Repairs were performed in Verdan's zone VI. Repaired tendons were distracted at constant speed until rupture. Tendon load and tendon distraction were continuously monitored. Benchmark values for load were measured as fingers we

Magnetic resonance arthrography is superior to other imaging techniques in evaluation of the glenohumeral joint. Normal variants that can be diagnostic pitfalls include the anterosuperior sublabral foramen, the Buford complex, and hyaline cartilage under the labrum. Anteroinferior dislocation is the most frequent cause of anterior glenohumeral instability and produces a constellation of lesions (anteroinferior labral tear, classic and osseous Bankart lesions, Hill-Sachs lesion). Variants of anteroinferior labral tears include anterior labroligamentous periosteal sleeve avulsion and glenoid lab

OBJECTIVE: The purpose of this article is to describe the MR imaging appearance of seven cases of posterior tibial tendon dislocation and subluxation.
CONCLUSION: Posterior tibial tendon dislocation is a rare but important entity usually related to a previous traumatic event. The clinical diagnosis is often missed because of its rarity. MR imaging provides important clues to the diagnosis and preoperative evaluation of this condition.

The medial intraarticular dislocation of the biceps tendon of the shoulder is a rare pathologic condition and its diagnosis before surgery is very important to properly repair the tears of rotator cuff tendons, the rotator cuff being nearly always involved. We report our experience in 8 patients (1.8%) with biceps tendon dislocation observed in a series of 432 patients submitted to MR studies to address several diagnostic issues. The MR patterns of biceps tendon dislocation and the related physiopathologic mechanisms are discussed. The MR images were retrospectively reviewed by two of the auth

Recent developments in the understanding of tendon healing and the effects of early motion have led to early controlled motion programs (ECM) for lacerated extensor tendons. The purpose of this study was to determine whether there were differences in length of treatment and final results when patients who had been treated with ECM with dynamic splinting were compared with patients who had been treated with traditional static extension splinting (SES) for zone III and zone IV extensor tendon lacerations. A retrospective study of patient charts from June 1984 through January 1990 was conducted.

We report the results of examination of patients before primary and secondary unilateral digital nerve repair. Two-point discrimination was uniformly recorded. The results show that significant overlap of adjacent digital nerves is common and this has important implications for interpretation of results after repair. Recommendations are made regarding the evaluation of outcome following surgery.

The position of the patella was studied prospectively in both knees of 45 consecutive patients (21 male and 24 female patients aged 16-48 years who were competitive [n = 17] or recreational [n = 28] athletes) who had unilateral patellofemoral pain syndrome without symptoms or signs of patellar instability at initial examination. In each knee, standardized anteroposterior, lateral, and tangential radiographs were obtained and six indexes of patellar position (the ratio of the patellar tendon to the greatest diagonal length of the patella, sulcus angles, lateral patellofemoral angle, lateral pat

In the period from 1983 to 1990 operations were conducted on 63 patients with postburn boutonnière contracture of 116 fingers. In 19 patients (42 fingers) tendoplasty of the extension apparatus of the fingers was carried out with simultaneous restoration of the dermatoadipose layer on the dorsal surface of the hand with an inguinal or dermatoadipose graft. In 39 patients the tendon of the extensor digitorum muscle was injured over several proximal interphalangeal articulations. A new method for treating boutonnière contracture is suggested. It is based on restoration of the continuity of the c

Extensor tendons ruptured in 12 patients as a result of osteoarthritis of the distal radio-ulnar joint. Rupture occurred without warning in ten cases and was sequential in five. Perforation of the dorsal capsule of the distal radio-ulnar joint, allowing contact between the roughened ulnar head and extensor tendons, was present in every case. The capsular performation was demonstrated by arthrography, which may be used to identify patients who are at risk of extensor tendon rupture. Loss of independent extension of the little finger is a valuable clinical sign because rupture of extensor digiti

A careful physical examination is absolutely necessary as a first evaluation of all disorders of the shoulder before performing any imaging technique that must be chosen according to the kind of suspected pathology. The best indications for performing an arthroscanner are the intraarticular lesions such as injuries of the glenoid labrum and of the articular capsule. Echography is the most performing technique for bicipital and rotator cuff tendinitis. CT scan helps to understand the antero-internal retro-coracoidal impingment syndromes as well as uninterpretable bone lesions at standard radiog

A case with a severe gun shot wound of the palmar ulnar distal forearm and wrist is presented. The initial treatment included reconstruction of the ulnar nerve by means of nerve grafts and soft tissue coverage using a free microvascular groin flap. This represents a procedure which is certainly applied only very exceptionally at our department. Inspite of consequent complete flap necrosis due to arterial thrombosis, sensory recovery was sufficient for protective sensibility. The probable reasons of arterial thrombosis are discussed.

Thirty-three consecutive patients with complete ruptures of the calcaneal tendon were treated by external fixation. The patients were assessed both objectively and subjectively, and the results classified as excellent, good, fair and poor. In 30 patients the result was excellent or good. There were no infections or re-ruptures. Two patients with fair results had sural nerve injury. The only patient with a poor result had Sudeck's atrophy. It is felt that this operation satisfies the need for a new technique which is simple and combines the advantages of both surgical and non-surgical treatment

Five cases are presented of bilateral simultaneous rupture of the quadriceps tendon and the English literature of six isolated case reports is reviewed. All the patients were men, and most were over 50 years old. The injury often happens in elderly people and there is diagnostic confusion with other causes of inability to use the legs, notably with mild strokes. In three of our five cases there was a delay in diagnosis. The cardinal features are diffuse swelling around the knee, a visible or palpable suprapatellar defect and the inability to lift the straight leg despite a functioning quadrice
