Remodeling of adjacent spinal alignments following cervical arthroplasty and anterior discectomy and fusion.

OBJECTIVE: To evaluate the effects of cervical artificial disc replacement (ADR) and anterior discectomy and fusion (ACDF) on adjacent spinal alignments. METHODS: The cohort consisted of 33 patients who undergone single-level cervical ADR (15 patients) and ACDF (18 patients) for radiculopathy, who had not had any previous spine surgery, and who had a minimum follow-up of 2 years. Whole-spine lateral radiographs were taken at the pre-operative and follow-up consultations. Cervical lordosis, thoracic kyphosis, lumbar lordosis, and sagittal balance were measured each time. The patients filled ou

Accuracy of free-hand pedicle screws in the thoracic and lumbar spine: analysis of 6816 consecutive screws.

BACKGROUND: Pedicle screws are used to stabilize all 3 columns of the spine, but can be technically demanding to place. Although intraoperative fluoroscopy and stereotactic-guided techniques slightly increase placement accuracy, they are also associated with increased radiation exposure to patient and surgeon as well as increased operative time. OBJECTIVE: To describe and critically evaluate our 7-year institutional experience with placement of pedicle screws in the thoracic and lumbar spine using a free-hand technique. METHODS: We retrospectively reviewed records of all patients undergoing

Endovascular management of chronic aortic dissection in patients with Marfan syndrome.

INTRODUCTION: Marfan syndrome patients are prone to aortic dilatation, dissection, and rupture. Success of aortic root replacement has generated a cohort of patients surviving longer and presenting with distal aortic dissection and enlargement. Thoracic endovascular stent-graft repair (TEVR) is being increasingly utilized to exclude aneurysms resulting from chronic aortic dissection. This report explores the role of TEVR in Marfan patients with this pathology. METHODS: Review of a prospectively maintained database identified seven patients with Marfan syndrome offered endovascular repair of a

Descending thoracic aortic rupture: role of endovascular stent-grafting.

BACKGROUND: The mortality of patients with descending thoracic aortic rupture who are treated by conventional surgery is high. Our current strategy for the management of descending thoracic aortic rupture is to treat seriously ill patients with endovascular stent-grafting using handmade grafts, and to treat other patients with traditional open repair. The aim of this study was to assess the early results of our strategy. METHODS: Twenty-nine consecutive patients with descending thoracic aortic rupture were referred to Sapporo Medical University Hospital from June 2001 to January 2004. Eightee

Aberrant Drainage to the Right Upper Pulmonary Vein From the Right Middle or Lower Lung: How Common?

BACKGROUND: Anatomical variations in pulmonary vein (PV) drainage are critical considerations during right upper or middle lobectomy, as unrecognized aberrant veins can increase surgical risks. Although such variations have been described in anatomical and radiologic studies, surgically verified data under a standardized thoracoscopic approach remain limited. This study investigates the prevalence and patterns of aberrant venous drainage from the right middle lobe (RML) and right lower lobe (RLL) into the right upper pulmonary vein (RUPV). METHODS: A total of 213 patients undergoing right upp

Postoperative anastomotic embolism and elusive chronic nausea following pulmonary artery tear during single-port thoracoscopic lobectomy: a case report and examination of potential mechanisms.

This paper details the diagnosis and treatment of a 66-year-old male patient with invasive adenocarcinoma in the left upper lobe of the lung, who experienced a pulmonary artery injury during a single-port thoracoscopic lobectomy. During surgery, the basal trunk artery’s root was torn due to enlarged lymph nodes encircling the pulmonary artery, necessitating an urgent conversion to thoracotomy for end-to-end anastomosis and reconstruction of the artery. Eighteen hours post-operation, the patient unexpectedly developed atrial fibrillation. A CTA scan confirmed an embolism in the left pulmonary b

Minimally Invasive Techniques for Intercostal Nerve Transfer: A Technical Note on Thoracoscopic and Robotic Approaches.

BACKGROUND AND OBJECTIVES: Brachial plexus injuries are a devastating and often life-altering event. For patients with flail limb and no intraplexal donors, intercostal (IC) to musculocutaneous nerve transfer allows for restoration of elbow flexion although the surgery can be long with a painful recovery. In this article, we demonstrate a minimal incision video-assisted thoracoscopic surgery (VATS) method and a robotic approach to IC harvesting for nerve to biceps brachii reanimation. METHODS: This study was performed on cadaveric donors. We initially used the VATS technique and placed 3 thor

Computed tomography-based thymoma-to-cranial intrathoracic volume ratio predicts the feasibility of video-assisted thoracoscopic surgery in a small dog: A case report.

BACKGROUND: Canine thymoma (CT) is the most common neoplastic disease of the cranial intrathoracic cavity. Reports of video-assisted thoracoscopic surgery-thymectomy (VATS-T) in dogs are increasing, but the surgical indication criteria remain unclear. This report highlights the value of a new criterion called the thymoma volume to cranial intrathoracic volume ratio (T/CI ratio) for evaluating the feasibility of VATS-T. CASE DESCRIPTION: A neutered 9-year-old Papillon weighing 3.5 kg had a mass of approximately 20 mm in the cranial intrathoracic cavity. The dog was in good general condition an

Robot-assisted vs. video-assisted thoracoscopic surgery in the question of radicality of mediastinal lymphadenectomy.

Mediastinal lymphadenectomy plays a crucial role in the surgical treatment of -non-small cell lung cancer (NSCLC), as accurate staging significantly influences subsequent therapy. Robot-assisted thoracoscopic surgery (RATS) has emerged over the past decade as an advanced minimally invasive approach with the potential to achieve high oncological radicality. This review explores the outcomes of mediastinal lymphadenectomy performed using the RATS technique and compares them with those of video-assisted thoracoscopic surgery (VATS). In most studies, a higher, at least comparable, total number of

Birt-Hogg-Dube syndrome: A case report and case study of primary spontaneous pneumothorax caused by folliculin gene mutation.

RATIONALE: Birt-Hogg-Dube (BHD) syndrome is a rare autosomal dominant disorder characterized by a triad of manifestations, including recurrent spontaneous pneumothorax, multiple cutaneous fibrofolliculomas, and renal neoplasms. Familial clustering of spontaneous pneumothorax should raise clinical suspicion of BHD syndrome. This syndrome is caused by pathogenic variants of the folliculin gene (FLCN gene) encoding folliculin, which predisposes to pneumothorax through dysregulated matrix metalloproteinase activity, leading to elastic fiber degradation in the pulmonary parenchyma. PATIENT CONCERN

[Management of Malignant Pleural Effusion].

Malignant pleural effusion is a common diagnosis in metastasized cancers. It is always of palliative character. Main symptoms are dyspnoea and reduced quality of life. Diagnosis is made by ultrasound-guided puncture of the pleural effusion (cytology) and often video-assisted thoracic surgery with biopsy of the pleural surface (histology). The goal of treatment is a fast, sustainable, minimally invasive, patient-centred therapy that increases quality of life. Besides systemic therapy and best supportive care the patient can be treated with local therapy including either pleurodesis (via drainag…

Pulmonary sequestration associated with pulmonary actinomycosis: A case report and literature review.

RATIONALE: Pulmonary sequestration (PS), a rare pulmonary disease, arises from congenital pulmonary vascular dysplasia. Meanwhile, pulmonary actinomycosis is a purulent infection of lung lesions triggered by the inhalation of actinomycetes, which is also uncommon. Even rarer is the occurrence of pulmonary actinomycete infection secondary to PS. Herein, we present a case report of such a rare occurrence. PATIENT CONCERNS: The 21-year-old female patient had been erroneously diagnosed with pneumonia and tuberculosis, presenting symptoms of cough, sputum, and hemoptysis; however, the implemented
