Outcome in pediatric rheumatic disease.

Outcome assessment has been a focus of recent research in rheumatic diseases and in pediatric rheumatology. The establishment of a preliminary core set of outcome variables for juvenile rheumatoid arthritis patients and a preliminary definition of improvement using these variables have been important steps toward standardization of outcome assessment. A population-based epidemiologic study has added to our knowledge of long-term outcome in juvenile arthritis and confirms the chronicity of disease that was found in previous studies. Standardization of outcome assessment in pediatric systemic lu

[Hypothermic effects on uveal blood flow and postoperative inflammation in vitrectomy].

Hypothermic effects on uveal blood flow and postoperative inflammation were evaluated in experimental vitrectomy on albino rabbits. Solutions used for intraocular perfusion were maintained at 9 degrees C and 22 degrees C. Following the vitrectomy, the rabbits' intraocular spaces were irrigated for 60 minutes. The temperature at various sites and the uveal blood flow were measured before and during the procedure. Aqueous protein concentrations were checked on postoperative days 1, 7, and 14. Another solution maintained at 37 degrees C was used as a control. There was a larger decrease in temper

Cyclosporin in Behçet's disease: results in 16 patients after 24 months of therapy.

The aim of this study was to evaluate the usefulness of cyclosporin in Behçet's disease. Sixteen subjects (10 males and 6 females; mean age 25.2 years) affected by the complete type of Behçet's syndrome received 5 mg/kg/day of cyclosporin for 24 months. A marked improvement of the symptoms was observed after three months of therapy. Within 6 to 12 months of treatment 14 of the 16 patients obtained a complete clinical remission. Biochemical and immunological parameters were controlled periodically to evaluate the clinical response and the possible side-effects. Two patients dropped out of the s

Behçet's disease with slowly enlarging midbrain mass on MRI: resolution following steroid therapy.

We describe a case of Behçet's disease with a slowly enlarging midbrain mass on magnetic resonance imaging, which resolved after 4 months of oral steroids.

Effect of Newcastle disease virus on ocular and paraocular tissues in experimentally inoculated chickens.

The effects of a mesogenic strain of Newcastle disease virus on ocular and paraocular structures were studied in 10-to-12-week-old chickens inoculated conjunctivally, intraocularly, intracerebrally, or intravenously. Paraffin-embedded tissues were examined by light and fluorescent microscopy using conventional staining and immunohistochemistry. Lesions were most severe in intraocularly inoculated chickens, where a marked iridocyclochoroiditis was evident from 8-12 hours up to 21 days postinfection. Viral antigens were detected in the iris, ciliary body, Schlemm's canal, and occasionally the le

Pars plana lensectomy and vitrectomy for complicated cataracts in juvenile rheumatoid arthritis.

Anterior surgical approaches to cataracts in juvenile rheumatoid arthritis (JRA) patients are compromised by chronic anterior and posterior uveitis, miotic pupils, and posterior synechiae. The authors retrospectively reviewed the results of pars plana lensectomy and vitrectomy in ten eyes from seven JRA patients with cataracts associated with chronic iridocyclitis and vitritis. Visual acuities improved from 20/100 or worse preoperatively to 20/60 or better postoperatively. Transient postoperative hypotony lasting 1 week to 4 months occurred in four of the ten eyes. Patients with glaucoma conti…

Rational acyclovir therapy in herpetic eye disease.

Acyclovir has been widely used against the various manifestations of eye disease due to herpes simplex since it first became generally available in the UK nearly five years ago. This paper discusses the rational indications for its use, through considerations of its pharmacology and pharmacokinetics, and through results of the many clinical trials that have been carried out to investigate its effects since its clinical efficacy was first demonstrated in 1979.

[Secondary forms of glaucoma].

The various forms of secondary glaucoma are discussed on the basis of their different pathogenesis. Elevation of IOP is most frequently associated with inflammation of the uveal tract. Vascular changes in the uvea, in particular rubeosis iridis but also angiomatoses of the retina, can cause secondary glaucoma. Tumors of the uvea and retina often lead to increased intraocular pressure; and lastly, blunt and perforating injuries are frequently followed by glaucoma. Treatment of all these different forms of secondary glaucoma is discussed.

Conjunctival lesions in periarteritis nodosa. A clinical and immunopathologic study.

Periarteritis nodosa is a disease with protean manifestations, yet there are few reports of ocular involvement. We treated a patient who was seen with conjunctival and anterior uveal involvement. The clinical histopathologic and immunopathologic characteristics are described. Immunopathologic studies are a useful adjunct in confirming the nature of this disorder.

Lacrimal gland enlargement in familial sarcoidosis.

Familial sarcoidosis is rare. It is unusual for lacrimal gland enlargement to be the initial manifestation of sarcoidosis. Two sisters are described with lacrimal gland enlargement as the initial manifestation of sarcoidosis. Gallium-67 scan was a useful adjunct to lacrimal biopsy in sarcoidosis. Sarcoid dacryoadenitis responded dramatically to a short term course of low dose prednisone.

Endothelial cell loss in herpes zoster keratouveitis.

Fourteen patients were followed up with slit-lamp, noncontact, and wide-field specular microscopical examination for anterior segment changes during unilateral attacks of keratouveitis due to varicella zoster virus (VZV). Ten patients had severe keratouveitis. Their affected eyes presented nonreflecting endothelial changes in different phases of the disease. The first non-reflecting changes suggesting virus endothelitis were observed at the onset of keratouveitis. These changes later recurred for several months. When the corneal oedema had subsided, the mean endothelial cell density of the aff…

Quantification of ocular inflammation: evaluation of polymorphonuclear leucocyte infiltration by measuring myeloperoxidase activity.

Myeloperoxidase (MPO) activity was measured in rabbit cornea and iris-ciliary body to quantitate the infiltration and accumulation of polymorphonuclear leucocytes (PMN's) following an inflammatory stimulus. Following injection of clove oil into the cornea, MPO activity could be detected in the cornea at 6 hr, reaching a maximum at 12 hr, and falling to non-detectable levels at 72 hr. MPO activity was only detected in the iris-ciliary body 24 hr after intracorneal clove oil injection. MPO activity in the iris-ciliary body increased in a dose-dependent manner following intravitreal injection of

Herpetic eye disease in rabbits after inoculation of autonomic ganglia.

Since herpes simplex virus (HSV) can cause persistent infection of autonomic ganglia of both humans and experimentally infected animals, we followed the pattern of eye disease and viral growth after HSV inoculation of one superior cervical ganglion in rabbits. Of 27 inoculated animals, eye disease or detectable virus developed in 18. Anterior uveitis was the most common clinical manifestation (94%), but conjunctivitis and dendritic keratitis were also frequent (60%). All 12 uveal-retinal specimens tested and five of seven ipsilateral superior cervical ganglia had detectable virus. If recurrent…

Intraocular coccidioidomycosis.

A case of unsuccessfully treated coccidioidomycosis with intraocular manifestations is presented. Fever, skin lesions and a variety of constitutional symptoms dominated the clinical course. At autopsy there were extensive systemic dissemination and conspicuous intraocular lesions involving the uveal tract. The ocular and systemic features and the immunologic aspects of Coccidioidomycosis are discussed. Previously reported intraocular cases are reviewed and compared to our case.
