The "lateral gutter drive-through" sign: an arthroscopic indicator of acute femoral avulsion of the popliteus tendon in knee joints.

Acute avulsions of the popliteus tendon and the lateral collateral ligament of the femur (peel-off lesion) are common and can both be missed during preoperative assessment and later overlooked during surgery if not specifically sought out. Arthroscopic observation of the direct avulsion sign of the torn end of the popliteus tendon occurs less frequently in patients with acute and subacute injuries. This study describes an arthroscopic sign that detects these tears when they either involve only the popliteal tendon or include the lateral collateral ligament assessed from the lateral gutter of t

Effects of posterior tibial tendon augmented with biografts and calcaneal osteotomy in stage II adult-acquired flatfoot deformity.

Adult-acquired flatfoot deformity (AAFD) is a well-known condition leading to flexible flatfoot deformity. However, only recently have the function and muscle strength for balancing opposing muscles been more appreciated in laboratory studies. With the advancements in collagen science in tendon structure, the rationale and concept of some of the most common procedures in tendon transfer have been challenged. The current availability of biograft technology has provided an alternative in augmentation procedures instead of sacrificing autologous tendons. This novel approach may offer a viable opt…

The effect of donor age and low-dose gamma irradiation on the initial biomechanical properties of human tibialis tendon allografts.

BACKGROUND: Most tissue banks recover and irradiate tibialis tendon allografts from donors aged up to 65 years. It is unknown whether donor … (<45 years), middle (46-55 years), and old (56-65 years). Within each age group, half of the paired tendons underwent tensile testing as single-strand grafts and the other half as double-strand grafts. One tendon from each donor pair was randomly assigned to undergo terminal sterilization with an absorbed dose of 1.46 to 1.80 Mrad (14.6-18.0 kGy) gamma ir …

Influence of psychomotor skills and innervation patterns on results of latissimus dorsi tendon transfer for irreparable rotator cuff tears.

This investigation was performed to analyze the influence of innervation and psychomotor skills on the outcome of latissimus dorsi transfer. Patients with the 10 best and 10 worst results after latissimus dorsi transfer for irreparable rotator cuff tears were selected. All patients meeting the inclusion criteria (n = 12) were subject to a psychomotor test battery (Motorische Leistungsserie) and electromyographic innervation assessment. There was no statistical difference between the 2 groups preoperatively in terms of the commonly tested factors known to influence the results of this procedure

Tendon-bone insertion repair and regeneration using polyglycolic acid sheet in the rabbit rotator cuff injury model.

BACKGROUND: The quality of tendons has considerable limitations regarding torn rotator cuff tendons. Tissue-engineering techniques using a biodegradable scaffold offer potential alternatives for recreating a valid tendon-to-bone interface. HYPOTHESIS: A polyglycolic acid (PGA) sheet could facilitate the regeneration of the rotator cuff tendon insertion in vivo. STUDY DESIGN: Controlled laboratory study. METHODS: An implant consisting of a PGA sheet, a rapidly absorbable material, was used to replace a completely resected infra-spinatus tendon insertion in 33 adult Japanese white rabbits. Th…

Extensor indicis-transfer compared with palmaris longus transplantation in reconstruction of extensor pollicis longus tendon: a retrospective study.

Either an extensor indicis transposition (EIT) or an intercalated free tendon graft (FTG) can be used for secondary reconstruction of the extensor pollicis longus (EPL) tendon. We reviewed 1469 cases of extensor tendons repaired between 1992 and 2003 and compared the results. In only 55 patients was an isolated secondary reconstruction of the EPL done. Forty-five patients (82%) were available for clinical follow-up after a mean of 4.3 (range 2-11) years (28 (62%) who had EIT and 17 (38%) who had FTG). Thumb function was assessed postoperatively using Geldmacher's criteria, and the uninjured th…

Transient bone loss of distal radius and ulna following clean-cut tendon injuries, repair and passive mobilisation.

The aim of this study was to investigate whether there is any significant bone loss of the ulna and radius following acute tendon-artery-nerve clean-cut injuries at the wrist level which were repaired and rehabilitated by early passive mobilisation. Fifty-eight patients who underwent such operation were enrolled in this study. Patients in Group I (n=28) had primary tendon repairs alone, in Group II (n=15) primary tendon and nerve repairs and in Group III (n=15) primary tendon, nerve and artery repairs. Bone mineral density (BMD) measurements of the ulna and radius were obtained during the firs

Stress distribution in the supraspinatus tendon with partial-thickness tears: an analysis using two-dimensional finite element model.

The purpose of this study was to clarify the stress distribution in the rotator cuff tendon with 3 types of partial-thickness tears by use of 2-dimensional finite element analysis. In the finite element model of the normal human shoulder, a tendon defect was created on the articular surface, on the bursal surface, or in the midsubstance close to the insertion. A tensile force was applied to the proximal end of the tendon, and the stress distribution was calculated. In all 3 types of tears, a high stress concentration appeared around the articular surface at the insertion and at the site of the

Distal biceps tendon rupture: biomechanical analysis of repair strength of the Bio-Tenodesis screw versus suture anchors.

The purpose of this study was to compare the strength and stiffness of distal biceps tendon ruptures repaired with either a Bio-Tenodesis screw or suture anchor technique in a cadaveric model. Ten matched pairs of cadaveric arms underwent simulated distal biceps tendon ruptures and were repaired with either a Bio-Tenodesis screw or suture anchor technique. We analyzed pullout strength, stiffness, and mode of failure for each construct. The mean pullout strength of the repair with a Bio-Tenodesis screw was significantly higher (192 N [SD, 42.5 N] vs 147 N [SD, 29.5 N], P < .013). Use of the Bio

[Spontaneous bilateral rupture of the Achilles tendon in an elderly woman undergoing prednisone pulse therapy with a history of polymyalgia rheumatica].

An 83-year-old woman presented at the neurology out-patient clinic with acute bilateral weakness of the calf muscles that had lasted for a few weeks. Ultrasound and MRI evaluation of the Achilles tendons revealed bilateral ruptures. Possible predisposing factors included treatment with prednisone pulse therapy for obstructive pulmonary disease and prior polymyalgia rheumatica. Surgical reconstruction of the tendons resulted in a major clinical improvement. Rupture of the Achilles tendons can occur spontaneously, and sometimes bilaterally. A predisposing factor is present in nearly every case o…

Bilateral patellar tendon rupture without predisposing systemic disease or steroid use: a case report and review of the literature.

Simultaneous bilateral patella tendon ruptures are very rare injuries of the knee extensor complex often associated with systemic disorders such as lupus erythematosus or rheumatoid arthritis. We describe the case of a 34-year-old man without concomitant systemic disease or steroid use and provide the most comprehensive review of the German and English literature. Furthermore, we discuss the predisposing factors and causal mechanisms as well as current diagnostic procedures and treatment options. In the literature review, only a few patients without systemic disorder or steroid medication pres

Patient-oriented functional outcome after repair of distal biceps tendon ruptures using a single-incision technique.

Using a patient-oriented outcome questionnaire, in addition to standard outcome measures, we sought to determine the outcome of patients who had repair of a complete rupture of the distal biceps tendon via a single anterior incision technique with suture anchors. We identified 62 patients who were treated operatively by a single surgeon over an 8-year period for a diagnosis of complete rupture of the distal biceps tendon. Of the patients, 9 could not be located for final follow-up, and 53 of 62 (85%) participated in the study. All patients were men, and their mean age was 42 years. All repairs
